    Abstract:

    Osmophobia (Os), defined as a perception of odors as unbearable during the headache attacks, has been described since ancient times as an accompanying symptom of migraine. More recently authors demonstrated that Os is much more relevant in migraine patients (M), with (MA) or without aura (MO), than in various primary headaches (PH). It has also been put in evidence that Os is very specific in the differential diagnosis between M and tension-type headache (TTH). Consequently, the 2004 edition of ICHD included in the Appendix (A1.1) Os among the proposed alternative diagnostic criteria of MO. In January 2013, a provisional revised version (ICHD-3 beta) of the previously released classification was offered for the approval of the scientific community. In this version Os disappeared from the Appendix, without any explanation.

    This study was planned to investigate the prevalence of Osmophobia in juvenile headache sufferers and to analyse the diagnostic utility of Osmophobia in order to distinguish migraine without aura from episodic tension-type headache. We examined 305 consecutive patients presenting at our Paediatric Headache Centre. A semistructured questionnaire was given to 275 selected patients affected by migraine or tension-type headache. The prevalence of Osmophobia during attacks was 18.5%, mainly in migraine patients (25.1%) vs. those with tension-type headache (8.3%). Osmophobia showed more specificity than phonophobia or photophobia in the differential diagnosis between migraine and tension-type headache. In conclusion, this study demonstrates that Osmophobia resulted in a symptom with poor sensitivity (27.1%) but high specificity (92%) that could become a supportive diagnostic criterion even in children for the differential diagnosis between migraine without aura and tension-type headache.

    Intolerance to smell is often reported by migraine patients. This study evaluates Osmophobia in connection with the diagnosis of migraine and episodic tension-type headache (ETTH). The characteristics of this symptom are also investigated. We recruited from our Headache Centre 1005 patients (772 female, 233 male; age 37 ± 11 years), of whom 677 were migraine without aura (MoA), 130 migraine with aura (MA) and 198 TTH. Patients with two or more forms of primary headache were excluded. Among migraine patients, 43.9% with MoA and 38.5% with MA reported Osmophobia during the attacks; none of the 198 TTH patients suffered this symptom. Most frequently offending odours were scents (63.9%), food (55.2%) and cigarette smoke (54.8%). Osmophobia appears structurally integrated into the migraine history of the patient. It seems to be a peculiar symptom favouring the diagnosis of migraine (MoA and MA) in the differential diagnosis with ETTH.

Medical records of the children admitted for headache between 1 December 2004 and 31 March 2005 were consecutively studied. A questionnaire was used to find the prevalence of allodynia, Osmophobia and red ear syndrome. We visited 96 children with headache. The range of age was 6–18 years. We classified migraine in 57%, other primary headaches in 25% and secondary headaches in about 18%. The presence of ipsilateral clinical allodynia was 14.5% in migraine, Osmophobia in 20% of migraine and red ear syndrome in about 24% of migraine cases and they were absent in the other two headache groups. Our study shows that features like Osmophobia, allodynia and red ear syndrome are not uncommon in migraine while they are absent in other types of headache.

    Aims: This study was planned to investigate the diagnostic utility of Osmophobia as criterion for migraine without aura (MO) as proposed in the Appendix (A1.1) of the International Classification of Headache Disorders (ICHD-II, 2004).Methods: We analysed 1020 patients presenting at 10 Italian juvenile headache centres, 622 affected by migraine (M) and 328 by tension-type headache (TTH); 70 were affected by headache not elsewhere classified (NEC) in ICHD-II. By using a semi-structured questionnaire, the prevalence of Osmophobia was 26.9%, significantly higher in M than TTH patients (34.6% vs 14.3%).Results: Osmophobia was correlated with: (i) family history of M and Osmophobia; and (ii) other accompanying symptoms of M. By applying these ‘new’ criteria, we found an agreement with the current criteria for the diagnosis of migraine without aura (MO) in 96.2% of cases; 54.3% of previously unclassifiable patients received a ‘new’ diagnosis.Conclusions: In conclusion, this study demonstrates that this new appro...

