    Abstract:

    Although considerable evidence suggests that Leisure-Time Physical Activity is associated with a reduced risk of endometrial cancer (EC), the shape of dose-response relationship has not been investigated and previous meta-analyses have not accounted for differences in measures of Physical Activity. To address such issues, we conducted linear and nonlinear dose-response meta-analyses by metabolic equivalent of task (MET)-hour/week and hour/week, respectively, based on observational studies published up to September 2013 identified from PubMed and Embase databases. Summary relative risks (RRs) and 95% confidence intervals (CIs) were calculated using a random-effects model. In the linear dose-response analysis, an increase in Leisure-Time Physical Activity by 3 MET-hour/week was associated with an ∼2% reduced risk of EC (summary RR = 0.98, p = 0.02, 95% CI = 0.95-1.00, I(2)  = 53%, p(heterogeneity)  = 0.06, three case-control studies and three cohort studies, 3,460 cases, range of Activity = 0-50 MET-hour/week) and an increase by an hour/week was associated with an ∼5% reduced risk of EC (summary RR = 0.95, p   0.69). In conclusion, an increase in Leisure-Time Physical Activity may continue to decrease EC risk, within the range of 0-50 MET-hour/week or 0-15 hour/week. Future studies should evaluate possible independent role of intensity of Physical Activity and effect modification by obesity.

However, none of the few prospective studies in women has shown a statistically significant reduction in colon cancer incidence or mortality associated with increased Leisure-Time Physical Activity. Purpose: In this prospective study, we asked whether Leisure-Time Physical Activity, body mass index, or body fat distribution could significantly influence the risk of colon cancer in women. Methods: The participants in this study were enrolled in the Nurses’ Health Study, which began in 1976. Every 2 years, the women provided additional personal information and information on medical risk factors and major medical events. The time spent per week at a variety of Leisure-Time Physical activities was determined, and the time spent at each Activity was multiplied by its typical energy expenditure, expressed in terms of metabolic equivalents or METs. The resulting values for each woman were added to yield an MET-hours-per-week score. Reported diagnoses of colon cancer were confirmed by review of hospital records and pathology reports. Relative risks and associated 95% confidence intervals were calculated. Results: In multivariate analyses that included body mass index, women who expended more than 21 MET-hours per week on Leisure-Time Physical Activity had a relative risk of colon cancer of 0.54 (95% confidence interval [CI] = 0.330.90) in comparison with women who expended less than 2 MET-hours per week. Women who had a body mass index greater than 29 kg/m 2 had a relative risk of colon cancer of 1.45 (95% CI = 1.02-2.07) in comparison with women who had a body mass index less than 21 kg/m 2 . A tendency toward higher colon cancer risk was observed for increasing waist-to-hip ratio (relative risk = 1.48 [95% CI = 0.88-2.49] for comparison of the highest quintile ratio [>0.833] to the lowest [<0.728]). Conclusions and Implications: The significant inverse association between Leisure-Time Physical Activity and incidence of colon cancer in women in this study is consistent with what has been found in men. Recommendations to increase Physical Activity and maintain lean body weight should receive greater emphasis as part of a feasible approach to the prevention of colon cancer.     The prevalence of Leisure-Time Physical Activity has increased in recent decades, but values differ significantly among countries. Owing to the improvement of living standards, a higher frequency of Leisure-Time Physical Activity is expected in Western countries such as Spain, but there is a lack of research involving large, representative samples during a prolonged temporal frame to confirm this tendency. Individual representative data from the 1987, 1993, 1995, 1997, 2001, 2011, and 2017 rounds of the Spanish National Health Survey were used. Statistical analyses were conducted from April 9, 2020 to May 5, 2020. A total of 114,813 participants (aged 43.9 [SD=16.7] years, 51.8% women) were included in the study. Crude linear trends in regular monthly and weekly Leisure-Time Physical Activity were estimated together using linear regression models across the survey years, which also served to estimate the regression coefficients (β) and 95% CIs for every year change. All age groups experienced significantly increased prevalence of regular Leisure-Time Physical Activity-either several times a month or several times a week-over time (p<0.001 for trend). Participants aged 16-17 years had the highest increase in the annual prevalence of regular Leisure-Time Physical Activity (β=0.8, 95% CI=0.7, 1.0). Additional adjusted multivariable logistic regression confirmed these trends. This study shows an increase in regular Leisure-Time Physical Activity among the adult Spanish population during the period from 1987 to 2017.
