Very low calorie diets in the form of liquid meal replacements given 6 weeks prior to surgery have proven to decrease Liver volume. The aim of our study was to assess the effect of a 6-week preoperative low calorie/regular diet on Liver volume and body weight in morbidly obese patients. A pilot study was carried out in 20 morbidly obese patients with an 800-kcal diet for 6 weeks. They were followed weekly to ensure proper compliance. CT scan was used for determining Liver volume every 2 weeks in order to assess the impact of the diet. Baseline values were taken as controls for every patient as tied measures. Statistical analysis was suitable to variable scaling and performed using SPSS v. 20.0. Parametric and non-parametric test for tied measures were done. Any p value lesser than 0.05 or 5 % was considered as statistically significant. Twenty patients adhered to the diet intervention for 6 weeks prior to LRYGB. Median ± SD age was 34.5 ± 11.5 years and 17 were female (85 %). Heart rate and blood pressure did not vary across the study. Initial median ± SD BMI was 46.02 ± 5.29 kg/m2 (range 38.7–54.8). Repeated and tied measurements across the 6 weeks of treatment within individuals resulted statistically significant for reducing BMI (p < 0.0001). CT scan assessed Liver volume initially as a control and in weeks 2, 4 and 6. Parametric and non-parametric assessment for multiple measurements also showed statistical significance among these values (p < 0.0001). Diet tolerability was additionally evaluated with a questionnaire showing more than 80 % of acceptability with discrete rates of nausea (15 %) and diarrhoea (15 %). Based on our results, we demonstrated that a very low calorie diet with home ingredients is capable for effectively reducing body weight and Liver Size in morbidly obese patients. This relatively short intervention (4 to 6 weeks) was accomplished in all our patients with a high frequency of compliance and a low rate of secondary effects.

Environmental Toxicology and Chemistry, 2003Co-Authors: Brendan J Galloway, Kelly R Munkittrick, Steve L Currie, Michelle A Gray, Allen R Curry, Craig S WoodAbstract:As part of a larger survey on cumulative effects within the Saint John River basin (Canada), a fish survey was conducted near Edmundston (NB, Canada) in the fall of 1999 using slimy sculpin (Cottus cognatus) and white sucker (Catostomus commersoni). The discharge environment receives effluent from the pulp mill, a paper mill, three sewage discharges, and tributaries receiving agricultural runoff. Sculpin collected downstream of the sewage discharges and pulp mill effluent had greater growth, condition, and Liver Size but no significant differences in gonad Size. Stable isotope data indicated slimy sculpin did not move between sites. Female sculpin collected downstream of the paper mill showed no significant differences in length, body weight, age, condition factor, Liver Size, and gonad Size compared to fish from reference sites. Female white sucker collected downstream of the pulp mill did not differ significantly in any measured parameter compared to reference fish. Liver Sizes of white sucker from the Saint John River were outside the range considered to be indicative of uncontaminated riverine sites. In 2000, sculpin collected downstream from a poultry-processing facility had larger Livers and lower condition factors, suggesting that the site is contaminated. We found no significant differences in sculpin length, weight, condition (except for males), and Liver Size in sculpin collected downstream from the pulp mill in October 2001. The responses of slimy sculpin and white sucker differed, perhaps in relation to differences in life history characteristics. Results from this study indicate the slimy sculpin is a suitable fish species for monitoring rivers that receive multiple industrial and municipal effluents.

Field collections included sampling of receiving water for chemistry and toxicity testing, and sampling of local fish for organ weights, hepatic MFO (ethoxyresorufin-O-deethylase, EROD) activity, plasma steroid levels, and levels of Liver dioxins. The main objectives of this study were to determine whether the discharge of effluent from pulp mills to sites other than Jackfish Bay was associated with physiological or biochemical disruptions in wild fish, whether there was any correlation between waste treatment and the presence of biological responses in wild fish, and whether there was any association between the use of chlorine as a bleaching agent and these responses. Although white sucker collected near bleached-kraft mills exhibited the highest EROD induction and dioxin levels, elevated enzyme activity was observed in fish from sites that did not use chlorine, and depressions in plasma sex steroid levels was not correlated with the level of EROD activity. The absence of chlorine bleaching or the presence of secondary treatment did not eliminate responses in fish, including decreased circulating levels of sex steroids, decreased gonadal Size, and increased Liver Size. This survey has shown that (a) induction of hepatic EROD enzymes and depressions of plasma sex steroid levels during gonadal growth are found downstream of several pulp mills; (b) these changes are seen at some mills without chlorine bleaching and at mills that have secondary treatment; (c) substantial dilutions of nontoxic effluent do not appear to remove these responses; (d) the dominant factor determining the presence or absence of responses appeared to be dilution level; and (e) lab toxicity tests on invertebrates, rainbow trout, and fathead minnows could not predict the presence of these responses in wild fish.

Current treatment of GSD III is based on frequent high-carbohydrate meals that have no effect on the cardiomyopathy. We describe a 2-mo-old infant presenting with a familial form of GSD III complicated with cardiomyopathy. As conventional treatment was unable to improve his sister's cardiomyopathy who was deceased at age 11 mo, we proposed an experimental treatment combining the use of synthetic ketone bodies (d,l-3-OH butyrate) as an alternative energy source, 2:1 ketogenic diet to reduce glucose intake and high-protein diet to enhance gluconeogenesis. Twenty-four months after the onset of this treatment, echocardiography showed an improvement of cardiomyopathy. Growth and Liver Size remained normal, and no side effects were observed. Blood glucose levels remained within the normal range and insulin levels decreased. These findings show that synthetic ketone bodies as well as low-carbohydrate, high-lipid, and high-protein diet may be a more beneficial therapeutic choice therapeutic choice for GSD III patients with cardiomyopathy. These encouraging data need to be confirmed in more GSD III patients presenting with cardiac or muscular symptoms.
