    Abstract The present study was carried out to determine the feasibility of using gel formulations for the transdermal delivery of triamcinolone acetonide (TA), which is one of the synthetic glucocorticoids, in conjunction with Phonophoresis, and to develop the carbopol gels of TA. For this purpose, the anti-inflammatory effects of the gel containing TA after the adoption of ultrasound were evaluated by investigating the in vivo change in the serum creatine phosphokinase (CPK) and histological findings. Following a muscle injury, the serum CPK activity decreased significantly in the TA gel group with Phonophoresis, comparing with that in the control group and the commercial gel group given ultrasound. In the gross finding, after a muscle injury, the TA gel group with Phonophoresis showed rapid moderation of the injury compared with the three other groups. The histological findings showed that the inflammation was relieved within 72 h after the injury from the TA gel group with Phonophoresis. These effects were considerably higher in the Phonophoresis group than in the other three groups. Overall, a TA gel using Phonophoresis might be used as a new transdermal delivery technique providing enhanced anti-inflammatory effects.

    Abstract The purpose of this study was to compare the treatment efficacy of topical application of hydrocortisone, therapeutic ultrasound (US) and Phonophoresis on the rat′s Achilles tendon (tendo calcaneus) repair process after tenotomy. The two treated groups with US were made in a pulsed mode. The irradiation of US was performed at a frequency of 1 MHz and an intensity of 0.5 W/cm 2 (SATA), for 5 min each session. The tendons were analyzed using the polarized light microscopy. The results showed that the treated group with the topical application of hydrocortisone has not been delivered transdermally and that the molecule of collagen responds to the ultrasonic stimulation. The treatment with Phonophoresis was the more efficient method. These findings allow us to conclude that the US stimulates the acceleration of tissue repair processes and induces the transdermal delivery of hydrocortisone in a therapeutic concentration on the tendon.     Background Subacromial impingement syndrome (SIS) is defined as the mechanical compression of subacromial structures between the coraco-acromial arch and the humerus during active elevation of the arm above shoulder height. Objectives The aim of the study was to investigate the effects of applications of iontophoresis and Phonophoresis, which has been used in addition to physiotherapy program on pain and disability level in patients with SIS. Methods The study was conducted with subjects (n=43) diagnosed with SIS. The subjects were divided into two groups randomly as iontophoresis group (IG) and Phonophoresis group (PG). Diclofenac sodium iontophoresis was applied to the first group (n=22), diclofenac sodium Phonophoresis was applied to the second group (n=21). In addition all of groups therapy program includes superficial heat, transcutaneous electrical nerve stimulation and exercise. The groups were evaluated before and after treatment in terms of pain and scores of shoulder disability. Visual analog scale was used to assess of pain (night pain, rest pain, pain with motion and pain in functional use), Shoulder Disability Questionnaire (SDQ) and Disabilities of the Arm Shoulder and Hand (DASH) were used to assess of shoulder disability. Results 43 patients with IS aged 18 to 65 years were recruited to the study. Demographic data were similar in the groups.     Abstract The aim of our study was to compare topical use of Aloe vera gel, pulsed mode ultrasound (US) and Aloe vera Phonophoresis on rat paw with collagenase-induced tendinitis. Edema size, tensile tendon strength, tendon elasticity, number of inflammatory cells and tissue histology were studied at 7 and 14 days after tendinitis induction. Pulse mode US parameters were: 1 MHz frequency, 100 Hz repetition rate, 10% duty cycle, and 0.5 W/cm 2 intensity, applied for 2 min each session. A 0.5 mL of Aloe vera gel at 2% concentration was applied for 2 min per session, topically and by Phonophoresis. Topical application of Aloe vera gel did not show any statistically significant improvement in the inflammatory process, whereas Phonophoresis enhanced the gel action reducing edema and number of inflammatory cells, promoting the rearrangement of collagen fibers and promoting also the recovery of the tensile strength and elasticity of the inflamed tendon to recover their normal pre-injury status. Results seem to indicate that Aloe vera Phonophoresis is a promising technique for tendinitis treatment, without the adverse effect provoked by systemic anti-inflammatory drugs.