    The aim of this study was to explore the description of the migraine attack of Pontius Pilate (a character in the novel The Master and Margarita by M. A. Bulgakov). Some of its features are analysed in light of current migraine literature. It is hypothesized that, at least in part, this description is based on the personal experience of the novel's author. We studied and analysed the text of the novel, other works by Bulgakov, his biography, including his medical training and practice, and the recently published diaries of Bulgakov and his wife E. S. Bulgakova. The novel contains a comprehensive description of a migraine attack. It includes a prodrome/aura of Osmophobia. Olfactory perception during or shortly before the migraine attack is altered to the point where neutral or even pleasant odours become unbearable. Bulgakov's extensive history of migraines is seen in his diary, the diary of his wife, letters and other literary works. This is one of the most detailed and extensive depictions of a migraine attack in literature, with Osmophobia described with great emphasis. It is likely that Pilate's migraine is described based on the personal history of the novel's author.

    Objective.—This study attempts to validate the alternative criteria for classification of migraine without aura (International Headache Society [IHS] A1.1) proposed in the appendix of The International Classification of Headache Disorders, 2nd edition. This method uses at least two of the associated symptoms (nausea, vomiting, photophobia, phonophobia, and Osmophobia) in category D of the IHS classification. Background.—In the appendix of The International Classification of Headache Disorders, 2nd edition, an alternative method of classification of migraine without aura is proposed. This method of classification has never been validated. Methods.—A total of 1480 consecutive headache patients in a tertiary care setting were evaluated at first visit. Headache-associated features, such as intensity, lifetime duration, frequency per month, duration, triggers, prodrome, percentage recurrence, and postdrome frequency, were recorded. In addition, medication satisfaction, acute and monthly disability, grading of headache days, sleep normality, mood, and habits were documented. Results.—Of the 1480 patients, 901 were initially classified as having migraine IHS 1.1. Using the proposed alternative method (IHS A1.1), 885 (98.2%) of these patients were reclassified as having migraine. The remaining 16 (1.8%) patients not classified had only nausea and none of the other specified associated symptoms. They also exhibited different characteristics from the IHS migraine population as a whole regarding their headache and other features. Conclusions.—This classification of migraine in a headache center population shows that the proposed use of any two of nausea, vomiting, photophobia, phonophobia, and Osmophobia in category D of the classification may be a valid alternative method of classification. This study also demonstrates that the standard IHS methodology includes a very small group of patients who appear to be different from other migraine patients.

  • Osmophobia and taste abnormality in migraineurs: a tertiary care study.

    Objectives.—This study assesses Osmophobia and taste abnormality for the first time in a large sample of migraine patients. Methods.—Seven hundred and twenty seven migraineurs were evaluated. Osmophobia, taste abnormality, and perfume or odor trigger were graded from 0 to 3. Results.—In patients with data, 24.7% of 673 patients complained of Osmophobia (12.5% occasional, 7% frequent, and 5.2% very frequent) and 24.6% of 505 of taste abnormality (13.5% occasional, 6.1% frequent, and 5.0% very frequent). Perfume or odor trigger of acute migraine occurred in 45.5% of 724 patients (22.7% occasional, 10.2% frequent, and 12.6% very frequent). Perfume or odor trigger was associated with Osmophobia in 61.5% and taste abnormality 62.1%. Osmophobia without taste abnormality occurred in 28.3% and taste abnormality without Osmophobia in 40.3%. A greater percentage of females than males had Osmophobia (25.7 vs. 17.5), taste abnormality (25.4 vs. 17.9), and perfume or odor trigger (49.3 vs. 22.1), all P < .0001. Conclusions.—Osmophobia and taste abnormality occur in about one quarter of migraineurs during an acute migraine attack while perfume or odor trigger migraine in almost 50% of patients. Osmophobia and taste abnormality in the acute migraine attack, as well as perfumes or odor as a migraine trigger, are more common in females than in males.
