    Abstract:

    OBJECTIVE: To investigate the clinical features and visual outcome of patients with peripapillary staphyloma. METHODS: The medical records of patients diagnosed as having peripapillary staphyloma at Seoul National University Children's Hospital, Seoul, Korea, between January 1, 1991, and December 31, 2003, were reviewed. Visual acuity and associated ocular and systemic abnormalities were recorded. RESULTS: Nineteen patients (21 eyes) with peripapillary staphyloma were included in the study. The mean age at initial examination was 21 months, and 42% (8/19) of the patients were girls. Two patients (11%) had bilateral peripapillary staphylomas. Only 2 eyes had visual acuity better than 20/200 at the last examination, and 7 eyes had severe myopia of more than -6 diopters. After occlusion therapy, 1 patient achieved a visual improvement from light perception to 20/30. Congenital cataract and Persistent Pupillary Membrane were present in 2 eyes each at initial examination. Complications observed during the follow-up period included acquired total cataract in 2 eyes and retinal detachment in 3 eyes. Scleral encircling buckling was performed in 1 eye with retinal detachment, and subsequently the retina was reattached. Only 2 patients had accompanying systemic abnormalities. CONCLUSIONS: Rarely, patients with peripapillary staphyloma can achieve significant visual improvement by occlusion therapy, although visual outcome is generally poor. Because associated ocular disease and refractive errors are not infrequent, complete ophthalmic examinations and regular follow-up are necessary.

    Abstract:

    To report a case with extensive Persistent Pupillary Membrane treated by sequential argon-YAG laser Membranectomy. A 33-year-old female had a prominent Persistent Pupillary Membrane obstructing the visual axis of the right eye, thereby preventing measurement of refraction even after mydriasis. Her uncorrected vision was 20/400. Firstly, the argon laser was used to photocoagulate the part of the Membrane adhering to the iris surface and so to prevent any bleeding caused by the YAG laser. Secondly the YAG laser was used to photodisrupt the Membrane remnant attached to the iris. Mild pigment dispersion was found in the anterior chamber, without associated microhemorrhages after the laser treatments. Spontaneous reabsorption of the free floating Membrane occurred 1 week after the Membranectomy. After an 8-year follow-up period, her right corrected visual acuity increased to 20/40 with high myopia. No ocular complications or Membrane recurrence were noted. Sequential argon-YAG laser Membranectomy may effectively be used to treat Persistent Pupillary Membrane without any long-term risk of major side effects.

    Abstract:

    We report a case of a Persistent Pupillary Membrane (PPM) associated with a cataract managed by sequential argon-YAG laser Membranectomy and phacoemulsification. A 30-year-old woman had an extensive PPM and anterior capsular opacity of the lens in the right eye. Best corrected visual acuity (BCVA) was 20/60. Argon laser photocoagulation was performed at the junction of the PPM and the iris collarette. Subsequently, a neodymium:YAG laser was used to sever the strands of the PPM. These laser procedures resulted in shrinkage of the PPM without hemorrhage in the iris or other complications. The patient had clear corneal sutureless phacoemulsification uneventfully on the next day. The BCVA improved to 20/25 3 months following the treatment. We conclude that combined sequential argon-YAG laser Membranectomy and modern phacoemulsification surgery can effectively be used to treat PPM associated with cataract without bleeding of the iris or other major side effects.

