    The Prevention and Incidence of Asthma and Mite Allergy (PIAMA) birth cohort was set up in The Netherlands with two main aims: to investigate the effect of Mite-allergen avoidance on the incidence of childhood asthma (Intervention study) and to assess lifestyle and environmental risk factors for childhood asthma (Natural History study). The baseline study population consisted of 3963 children born in 1996– 1997. Questionnaires were completed by parents during pregnancy, when the child was 3 months old, then annually from 1 up to 8 years and by parents as well as children at 11 and 14 years. Data were obtained on child and family characteristics, a wide range of environmental and lifestyle exposures and on asthma and other allergic and respiratory outcomes. Home visits and clinical examinations were conducted in sub-groups at the ages of 1, 4, 8 and 12 years to obtain objective measures. These included anthropometric measures, measurements of lung function, bronchial hyper-responsiveness and blood pressure, house dust and blood samples, which were used to determine total and specific IgEs, cholesterol and HbA1c and to extract DNA.

    We investigated associations of time in bed and multiple sleep quality characteristics with cardiometabolic markers in children. Data from the prevention and incidence of asthma and Mite Allergy study, a population-based prospective birth-cohort study started in 1996-1997 in the Netherlands, were analysed. In total 1481 children aged 11-12years completed a questionnaire (including questions on sleep) and underwent a medical examination. We measured body mass index, waist circumference, total- and high-density lipoprotein cholesterol, blood pressure and glycated haemoglobin. Results showed that in girls, some sleep characteristics were related to anthropometrics (body mass index, waist circumference) and cholesterol. Girls who had a long time in bed (11-12.5h) had 0.16 lower body mass index z-score (95% confidence interval -0.31; -0.01) and 0.99cm smaller waist circumference (95% confidence interval -2.01; -0.13) compared with girls who spent 10-10.5h in bed. Girls who went to bed late and rose early had 0.16mm higher total cholesterol (95% confidence interval 0.01; 0.31) and 0.08mm higher high-density lipoprotein cholesterol (95% confidence interval 0.01; 0.14) than early to bed/early rise' girls. Girls with night-time awakenings had 0.14mm higher total cholesterol (95% confidence interval 0.03; 0.25) than girls without night-time awakenings. Girls who felt sleepy/tired 1day per week had 0.10mm lower high-density lipoprotein cholesterol (95% confidence interval -0.16; -0.04) and 0.17mm higher total cholesterol/high-density lipoprotein cholesterol ratio (95% confidence interval 0.02; 0.32) than girls who did not feel sleepy. No associations were found for boys. Sleep characteristics were not related to blood pressure and glycated haemoglobin, and effect sizes of the associations in girls were small. Therefore, we consider it premature to propose that improved sleep could reduce cardiovascular risk during childhood.

    Compared with nonbreastfed children, breastfed children tend to have a lower body mass index (BMI) at about 1 year of age. How the BMI of breastfed children develops after the first year when this difference in BMI at 1 year of age is considered is not clear. The authors studied the association between breastfeeding and BMI development from 1 to 7 years of age independently of BMI at 1 year of age. Longitudinal BMI data reported by parents of 2,347 Dutch children born in 1996–1997 who participated in the Prevention and Incidence of Asthma and Mite Allergy birth cohort study were collected. Linear regression and mixed-effects models were used for data analyses. Mean BMI at 1 year of age was 17.2 kg/m 2 (standard deviation, 1.4). Compared with nonbreastfed children, children breastfed for >16 weeks had a lower BMI at 1 year of age, after adjustment for confounders (b ¼� 0.22, 95% confidence interval: � 0.39, � 0.06). The association between breastfeeding and BMI between 1 and 7 years of age was negligible, while a high BMI at 1 year of age was strongly associated with a high BMI between 1 and 7 years of age in the same model. Objective: We prospectively studied breast milk content of n-6, n-3, and trans fatty acids in relation to allergic symptoms at the ages of 1 and 4 years. Methods: Fatty acid content was determined in breast milk samples of 265 (158 allergic and 107 nonallergic) mothers of children participating in the Prevention and Incidence of Asthma and Mite Allergy study. Outcome variables studied were parental reported eczema at age 1 year, eczema at age 4 years, asthma at age 4 years, and, in a subgroup of 133 children, sensitization at age 4 years. Results: In children of mothers with Allergy, breast milk n-3 long chain polyunsaturated fatty acids and the ratio between n-3 and n-6 long chain polyunsaturated fatty acids were inversely associated with asthma and with persistent symptoms (eczema at age 1 year and eczema at age 4 years and/or asthma at age 4 years), but no associations between breast milk fatty acids and sensitization were observed. In children of mothers with Allergy, also trans fatty acids tended to be inversely associated with allergic symptoms. In children of mothers without Allergy, no associations between breast milk fatty acids and allergic symptoms were observed, but α-linolenic acid (18:3n-3) was positively associated with sensitization.     Background For respiratory allergic disorders in children, sublingual immunotherapy has been developed as an alternative to subcutaneous immunotherapy. Sublingual immunotherapy is more convenient, has a good safety profile and might be an attractive option for use in primary care. A randomized double-blind placebo-controlled study was designed to establish the efficacy of sublingual immunotherapy with house dust Mite allergen compared to placebo treatment in 6 to18-year-old children with allergic rhinitis and a proven house dust Mite Allergy in primary care.     We investigated associations of time in bed and multiple sleep quality characteristics with cardiometabolic markers in children. Data from the prevention and incidence of asthma and Mite Allergy study, a population-based prospective birth-cohort study started in 1996-1997 in the Netherlands, were analysed. In total 1481 children aged 11-12years completed a questionnaire (including questions on sleep) and underwent a medical examination. We measured body mass index, waist circumference, total- and high-density lipoprotein cholesterol, blood pressure and glycated haemoglobin. Results showed that in girls, some sleep characteristics were related to anthropometrics (body mass index, waist circumference) and cholesterol. Girls who had a long time in bed (11-12.5h) had 0.16 lower body mass index z-score (95% confidence interval -0.31; -0.01) and 0.99cm smaller waist circumference (95% confidence interval -2.01; -0.13) compared with girls who spent 10-10.5h in bed. Girls who went to bed late and rose early had 0.16mm higher total cholesterol (95% confidence interval 0.01; 0.31) and 0.08mm higher high-density lipoprotein cholesterol (95% confidence interval 0.01; 0.14) than early to bed/early rise' girls. Girls with night-time awakenings had 0.14mm higher total cholesterol (95% confidence interval 0.03; 0.25) than girls without night-time awakenings. Girls who felt sleepy/tired 1day per week had 0.10mm lower high-density lipoprotein cholesterol (95% confidence interval -0.16; -0.04) and 0.17mm higher total cholesterol/high-density lipoprotein cholesterol ratio (95% confidence interval 0.02; 0.32) than girls who did not feel sleepy. No associations were found for boys. Sleep characteristics were not related to blood pressure and glycated haemoglobin, and effect sizes of the associations in girls were small. Therefore, we consider it premature to propose that improved sleep could reduce cardiovascular risk during childhood.

