Microsporum Gypseum infection in the Siena area in 2005-2006.

Gallo, Michele FimianiAbstract:Fourteen cases of dermatophytosis caused by Microsporum Gypseum, representing 6.8% of all dermatophytic infections reported, were diagnosed in Siena, Italy, between 2005 and 2006. There were as follows: six cases of tinea corporis, one case of tinea corporis associated with tinea capitis, one case of tinea corporis associated with tinea barbae, one kerion on the head, one tinea cruris, one tinea faciei, one tinea barbae, two onychomycosis. In the three subjects with tinea corporis, the clinical appearance was impetigo-like, psoriasis-like and pityriasis rosea-like respectively. In another case, the lesion was indicative of tinea imbricata. The diagnosis was based on mycological examination. In six cases, the source of infection was a cat, whereas in the others it was contact with soil.

It is caused by prolonged use of topical steroids, sometimes prescribed as a result of incorrect diagnosis. The cases reported in the literature have different clinical presentations and have generally been in adults. We report three children with tinea incognito in whom the lesions were psoriasis-like, eczema-like, and lichenoid, respectively. Diagnosis was confirmed by mycologic examination, which led to the identification of Microsporum Gypseum, a geophilic dermatophyte which is an infrequent agent of mycotic infection in humans.

Wir beschreiben die ersten vier der in Italien zwischen 1990 und 1997 beobachteten Falle von Nagelmykosen mit Microsporum Gypseum als Erreger. Das klinische Bild war in allen Fallen durch eine distale subunguale Ony-chomycoses gekennzeichnet. Die Lasionen waren in zwei Patienten asymptomatisch. Drei der Patienten wurden mit Itraconazol gemass dem Pulstherapieschema behandelt, wahrend ein Patient lokal 6 Monate lang mit Ciclopirox behandelt wurde. Alle Falle heilten klinisch und mykologisch vollstandig ab. Wir beschreiben diese Falle wegen ihrer Seltenheit.

Bei den drei Pilzarten fanden sich zeitliche Unterschiede in der Assimilation der funf angebotenen Aminosauren. Ubereinstimmend wurden von allen Pilzen zuerst Lysin und Glutamat, dann mehr oder weniger verzogert die anderen drei Aminosauren Glycin, Leucin und Phenylalanin abgebaut. Der Verlauf der Aminosaureassimilation korrelierte mit der Trockengewichtszunahme des Pilzmyzels. Summary: The amino acid concentration in culture media of Microsporum Gypseum, Trichophyton rubrum and Trichophyton mentagrophytes has been determined in relation to the dry weight of the fungal mycelia during 24 days of cultivation. The three fungus species investigated showed a different time pattern of assimilation of the five amino acids added. All species degraded lysine and glutamate first, but glycine, leucine and phenylalanine with more or less retardation. The time course of amino acid assimilation showed correlations with the increase in dry weight of the fungal mycelia.

Cystin wurde schneller abgebaut als Glutaminsaure, wahrend umgekehrt der Myzelzuwachs mit Glutaminsaure wesentlich besser war als mit Cystin. Bei Kombination beider Aminosauren fanden wir eine Beschleunigung sowohl des Aminosaureabbaues als auch des Myzelwachstums. Summary: Using cystine and glutamicacid, alone or in combination, assources of nitrogen, we found in Microsporum Gypseum a different time pattern for amino acid degradation and mycelial growth. Cystine was degraded faster than glutamic acid, whereas the mycelial growth was markedly better with glutamic acid than with cystine. With the amino acids given in combination, we found a higher rate of both, amino acid degradation and mycelial growth.

Dystrophic onychomycosis due to Microsporum Gypseum.

The infection had a 2-year history, occurring in the site of a nail trauma sustained in a fall from a horse. Diagnosis was based on mycological examination (direct microscope observation and culture). Clinical and mycological recovery was achieved after local treatment with ciclopiroxolamine nail solution and pulsed systemic terbinafine therapy.
