To determine if the Teen Outreach Program (TOP), a youth development and service learning Program, can reduce sexual risk-taking behaviors compared with a business as usual or benign counterfactual.Methods. We synthesized results of 5 independent studies conducted in 5 geographically and ethnically diverse locations between 2011 and 2015 with 17 194 middle and high school students. Each study cluster-randomized classes, teachers, or schools to treatment or control groups and included the students enrolled in those clusters at baseline in an intent-to-treat analysis. Multilevel models tested impacts on recent sexual activity, recent unprotected sexual activity, and sexual initiation among the sexually inexperienced at baseline at approximately 1 and 2 years after baseline.Results. Journal of Community Psychology, 2001Co-Authors: Joseph P Allen, Susan PhilliberAbstract:This study examined a highly successful, well-documented, national Program to prevent teenage pregnancy and school failure—the Teen Outreach Program—to address a fundamental question: How well can a developmentally focused, broadly targeted prevention Program address the needs of those students within the Program who are at the highest risk of problematic behavior. The hypothesis that the developmental focus of a broadly targeted intervention would lead it to have greater Program efficacy among those young people who began the Program at greatest risk was examined with multisite data collected on more than 3,300 Teen Outreach and comparison group students. Results confirmed prior findings regarding the overall efficacy of the Teen Outreach Program, and indicated that the Program appeared most effective for those students at greatest initial risk of the problem behaviors being targeted. Implications for the targeting of the Teen Outreach Program specifically and of similar primary prevention Programs more generally are discussed. © 2001 John Wiley & Sons, Inc.

To consider whether Youth All Engaged! (a text message intervention) intensified the effects of the adolescent pregnancy prevention Teen Outreach Program (control) for youths.Methods. In this trial performed in Denver, Colorado, from 2011 to 2014, we randomized 8 Boys & Girls Clubs each of 4 years into 32 clubs per year combinations to ensure each club would serve as a treatment site for 2 years and a control site for 2 years. Control intervention consisted of the Teen Outreach Program only. We enrolled 852 youths (aged 14–18 years), and 632 were retained at follow-up, with analytic samples ranging from 50 to 624 across outcomes. We examined Program costs, and whether the intervention increased condom and contraceptive use, access to care, and pregnancy prevention.Results. Control Program costs were $1184 per participant, and intervention costs were an additional $126 per participant (+10.6%). Loane, Paula ParksAbstract:Abstract Background: Previous studies have shown that asthma education and case management may reduce asthma emergency care, hospitalizations, and expenditures. Objective: We sought to study the effect of an asthma Outreach Program (AOP), a team-based, case-management intervention, on emergency ward (EW) and hospital use. Methods: Fifty-seven patients aged 1 to 15 years with the diagnosis of asthma based on the usual clinical practice criteria who were continuously enrolled in a staff-model health maintenance organization for a period of at least 2 consecutive years were randomized into 2 intervention groups. The control group received a single intensive asthma education intervention, and the AOP group received the same initial education but then was followed-up by an asthma case management nurse throughout the intervention period. Results: EW visits, hospitalizations, and total outside-of-health-plan expenditures (consisting of EW and hospital expenses, as well as miscellaneous costs, such as ambulance, durable medical equipment, tertiary referrals, and home care) were assessed from claims filed for a year before and after enrollment. Control group patients experienced significant reductions in EW visits (39%), hospitalizations (43%), and outside-of-health-plan costs (28%), possibly as a result of the baseline educational intervention received by all enrolled patients, in conjunction with regression to the mean. AOP group patients experienced significant reductions in EW visits, (73%, P = .0002), hospitalizations (84%, P = .0012), and outside-of-health-plan use (82%, P P P P Conclusions: Asthma patients in a staff-model health maintenance organization decreased their resource use between 57% to 75% by participation in an AOP as compared with a randomized control group receiving only an educational intervention. Reduction in Resource Utilization by an Asthma Outreach Program

Loane, Paula ParksAbstract:Objective: To study the effect of an asthma Outreach Program on emergency ward and hospital utilization. Design: Before and after trial. Setting: Ambulatory patients in a staff-model health maintenance organization with a 70% black inner-city population. Patients: Fifty-three patients aged 1 to 17 years with the diagnosis of asthma based on usual clinical practice criteria. Intervention: Asthma emergency ward visits and hospitalizations were monitored over a 6-month to 2-year period after enrollment in the Program. Patients were scheduled for one-on-one orientation visits with the asthma Outreach nurse and instructed in asthma management, medications, triggers, and use of inhalers and peak flowmeters. An individualized step-care treatment Program was outlined for each patient by the nurse, primary care pediatrician, and, when appropriate, an allergist. The Outreach nurse maintained personal or telephone contact with the families on a regular basis to assure understanding of and compliance with the treatment plan. Outcome Measures: Outcome was assessed by measuring annualized emergency ward and hospital admissions for each patient for an equal period before and after entry into the Program. Results: Emergency ward admissions were reduced 79% (from a rate of 72 visits per year to 15 visits per year, P P Conclusions: Patients enrolled in a pediatric asthma Outreach Program experienced a significant reduction in utilization of emergency ward and hospital admissions, resulting in reduced cost of care.