    Universal ocular screening of infants is not a standard procedure in children’s health care system in China. This pilot study investigated prevalence of ocular abnormalities of 6 weeks-age infants using wide-field digital imaging system. Infants aged 6 weeks around were consecutively enrolled in a public hospital between April 2015 and August 2016. All the infants who were enrolled in the study underwent vision assessment, eye position examination, external eye check, Pupillary light reflex, red reflex examination, anterior and posterior ocular segments were examined using flashlight, ophthalmoscope, and wide-field digital imaging system. A total of 481 infants at 45.1 ± 6.1 days after birth were enrolled in the study. 198 infants had abnormal findings (41.2%). Retinal white spots and retinal white areas were the most common findings (42.9% of abnormalities and 17.7% of all infants screened). The second major finding was retinal hemorrhage (16.2% of abnormalities and 6.7% of all infants screened). Other abnormal findings include retinal pigmentation, concomitant exotropia, neonatal dacryocystitis, retinopathy of prematurity, ‘albinism-like fundus’, congenital nasolacrimal duct obstruction, familial exudative vitreoretinopathy, immature retina, corneal dermoid tumor, large physiologic cupping of optic disc, congenital Persistent Pupillary Membrane, entropion trichiasis, subconjunctival hemorrhage, congenital cataract, vitreous hemorrhage, ptosis and choroidal nevus. Intervention of any form was required in 22 infants, which accounted for 11.1% of abnormalities detected and 4.6% of all infants screened. Universal ocular screening is not only necessary for preterm infants but also for full-term infants. Addition of red reflex examination with wide-field digital imaging system can enhance the sensitivity of screening for ocular fundus abnormities. Further study with a long-term follow-up is needed in the future.

    Purpose: To evaluate the. long-term visual outcome of eyes with hyperplastic Persistent Pupillary Membrane. Patients and Methods: We adopted a retrospective approach involving 39 eyes of 24 Korean patients diagnosed as having hyperplastic Persistent Pupillary Membrane, excluding patients who could not be observed beyond 3 years of age and eyes that had any other ocular anomalies that affect vision. Final best-corrected visual acuity and refractive status were compared according to treatment type and laterality. Mean visual acuity was transformed to the logarithm of the minimum angle of resolution (logMAR). Results: The mean final best-corrected visual acuity was 0.32 (standard deviation, ′ 0.41). Five eyes had a best-corrected visual acuity less than 20/70 at the last follow-up. Four eyes were unilateral (deprivation amblyopia) and one eye was bilateral (anisometropic amblyopia). There were no significant differences in final best-corrected visual acuity according to treatment type or laterality. Significant anisometropia was found in 11 of the 24 patients. The mean absolute value of anisometropia was greater in the surgical treatment group than in the medical treatment and observation groups (P = .048). Conclusions: The visual prognoses for individuals with hyperplastic Persistent Pupillary Membrane can be relatively good if the condition is appropriately managed. Unilaterality and poor initial visual acuity are indicators of a poor visual outcome. Surgery is effective when the opaque Membrane retards visual maturation. Close follow-up with evaluation of visual acuity and refractive status is mandatory because this condition can lead to deprivation amblyopia and anisometropic amblyopia.

  • Results of Surgical Treatment for the Children with Persistent Pupillary Membrane

    Purpose: To evaluate the long-term results of surgical treatment for the children with Persistent Pupillary Membrane. Method: A retrospective study comprised of 12 eyes of 11 Korean patients who underwent surgical removal of Persistent Pupillary Membrane who could be followed up beyond 4 years of age, excluding eyes with any other vision-affecting ocular anomalies. The preoperative and final visual acuity, age at surgery, final refractive status, surgical complication and combined ocular abnormality were reviewed. Mean visual acuity was calculated using logarithm of minimum angle of resolution (logMAR). Results: Mean final BCVA was 20/257 (logMAR 1.110.68) in the unilateral 5 eyes and 20/33 (logMAR 0.220.25) in the bilateral 7 eyes, and there was significant difference (p=0.03).Of the 12 eyes, five eyes were under 0.3 at the last follow-up. Four eyes were unilateral (deprivation amblyopia) and one eye was bilateral (anisometropic amblyopia). Significant anisometropia was found in 7 of the 11 patients. One mild capsular opacity occurred as a postoperative complication. Conclusions: The visual prognoses for eyes with surgically removed Persistent Pupillary Membranes were relatively good if they were bilateral. However, they showed poor visual outcome if they were unilateral in this study, so the early surgical intervention and aggressive management will be helpful if the Persistent Pupillary Membrane is unilateral. Close follow-up with evaluation of visual acuity and refractive status is mandatory because this condition can lead to deprivation amblyopia and anisometropic amblyopia.
