It is a severe disease that may lead to destruction of the nail plate. It affects fingernails more than toenails. Early diagnosis is important due to its aggressive behavior. Histopathology should be carried out, but in many occasions it is not enough to come to a conclusive diagnosis. Dermatoscopy, a complementary tool, has proven to be useful in its diagnosis, management, and prognosis. Currently, there is very little data regarding dermatoscopy of nail Lichen Planus. Methods Dermatoscopic photographic data of 11 patients having 79 nails affected with nail Lichen Planus, seen in a specialized nail disease facility, were selected and analyzed. The data was confirmed with histopathological analysis. Results Dermatoscopy showed abnormalities of the nail matrix, with trachyonychia in 40.51% and pitting in 34.18%. As to nail bed anomalies, there was chromonychia in 55.70%, fragmentation of body of nail in 50.63%, splinter hemorrhage in 35.44%, onycholysis in 27.85%, and subungual keratosis in 7.59%. Concerning anomalies that involved nail matrix, bed, and perionychial region altogether, there were longitudinal streaks in 82.28% and anonychia in 1.27%. Paronychia was present in 31.65% of the cases. Conclusion Considering that nail Lichen Planus is an underdiagnosed disease with severe consequences, early diagnosis is essential. This descriptive study of dermatoscopic characteristics of nail Lichen Planus would highlight some key changes in the course of the disease that will contribute to early diagnostic suspicion, early treatment, and could improve prognosis.

Lesions most commonly occur on the limbs and on the dorsal aspect of the trunk. At the same time often leukoplakia of mucous membranes as well as nail disorders are seen. There are numerous variants of Lichen Planus which can be distinguished from the classical form on the basis of morphology and distribution of the lesions. The typical primary lesion of Lichen Planus may be replaced by other forms, such as patches, hyperkeratoses, ulcerations, or bullous lesions. Moreover, distribution patterns of these lesions may vary and include erythrodermic, inverse or linear arrangements. In contrast to these numerous clinical features, histologic findings remain characteristic in the variants, so that the diagnosis can be made securely. Differential diagnoses of Lichen Planus include diverse dermatoses such as bullous pemphigoid or paronychia.

Management of Oral Lichen Planus

Most dental and medical practitioners see patients with Lichen Planus, but not all are recognized as having the disease. Patients with Lichen Planus may have concomitant involvement of the disease in multiple sites. Oral Lichen Planus lesions usually have a distinctive clinical morphology and characteristic distribution, but oral Lichen Planus may also present a confusing array of patterns and forms, and other disorders may clinically mimic oral Lichen Planus. The etiopathogenesis of Lichen Planus appears to be complex, with interactions between genetic, environmental, and lifestyle factors. Much has now been clarified about the etiopathogenic mechanisms involved and interesting new associations, such as with liver disease, have emerged. The management of Lichen Planus is still not totally satisfactory in all cases and there is as yet no definitive treatment that results in long term remission, but there have been advances in the control of the condition. Amongst the many treatments available, high potency topical corticosteroids remain the most reliably effective, though topical cyclosporine, topical tacrolimus, or systemic corticosteroids may be indicated in patients whose condition is unresponsive to topical corticosteroids.
