Journal of Educational Administration, 2002Co-Authors: Lance D FusarelliAbstract:This article examines the loosely coupled nature of the US educational system and explores recent systemic reform initiatives designed to improve education through more tightly coupled education policy and practice. The utility and limitations of loose coupling as an Organizational Construct are examined and critiqued. A number of significant forces are exerting ever‐greater pressure on policymakers to more tightly couple US education, including environmental pressures, the emergence of powerful new institutional actors, an emergent institutional capacity, and institutional isomorphism. After reviewing the effectiveness of systemic reform initiatives in several states, the article concludes that education in the USA is moving toward a system of fragmented centralization in which policymakers have greater opportunity to craft more coherent, systemic education policy amidst competing demands for limited resources.

The health care utility model: a novel approach to doing business

The concept is to bypass the major drug makers and to manufacture or subcontract for generic pharmaceuticals that its 1,400-plus member hospitals need. What’s unique about the venture is that it is established as a nonstock, nonprofit 501(c)(4) social welfare organization. The money to run the operation comes from the customers (the hospitals) and philanthropic organizations, not banks or investors in need of a healthy return. The aim is to combine the efficiency of a pro-competitive private enterprise with the equitable mission of a pro–social welfare organization. This new Organizational Construct represents an opportunity that could be adapted and implemented in other areas of the health care landscape

changing physician behavior to improve disease prevention
Preventive Medicine, 1994Co-Authors: Stuart J Cohen, H W Halvorson, C A GosselinkAbstract:Physicians often fail to provide nationally recommended preventive services for their patients. Addressing this, we have reviewed selected literature on changing physician behavior using the Organizational Construct of the "readiness for change" transtheoretical model. This model suggests that behavior evolves through stages from precontemplation, to contemplation, to preparation, to initiation, and to maintenance of change. Traditional continuing medical education may affect knowledge and beliefs, but rarely results in behavior change. However, motivational strategies such as practice feedback reports and influential peers can foster stage change. Successful interventions aimed at physicians preparing for change frequently use an office-system approach that targets not only physicians, but office staff and patients as well. Illustrating how the readiness to change model can guide the design and implementation of interventions, we describe strategies being used in a statewide randomized controlled trial to improve cancer prevention counseling and early detection by primary care physicians. The multistage interventions of Partners for Prevention include support from a medical liability carrier, a motivational videotape, a task-delineated office manual, chart flowsheets, patient activation forms, practice feedback reports, a designated prevention coordinator within each practice and regular telephone calls and office visits by project staff.

mission assurance an operating Construct for the department of defense
2012Co-Authors: Jeffrey R WoodenAbstract:Abstract : There have been a significant amount of studies and papers written on the individual yet interrelated disciplines of Continuity, Crisis Management, Emergency Management, Critical Infrastructure Protection and Pandemic Planning. However, to date none of them have taken a holistic look at covering those critical areas under a single unifying management and Organizational Construct. The proposed Construct; Mission Assurance, would allow for comprehensive and integrative planning, to ensure both mission and personnel are taken care of in the event of natural or man-made catastrophe. This qualitative study utilized a mix-methodology of case study and ground theory to determine if the Mission Assurance Construct could provide for better utilization of resources as well as enhancing disaster and emergency response programs. The results showed that while there are significant cultural, Organizational and in some cases individual biases to overcome, Mission Assurance is a Construct that could allow for enhanced resource utilization and provide for integrative planning in developing a coordinated all-hazards response.
