Moxifloxacin: a respiratory fluoroquinolone.

Moxifloxacin is a fourth-generation fluoroquinolone that has been shown to be effective against Gram-positive, Gram-negative, and atypical strains, as well as multi-drug resistant Streptococcus pneumoniae. Objective: To review and update the clinical efficacy of Moxifloxacin in the treatment of respiratory infections. Method: To perform a systematic review of publications on the clinical efficacy of Moxifloxacin in respiratory infections. Results: The clinical efficacy of Moxifloxacin has been shown in controlled studies of community-acquired pneumonia, exacerbations of chronic bronchitis and acute bacterial rhinosinusitis. Moxifloxacin has demonstrated a faster resolution of symptoms in community-acquired pneumonia and exacerbations of chronic bronchitis patients compared with first-line therapy together with excellent eradication rates. Moxifloxacin in the management of exacerbations of chronic bronchitis and COPD
International journal of chronic obstructive pulmonary disease, 2007Co-Authors: Marc MiravitllesAbstract:Bacteria are isolated in more than 50% of exacerbations of chronic bronchitis (CB) and chronic obstructive pulmonary disease (COPD). The most prevalent respiratory pathogens include Gram-positive (Streptococcus pneumoniae) and Gram-negative (Haemophilus influenzae, Moraxella catarrhalis) microorganims. Moxifloxacin is a fourth-generation fluoroquinolone that has been shown to be effective against respiratory pathogens, including atypicals and those resistant to most common antibiotics. The bioavailability and half-life of Moxifloxacin provides potent bactericidal effects at a dose of 400 mg once daily. Among the fluoroquinolones, the ratio of the area under the concentration-time curve (AUC) to minimal inhibitory concentration of Moxifloxacin is the highest against S. pneumoniae. Moxifloxacin has demonstrated better eradication in exacerbations of CB and COPD compared with standard therapy, in particular, with macrolides. Patients treated with Moxifloxacin showed a prolonged time to the next exacerbation and observational studies suggest that Moxifloxacin induces a faster release of symptoms of exacerbation. Some guidelines recommend the use of Moxifloxacin as first-line therapy in bacterial exacerbations in patients with moderate to severe COPD and in patients with mild COPD with risk factors. The current article reviews the use of Moxifloxacin in bacterial exacerbations of CB and COPD.

Expert opinion on pharmacotherapy, 2005Co-Authors: Marc MiravitllesAbstract:Moxifloxacin is a fourth-generation fluoroquinolone that has been shown to be effective against respiratory pathogens, including Gram-positive (Streptococcus pneumoniae), Gram-negative (Haemophilus influenzae, Moraxella catarrhalis), and atypical strains (Chlamydia pneumoniae, Mycoplasma pneumoniae), as well as multi-drug resistant S. pneumoniae, including strains resistant to penicillin, macrolides, tetracyclines, trimethoprim/sulfamethoxazole and some fluoroquinolones. Moxifloxacin is highly concentrated in lung tissue, and has demonstrated rapid eradication rates. The bioavailability and half-life of Moxifloxacin provides potent bactericidal effects at a dose of 400mg/day. The ratio of the area under the concentration-time curve to MIC of Moxifloxacin is the highest among the fluoroquinolones against S. pneumoniae. The clinical efficacy of Moxifloxacin has been shown in controlled studies of community-acquired pneumonia (CAP), exacerbations of chronic bronchitis (CB) and acute bacterial rhinosinusitis. Moxifloxacin has demonstrated a faster resolution of symptoms in CAP and exacerbations of CB patients compared with first-line therapy. It has also demonstrated better eradication in exacerbations of CB compared with standard therapy, in particular the macrolides. Treatment guidelines should take into account the results of clinical trials with Moxifloxacin in order to establish the role of this antimicrobial in the therapeutic arsenal against respiratory tract infections.

METHODS: Two cases of fungal keratitis in which the addition of topical Moxifloxacin or Moxifloxacin and gentamicin led to an improved clinical response to topical voriconazole were reviewed retrospectively. RESULTS: One patient with clinical resistance of his fungal keratitis to both topical voriconazole and natamycin had resolution of his keratitis with the addition of topical Moxifloxacin and gentamicin to voriconazole. One patient who had a poor response to topical voriconazole had a dramatic response to the increase of the voriconazole regimen and addition of Moxifloxacin. CONCLUSIONS: In a subset of patients with fungal keratitis, the addition of topical Moxifloxacin 0.3% or Moxifloxacin 0.3% and gentamicin 0.3% may enhance the therapeutic effect of topical voriconazole 1%.

Cure Rate of Fungal Keratitis With Antibacterial Therapy.

METHODS: A retrospective review of patients with culture-proven fungal keratitis who initially received Moxifloxacin 0.3% monotherapy was performed. RESULTS: Eleven patients with culture-proven fungal keratitis were initially treated with Moxifloxacin. One case each of Curvularia and Alternaria keratitis resolved with Moxifloxacin monotherapy (18%). CONCLUSIONS: Moxifloxacin may have a significant clinical therapeutic effect in a subset of patients with fungal keratitis. Review of the literature in combination with the current study suggests that in patients with clinical features suggestive of fungal keratitis, if rapid diagnostic tests are negative or not available, pending culture results, initial therapy should include a fluoroquinolone (Moxifloxacin or gatifloxacin) and/or an aminoglycoside (tobramycin or gentamicin).
