use of Perfluorodecalin during vitrectomy in diabetics
Journal Francais D Ophtalmologie, 1993Co-Authors: A Mathis, V Pagot, A Idder, F MalecazeAbstract:: Thirty eyes in 27 patients were operated for complications of proliferative diabetic retinopathy using a perfluorocarbon liquid, highly purified Perfluorodecalin; 3 patients were lost to follow-up leaving 27 eyes for evaluation. Fourteen eyes presented traction-rhegmatogenous retinal detachment; 10 eyes underwent vitrectomy for traction retinal detachment and 3 eyes for progressive fibrovascular proliferation: in these 13 eyes, dissection of fibrovascular proliferation was complicated by iatrogenic holes with surrounding retinal detachment. The intraoperative use of Perfluorodecalin facilitated flattening of the retina and endophotocoagulation. Perfluorodecalin was removed at the end of the procedure, and intraocular tamponade was provided by silicone oil in 12 cases, and by gas in 15 eyes. Anatomical success was obtained in 21/27 cases with a minimum of 6 months and a maximum of 25 months of follow-up (mean follow-up: 10.9 months). Complications from the intraoperative use of Perfluorodecalin were observed in two cases: one patient had residual droplets of Perfluorodecalin, which were removed during a reoperation; in another patient, small subretinal droplets of Perfluorodecalin were observed postoperatively without apparent toxicity after 22 months of follow-up. Eleven eyes had improved vision, 8 eyes had the same vision, and 8 eyes had decreased vision. The main advantage of intraoperative use of Perfluorodecalin was to allow easy flattening of the retina and effective endophotocoagulation under optimal visual conditions.

Twenty-four patients were operated on for giant retinal tears using Perfluorodecalin and silicone oil tamponade. All patients underwent pars plana vitrectomy, unfolding of the giant retinal tears by Perfluorodecalin, Perfluorodecalin-silicone oil exchange, and endophotocoagulation. The lens was removed in 10 of 14 phakic patients, and encircling scleral buckle was placed in 18 cases. Twenty-three of 24 retinas remained successfully attached with a minimum of 6 months of follow-up. Short-term results of intraoperative use of Perfluorodecalin and silicone oil tamponade in the management of giant retinal tears are encouraging. Perfluorodecalin offers the advantage of low cost compared with other perfluoro-carbon liquids like perfluoro-n-octane. The exchange with silicone oil offers the advantages of easy removal of Perfluorodecalin and absence of posterior slippage of the retinal tear.

Retained Perfluorodecalin after retinal detachment surgery

Retained Perfluorodecalin postoperatively has been reported to be associated with retinal damage and other complications. We report on a case of retained intraocular Perfluorodecalin for eleven postoperative days after retinal detachment surgery with good anatomical and visual outcome after one year follow-up.

The intraoperative use of Perfluorodecalin allowed flattening of the retina and endophotocoagulation to be carried out easily. Perfluorodecalin was removed at the end of the procedure, and intraocular tamponade was provided by silicone oil in 5 eyes and by a long-acting gas in 4 eyes. Anatomic success was obtained in 8/9 cases with a minimum follow-up of six months; 4 eyes had improved vision, 2 had the same vision, and 3 had decreased vision. The main advantage of intraoperative use of perfluordecalin is to allow easy flattening of the retina and effective endophotocoagulation under optimal visual conditions.

Methods Twelve donor dogs were killed and kept under mechanical ventilation for 3 hours. Heart–lung blocs were harvested after retrograde pulmonary hypothermic flush with Perfadex. Left lung grafts were randomly transplanted into 12 weight-matched recipient animals. Animals were divided into 2 groups: control (standard mechanical ventilation, n = 6) and PLV (partial liquid ventilation, n = 6). Forty-five minutes after transplantation, the animals in the PLV group received Perfluorodecalin (15 ml/kg) via orotracheal tube. All animals received volume-controlled ventilation (Fio 2 1.0, PEEP 5 cm H 2 O) over 6 consecutive hours. Thereafter, blood-gas analysis, ventilatory mechanics and hemodynamics were registered at 30-minute intervals. After 6 hours of reperfusion the animals were killed and the transplanted lungs were extracted to obtain the wet/dry weight ratio. Results There were significant differences in pulmonary arterial pressure, which were higher in control group animals ( p 2 than those in the PLV group ( p 2 ( p p Conclusions PLV with Perfluorodecalin yields functional results compatible with life in this model. Nonetheless, pulmonary gas exchange and mechanics were superior after reperfusion in animals given conventional mechanical ventilation up to 6 hours after left lung allotransplantation.
