Energy attenuator for pediatric application of an automated external defibrillator.

Snyder, Hans Patrick Griesser, Tom Solosko, Konrad Chan, Teresa SkarrAbstract:Although automatic external defibrillators (AEDs) are extensively deployed to rapidly treat sudden cardiac arrest in adults, their applicability for children is presently limited. It is desirable to extend the indications for this Lifesaving Equipment to all ages, even though AED application to children will be rare compared with adults. It is imperative that the inherent simplicity of present adult AED operation not be compromised to extend its use to include children. We propose a method that does not affect the normal operation of an AED on adults. For adults, unmodified AEDs would be used normally with adult electrodes. However, special pediatric electrodes would be available as a disposable accessory. When used with the AED, the delivered energy would be reduced within the electrodes, and only a portion of the energy output by the AED would be delivered to the pediatric patient. These electrodes could be used in conjunction with currently deployed AEDs with electrocardiographic analysis algorithms appropriate for children. This eliminates the need for a separate AED specifically for children or the purchase of a new AED with pediatric capability to replace previously deployed models.

No body is expendable: Medical rationing and disability justice during the COVID-19 pandemic.

PilarskiAbstract:The health threat posed by the novel coronavirus that caused the COVID-19 pandemic has particular implications for people with disabilities, including vulnerability to exposure and complications, and concerns about the role of ableism in access to treatment and medical rationing decisions. Shortages of necessary medical Equipment to treat COVID-19 have prompted triage guidelines outlining the ways in which Lifesaving Equipment, such as mechanical ventilators and intensive care unit beds, may need to be rationed among affected individuals. In this article, we explore the realities of medical rationing, and various approaches to triage and prioritization. We discuss the psychology of ableism, perceptions about quality of life, social determinants of health, and how attitudes toward disability can affect rationing decisions and access to care. In addition to the grassroots advocacy and activism undertaken by the disability community, psychology is rich in its contributions to the role of attitudes, prejudice, and discriminatory behavior on the social fabric of society. We call on psychologists to advocate for social justice in pandemic preparedness, promote disability justice in health care settings, call for transparency and accountability in rationing approaches, and support policy changes for macro- and microallocation strategies to proactively reduce the need for rationing. The right-to-die: state policymaking and the elderly.

GlickAbstract:Abstract The life prolonging power of medical technology and traditional ethics which require the maintenance of life have generated growing political demand for new law to regulate the use of Lifesaving Equipment and techniques, and to establish the rights of individuals to determine the course of their own treatment. This research maps the politics of the right-to-die through an analysis of state legislation and appellate court decisions and interviews with political participants in three states. It also examines the role of interest groups of and for the elderly in shaping this policy. The research has discovered that, with an important exception, state law has become more supportive of patients'rights since the earliest legislation, but that state courts and legislatures often compete in policymaking. However, the elderly have not substantially sought to influence this policy in either branch of government. The political potential of the elderly is examined.