    Compared with nonbreastfed children, breastfed children tend to have a lower body mass index (BMI) at about 1 year of age. How the BMI of breastfed children develops after the first year when this difference in BMI at 1 year of age is considered is not clear. The authors studied the association between breastfeeding and BMI development from 1 to 7 years of age independently of BMI at 1 year of age. Longitudinal BMI data reported by parents of 2,347 Dutch children born in 1996–1997 who participated in the Prevention and Incidence of Asthma and Mite Allergy birth cohort study were collected. Linear regression and mixed-effects models were used for data analyses. Mean BMI at 1 year of age was 17.2 kg/m 2 (standard deviation, 1.4). Compared with nonbreastfed children, children breastfed for >16 weeks had a lower BMI at 1 year of age, after adjustment for confounders (b ¼� 0.22, 95% confidence interval: � 0.39, � 0.06). The association between breastfeeding and BMI between 1 and 7 years of age was negligible, while a high BMI at 1 year of age was strongly associated with a high BMI between 1 and 7 years of age in the same model.     Background Reduction of allergen exposure from birth may reduce sensitization and subsequent allergic disease. Objective To measure the influence of Mite allergen-impermeable mattress encasings and cotton placebo encasings on the amount of dust and Mite allergen in beds. Methods A total of 810 children with allergic mothers took part in the Prevention and Incidence of Asthma and Mite Allergy (PIAMA) study. Allergen-impermeable and placebo mattress encasings were applied to the childrens' and the parents' beds before birth. Dust samples were taken from the beds of children and their parents before birth and 3 and 12 months after birth. Extracts of dust samples were analysed for Mite allergens (Der p 1 and Der f 1). Results Active mattress encasings were significantly more effective in reducing dust and Mite allergen levels than placebo encasings. Mite allergen levels were low in general and the treatment effect was modest. Twelve months after birth, mattresses with active mattress encasings had about half the amount of Der 1 (Der p 1 + Der f 1)/m(2) , compared to mattresses with placebo encasings, for the child's and the parental mattress. Conclusion This study shows that Mite-impermeable mattress encasings have a significant but modest effect on dust and Mite allergen levels of mattresses with low initial Mite allergen levels, compared to placebo.

    Abstract:

    The Prevention and Incidence of Asthma and Mite Allergy (PIAMA) birth cohort study was initiated in 1996. Children born to allergic mothers were enrolled in a double-blind placebo-controlled trial for evaluating the use of Mite-impermeable mattress and pillow covers. Children born to allergic and non-allergic mothers were enrolled in a 'natural history' study to assess the role of environmental and dietary risk factors for the development of allergic disease in childhood. Recruitment started by distributing a validated screening questionnaire among >10,000 pregnant women during their first visit to a prenatal health clinic. Allergic mothers-to-be were invited to participate in the intervention study. Allergic, and a random sample of non-allergic, mothers-to-be were invited to participate in the 'natural history' arm of the study. In the intervention study, homes were visited before birth, 3 months after birth, and 12 months after birth for the collection of dust samples from floors and mattresses. In addition, the homes of about one-third of the children in the 'natural history' part of the study were visited for dust collection when the children were 3 months of age. The intervention study started with 855 participants and the 'natural history' study with 3,291 participants. Follow-up at 3 years of age has now been completed with satisfactory compliance (>90%,). A medical investigation and home visit at 4 years of age are nearing completion. Preliminary results show that Mite-allergen levels were lower than found in previous Dutch studies, and that the intervention measure had a significant effect on Mite-allergen levels, without important clinical benefits up to age 2 years old. The allergic families lived in homes with fewer 'triggers' such as pets, smoking and carpets than the non-allergic families, regardless of the intervention. The ongoing PIAMA cohort study will probably reveal useful information concerning effects of allergen load and reduction in the setting of a relatively low Mite-allergen exposure, as well as other variables on the development of allergic manifestions and asthma.
