Cow's Milk allergy versus cow Milk Intolerance

BahnaAbstract:Background: Although cow's Milk allergy (CMA) and cow's Milk Intolerance (CMI) are two different terms, they are often used interchangeably, resulting in confusion both in clinical practice and in research reports. Objective: To promote the appropriate differential use of the terms CMA and CMI. Methods: Highlighting the differences in clinical and laboratory findings between CMA and CMI. Information was derived from reviewing the literature on these two topics, supplemented by the clinical experience of the author. Results: CMA is an immunologically mediated reaction to cow's Milk proteins that may involve the gastro-intestinal tract, skin, respiratory tract, or multiple systems, ie, systemic anaphylaxis. Its prevalence in the general population is probably 1 to 3%, being highest in infants and lowest in adults. Even though it can cause severe morbidity and even fatality, dietary elimination is associated with good prognosis. However, CMI should refer to nonimmunologic reactions to cow's Milk (CM), such as disorders of digestion, absorption, or metabolism of certain CM components. The most common cause of CMI is lactase deficiency, which is mostly acquired during late childhood or adulthood. It has high racial predilection, being highest in dark-skinned populations and lowest in northern Europeans. Lactose Intolerance is generally a benign condition, with symptoms limited to the gastro-intestinal tract, yet the primary acquired type lasts for a lifetime. Symptoms can be well ameliorated by reducing the intake of CM or using lactose-hydrolyzing agents. Conclusions: Adverse reactions to CM should be differentiated into immunologic (CMA) and nonimmunologic (CMI). The latter is still a general term that comprises several conditions and requires further differentiation.

Milk consumption in older Americans.

FischerAbstract:OBJECTIVES: The purpose of this study was to identify individual characteristics associated with types and frequency of Milk consumption in older American adults. METHODS: A national probability-based sample (response rate = 91%) completed a telephone survey. Generalized logit and cumulative logit analyses were used to identify predictors of and barriers to fluid Milk consumption in 494 elderly people. RESULTS: The likelihood of drinking skim or 1% Milk rather than whole Milk increased with nutrition knowledge, income, trying to reduce cholesterol intake, and being female (P < .05). Frequency of Milk consumption was higher with nutrition knowledge, frequency of Milk consumption during adolescence, and following a diabetic diet but was lower with Milk Intolerance. CONCLUSIONS: The present results could be used to develop intervention strategies for improving Milk consumption rates among older adults. The Influence of Milk Intolerance on Milk Consumption in Older Adults

FischerAbstract:Abstract LEARNING OUTCOME: To examine the impact of Milk Intolerance on Milk consumption in older adults. Current public health policy emphasizes optimizing calcium intakes in the elderly. Fluid Milk is a readily available source of calcium, but consumption may be limited by Milk Intolerance. Thus, the purpose of this study was to examine the impact of Milk Intolerance on Milk consumption in community-dwelling elders. A national probability-based sample of older adults in their 60s (n=132), 70s (n=157) and 80s (n=134) completed a telephone survey. Of the total sample, 18% (n = 87) indicated they did not tolerate Milk. From Chi square analyses, it was found that Milk Intolerance was twice as likely to occur in minority elders than in Caucasians (33% vs. 17%, p≤.02), and twice as likely to occur in women than in men (21% vs. 10%, p≤.01), but did not increase with age. Compared to individuals who tolerate Milk, Milk intolerant elders were more likely to avoid Milk during adolescence (29% vs. 15%, p≤.05) and to avoid Milk currently (57% vs. 22%, p≤.001), and were less likely to be concerned with consuming enough calcium (31% vs. 47%, p≤.02). Milk tolerant individuals were nearly twice as likely to drink Milk at least once a day compared to Milk intolerant individuals (56% vs. 28%, p≤.001). In conclusion, Milk intolerant elders appear to be at high risk for low calcium intakes because of their long history of Milk avoidance and their lack of concern about getting enough calcium as adults. Perceived Milk Intolerance Is Related to Bone Mineral Content in 10- to 13-Year-Old Female Adolescents

The purpose of this study was to determine associations among lactose maldigestion status, perceived Milk Intolerance, dietary calcium intake, and bone mineral content in early adolescent girls. METHODS. Subjects were 291 girls who participated in a substudy of the multiple-site project Adequate Calcium Today. Lactose maldigestion status was determined with hydrogen breath testing, and questionnaires were used to assess perceived Milk Intolerance. Dietary calcium intake was estimated from a semiquantitative food frequency questionnaire. Anthropometric and dual-energy x-ray absorptiometric measurements (total body, spine L2–L4, total hip, and hip femoral neck) were standardized across sites. RESULTS. Of the 230 girls who completed breath hydrogen testing, 65 were Asian, 76 were Hispanic, and 89 were non-Hispanic white. A total of 100 girls experienced increases in breath hydrogen levels of >20 ppm and were classified as lactose maldigesters. Of the 246 participants who completed useable perceived Milk Intolerance questionnaires, 47 considered themselves to be Milk intolerant. Of the 47 girls self-reporting perceived Milk Intolerance, 40 completed breath hydrogen testing and 22 were not maldigesters. Girls with perceived Milk Intolerance consumed an average of 212 mg of total food calcium per day less than girls without perceived Milk Intolerance. Spinal bone mineral content was significantly lower in the girls with perceived Milk Intolerance, compared with the girls without perceived Milk Intolerance. When girls with lactose maldigestion were compared with girls without lactose maldigestion, there were no significant differences in calcium intake or bone measures. CONCLUSIONS. These results suggest that, starting as early as 10 years of age, self-imposed restriction of dairy foods because of perceived Milk Intolerance is associated with lower spinal bone mineral content values. The long-term influence of these behaviors may contribute to later risk for osteoporosis.
