    Abstract:

    The healing process of wounds made by a combination laser was studied in 90 rats. The laser system enabled both separate and combined use of CO2, and Nd:YAG laser irradiations. The laser wounds and the control excision wounds made by alligator forceps appeared on both sides of the tongue. Specimens from the wound sites were taken immediately, 6 h, and 1, 2, 4, 7, 11, 21, 28, and 42 days after surgery. The wound-healing process was studied by macroscopic evaluation before preparing the specimens for light microscopy. Some dilferences were noted in the wound-healing process among the three groups into which the experimental animals were divided. Tissue coagulation damage was most extensive in the Nd:YAG laser sites, where it was observed in its full extent 4 days after surgery. Epithelial cells were seen to begin to proliferate in all the wounds 6 h after surgery. Re-epithelialization was completed by between 7 (CO2) and 21 days (Nd:YAG) at all the wound sites. The inflammatory cell infiltration was more prominent in the Nd:YAG and the COrNd:YAG combination laser wounds than in the COi and excision wounds during healing. Tissue regeneration occurred faster with less contraction in the combination CO2-Nd:YAG wounds than in Nd:YAG wounds. The best macroscopic healing result was seen in the CO2, wound sites. The combination laser was effective both at cutting and at coagulating tissue. Combining the CO2 and Nd:YAG laser irradiation into one beam resulted in a greater incision depth than what could have been expected from using the two lasers separately.

    BACKGROUND: Chronic alcohol consumption is a major risk factor for oral and pharyngeal cancer. Besides other mechanisms a toxic effect of ethanol and/or its metabolite acetaldehyde on the oral Mucosa and a resulting increased cell regeneration seem to play an important role in tumor promotion. METHODS: In the present study the effect of chronic ethanol consumption on the morphology of the oral Mucosa of 20 male wistar rats that had been fed nutritionally adequate liquid diets containing 36% of total calories either as ethanol or isocaloric carbohydrates for six months was investigated. RESULTS: Morphometric analysis showed that in the ethanol rats, the size of the basal cell nuclei of the oral Mucosa from the floor of the Mouth, the edge of the tongue, and the base of the tongue was significantly increased (p < 0.001). The size of the basal cell layer in these rats was increased, and the stratification of the cells was altered. Mean epithelial thickness of the Mucosa from the floor of the Mouth was significantly reduced in the ethanol rats (p < 0.01). CONCLUSIONS: In conclusion, our results indicate that chronic ethanol consumption causes oral Mucosa atrophy associated with hyperregeneration, which may result in an enhanced susceptibility of the Mucosal epithelium to chemical carcinogens.

    : We report the case history of a 60-year-old male caucasian with lichen ruber planus of the oral cavity. After having this condition for 5 years, a medium-grade differentiated squamous cell carcinoma developed on the same side. Since a history of tobacco and alcohol abuse was negative, the Lichen ruber planus was considered to be a precancerous lesion without any other concomitant condition.

    : In a prospective study we examined the effect of preoperative chemotherapy with cisplatinum and 5-fluorouracil (5-FU) in 40 patients with advanced carcinoma of the oral cavity. Histopathologic grading, cell kinetics and immunohistochemical parameters of the Mucosa were determined at two different locations. Prior to therapy, histologic dysplasias in the Mucosa "close" to the tumor were observed in 75 percent of the patients. Dysplastic changes of the Mucosa "far" from tumor were also found in more than one-third of the patients. Our results show that more advanced dysplasias improve only temporarily under the influence of preoperative systemic chemotherapy. Moreover, new dysplasias appearing during chemotherapy and persisting after its termination are probably induced. The results point to a possible carcinogenic potential of the combined chemotherapy schema used.

    Abstract:

    Recent research has shown statistical associations between dental infections and cancer in general but the role of oral microbiota in carcinogenesis is unclear. Oral micro-organisms up-regulate cytokines and other inflammatory mediators that affect the complex metabolic pathways and may thus indeed be involved in carcinogenesis. Microbial populations on Mouth Mucosa differ between healthy and malignant sites and certain oral bacterial species have been linked with malignancies. Oral microbes also have carcinogenic metabolites, such as acetaldehyde produced from ethanol. In this chapter we briefly review current knowledge about the interaction between oral microorganisms, oral infections and cancer. The focus is both on oral and ear-nose-throat cancer and malignancies in other organs.

  • Detection of p53 mutation in Mouth Mucosa smears of patients with oral squamous epithelial carcinoma

    Das Tumorsuppressorgen p53 kodiert fur ein wichtiges Regulatorprotein des Zellzyklus und hat hierdurch wahrscheinlich eine Bedeutung fur die Entstehung einer Vielzahl von malignen Erkrankungen, u. a. auch fur das Plattenepithelkarzinom der Mundhohle. Es ist im Zusammenhang mit der Krebsentstehung das am haufigsten mutierte Gen und schon relativ gut untersucht. Daher wahlten wir es aus, um herauszufinden, inwieweit Abstriche der Mundschleimhaut geeignet sind, Material zum Nachweis von Mutationen in einem Gen zu liefern, das in Zusammenhang mit der Entstehung von Plattenepithelkarzinomen der Mundhohle steht, denn Abstriche sind mit wesentlich geringerem Aufwand zu gewinnen als bioptisches Material. Wir untersuchten Probeexzisionen, Tumorabstriche und Abstriche gesund erscheinender Schleimhaut von 32 Patienten mit Mundhohlenkarzinomen sowie Schleimhautabstriche von 35 gesunden Kontrollpersonen mit Polymerasekettenreaktion (PCR) und Temperaturgradientengelelektrophorese (TGGE). Vierzehn der 32 Tumorpatienten wiesen Mutationen von p53 auf, und in jedem Fall wurden die Mutationen sowohl in der Biopsie als auch im Tumorabstrich nachgewiesen. In vier Fallen wurde die Mutation auch im Abstrich der gesund erscheinenden Mukosa gefunden. Nach unseren Untersuchungen hat sich die Abstrichnahme als geeignete Methode erwiesen, Material fur die Detektion von Genmutationen in Plattenepithelkarzinomen der Mundschleimhaut zu gewinnen.

    : Tumour-suppressor gene p53 encodes for an important cell-cycle regulatory protein and is therefore probably important for the development of many malignant diseases, e.g. squamous cell carcinoma of the Mouth. This gene has mutated most frequently in connection with the development of cancer, so it has been well explored. Hence we chose it to find out whether swabs of the oral Mucosa are suitable for supplying material for the detection of mutations in a gene that is connected with the development of oral squamous cell carcinomas, because swabs are easier to obtain than biopsies. We examined biopsies, swabs from the tumour, and swabs from Mucosa that appeared healthy from 32 patients with oral squamous cell carcinoma and Mucosal swabs from 35 healthy persons with polymerase chain reaction (PCR) and temperature gradient gel electrophoresis (TGGE). Fourteen of the 32 patients with a tumour showed mutations of p53, and in all cases the mutation could be demonstrated both in the biopsy and in the tumour swab. In four cases the mutation was also found in the swab of normal Mucosa. Our investigations revealed that swabs are a suitable method for obtaining material for the detection of gene mutations in oral squamous cell carcinomas.
