Iran’s Struggling Health System: An Increase in Natural Childbirth: A Case Study

This study evaluated the changes in the vaginal delivery and cesarean before and after implementation of the 7th package of health reform in public and private hospitals of Isfahan Province in 2014. Methods : This was a quasi‑experimental research. The population of this study contained all public and private hospitals in Isfahan Province covered by the Health Reform Program. It included 22 public and 6 private hospitals. The data collected from the vice chancellor of treatment in 6 months before and after the implementation of the program have been analyzed. SPSS software version 18.0 (SPSS Inc., Chicago, IL, USA) and independent t‑test have been employed for data analysis. Results : It was demonstrated that the increasing rate of NVD (P = 0.001) and decreasing rate of CS in public hospitals (P = 0.027) after the implementation of the plan were signifcant. On the other hand, the increasing rate of CS in private hospitals was signifcant (P = 0.026). Conclusions : Although this scheme has achieved its targets in public hospitals of Isfahan Province, it has not met its objectives in private hospitals. It seems CS operations were shifted from public hospitals to private ones, which is conflict with the objectives of the health reform plan. Effects of Relative Preparing Program for Continuous Labor Support in Natural Childbirth on Parturients’ Labor Pain and Satisfaction and Nurses’ Satisfaction
2016Co-Authors: Sureeporn Kritcharoen, Pranee Pongpaiboon, Kanjanee Phon-in, Warangkana Chatchawet, Supap MaksuwanAbstract:This quasi-experimental research aimed to assess the effect of a relative preparing program on labor pain in first stage of labor, and satisfaction of parturients, and nurses on continuous labor support given by the relatives of pregnant women. The subjects consisted of 60 pregnant women, 30 experimental group and 30 control group and 14 nurses. The experimental group received relatives training in continuous labor support. The control group received routine ante natal care. Parturients’ pain were measured by visual analog scale, while satisfaction of parturients and nurses were measured by satisfaction questions. The data were analyzed using descriptive statistics and independent t-test. Labor pain of both groups in first stage of labor was not different. The parturients in the experimental group had significantly higher satisfaction than the control group (t=2.214, p < .05). The nurses were more satisfied on the continuous support of relatives of the experimental group than the control group on: massage (t=3.167, p < .01), encourage movement (t=4.463, p < .001), and psycho-social support and providing comfort of the parturients (t=2.366, p < .05). Relative training for continuous labor support in Natural Childbirth is a program that nurses and parturients were satisfied with continuous labor support by relative and should be applied in routine practice for effective caring of Natural Childbirth parturients during labor.

Development of a Clinical Nursing Practice Guideline for Clients with Natural Childbirth

The committee consisted 11 persons to develop clinical practice guideline. The content validity index was tested by 5 experts. The reliability was tested by 6 nurses. The effectiveness of the clinical practice guideline was evaluated in 2 parts: 1) able to practiced and 2) difficulty and simplicities in practice. The satisfaction was evaluated by nurses and laboring women. In addition, the obstacles or the problems of using the clinical guideline were evaluated.The CVI was 0.92 and has a high reliability with internal consistency of 93%. It was reported that using this guideline could be most easily practiced for the third and fourth stages of labor. The nurses were satisfied with this guideline at a high level. The laboring women also reported high satisfaction with receiving care under the guideline. The obstacles to using the guideline were reported by the nurses in 3 aspects: 1) laboring women and their relatives, 2) overload of the task of the nurses, and 3) arrangement of the labor unit. The developmental clinical practice guideline has high validity and reliability. The nurses who used it and the women who received care under the guideline experienced high satisfaction. This guideline should be used for sustaining Natural Childbirth.

The participants were 80 primiparous postpartum women. They were assigned into 2 groups by simple randomization, 40 Natural Childbirth care group (experimental group ) and 40 routine Childbirth care group (control group ). The visual analogue scale was used to measure postpartum perineal pain at 4, 24, and 48 hours postpartum. The maternal-infant attachment questionnaire was used to measure maternal-infant attachment between 24-48 hours postpartum. The maternal-infant attachment questionnaire had Cronbach’s alpha coefficient .80. The data analysis used independent t-test for difference for postpartum perineal pain of mothers between groups at 4, 24, and 48 hours postpartum and mother-infant attachment between groups during 24-48 hours. The results showed that among 3 times postpartum (4 th hour, 24 th hour, and 48 th hour ) the perineal pain in the experimental group was lower than those in the control group significantly (p <0.001). However, there was no different in mother-infant attachment when comparing between groups at 24 and 48 hours. The results of this study indicate that it is better to use the Natural Childbirth care for the women rather than routine Childbirth care in order to lessen postpartum perineal pain. This will enhance the quality of nursing care.

the effects of Natural Childbirth versus normal labor on perineal tear of primiparous women

Materials and methods: The subjects were 80 primiparous women. They were divided into two groups: 40 in an experimental group and 40 in a control group. The experimental group received Natural Childbirth program in 6 aspects: 1) humanized health care, 2) psychosocial and emotional support, 3) avoiding routine interventions, 4) not disturbing the Natural process of birth, 5) use of non-pharmacological pain relief methods, and 6) strong promotion of the Natural process of birth. A questionnaire regarding the perineal tear, was used to collect the data. The data were analyzed by descriptive statistics, t-test, chi-square test, and the Mann-Whitney U test. Results: It was found that 27.50% of women in experimental group had no perineal tear, 32.50% and 40.00% had a first and second degree tear respectively. All of the women in the control group had an episiotomy and had a second degree perineal tear. The proportion of perineal tear among of women in the experimental group was significantly less than among women in the control group (z=5.73, p<0.001). Conclusion: The primiparous women who had a Natural Childbirth without an episiotomy had the proportion of perineal tear significantly less than women in the control group with routine episiotomy.

Effect of psychological intervention on psychological problems in primipara Natural Childbirth
Modern Preventive Medicine, 2012Co-Authors: Zhang Ju-honAbstract:OBJECTIVE To investigate the effect of psychological intervention on psychological problems in primipara Natural Childbirth.METHODS 84 cases of primipara Natural Childbirth hospitalized in our hospital from November 2009 to November 2010 were chosen as the research object.In accordance with the implementation of the intervention,they were divided into two groups,namely the control group and the experimental group.Two groups were all with 42 cases.The scores of self-rating scale for anxiety before and after the implementation of different interventions were taken for comparative analysis.RESULTS In the comparison of Self-Rating Anxiety Scale(SAS) score for the two groups of patients before and after the implementation of the intervention,the results after the intervention of the experimental group were better than the control group.Compared in the somatization factor of the SCL-90 factor,forced factor,and depression factor,the difference was significant,the experimental group was better than the control group,and P﹤0.05,there was statistically significant.CONCLUSION Psychological interventions can be effective in improving maternal psychological state and coping style,which is an effective measure to deal with psychological problems.
