    AbstractObjective: The purpose of this study is to examine the referrals made to a Pediatric Ophthalmology practice in south Western Ontario. Timing of initiating a referral for children presenting with esotropia and the management offered prior to referral will be examined to identify room for improvement.Methods: Retrospective chart review of 326 children diagnosed with esotropia with an age of onset <5 years. Data collected included the referring doctor (optometrist or physician), age at onset of the disease, age at referral, clinical characteristics of esotropia at presentation, management, and stereopsis outcome.Results: A total of 326 charts were reviewed; 63.7% were referred by optometrists and 36.2% by physicians. The mean delay in referral for all cases was 24.64 ± 25.5 months for optometrists and 17.82 ± 19.9 months for physicians.     The fate of Pediatric Ophthalmology may be in jeopardy. For the past 20 years, there has been declining interest in the field compared to other subspecialties in Ophthalmology, as fellowship positions and jobs remain unfilled. Of those fellows who do match in Pediatric Ophthalmology and strabismus, many are international medical graduates who often return to their native countries to practice, further diminishing the supply of Pediatric ophthalmologists in the United States. In previous surveys, resident graduates have expressed disinterest in the field, reluctance to work with children, inadequate reimbursements, and insufficient interactions with faculty as reasons not to pursue this subspecialty. Millions of people throughout the United States do not have access to Pediatric ophthalmologists, highlighting the issue of unequal distribution. As more Pediatric ophthalmologists retire, there is concern that there will not be enough providers to meet the demands of this subspecialty. Although many of these factors deterring residents from entering this field have been resolved, the major issue of financial reimbursements has not been adequately addressed.     Recent literature reports that patients and parents of Pediatric patients prefer their physician to wear a white coat and to address them informally. This study aims to characterize current practice patterns of Pediatric ophthalmologists regarding their use of white coats and salutations during outpatient Pediatric encounters. An eight-question survey was e-mailed to members of the American Academy of Pediatric Ophthalmology and Strabismus in 2012. The questions focused on clinical setting, use of white coats in out-patient encounters, and preferred language used to address the patient's parents. Surveys not completed in full were excluded from data analysis. Of approximately 1,266 members who received the survey, 606 completed the survey. Five hundred ninety-nine surveys were included in the data analysis. Sixty-three percent of attending physicians and 80% of fellows reported they did not routinely wear white coats while examining outpatient children. Forty-six percent of attending physicians and 48% of fellows addressed the patient's parents as "mom" or "dad". There was no significant association between wearing a white coat and type of practice setting, practice characteristics, or location in a children's hospital for attending physicians or fellows. Contrary to preferences expressed by patients and their parents, a majority of Pediatric ophthalmologists do not routinely wear white coats during Pediatric outpatient examinations.     Abstract:

    The prevalence of myopia is high and increasing. Approximately 5 billion people around the world are expected to be myopic by the year 2050. Methods to slow the progression of myopia and therefore potentially decrease the associated sight-threatening complications have been the subject of a number of investigations. A workshop, sponsored by the United States Food and Drug Administration (FDA) Center for Devices and Radiological Health, American Academy of Ophthalmology, American Academy of Optometry, American Association for Pediatric Ophthalmology and Strabismus, American Optometric Association, American Society of Cataract and Refractive Surgery, and Contact Lens Association of Ophthalmologists, Inc, convened myopia experts from around the world to discuss principles to consider in the design of clinical trials investigating the effectiveness and safety of myopia control devices. Experts discussed parameters such as study endpoints, duration, enrollment criteria, patient-reported outcomes, recruitment, and retention. The discussions among the experts, FDA, and audience members should help to facilitate the development and evaluation of reasonably safe and effective myopia control devices.
