Methods The workload data and injury incidence of 32 players were monitored throughout 2 seasons. Multiple regression was used to compare cumulative (1, 2, 3 and 4-weekly) loads and acute:chronic (A:C) workload ratios (acute workload divided by chronic workload) between injured and non-injured players for specific GPS and accelerometer-derived variables:total distance (TD), high-speed distance (HSD), accelerations (ACC) and total load. Workloads were classified into discrete ranges by z-scores and the relative risk was determined. Results A very high number of ACC (≥9254) over 3 weeks was associated with the highest significant overall (relative risk (RR)=3.84) and non-contact injury risk (RR=5.11). Non-contact injury risk was significantly increased when a high acute HSD was combined with low chronic HSD (RR=2.55), but not with high chronic HSD (RR=0.47). Contact injury risk was greatest when A:C TD and ACC ratios were very high (1.76 and 1.77, respectively) (RR=4.98). Conclusions In general, higher accumulated and acute workloads were associated with a greater injury risk. However, progressive increases in chronic workload may develop the players9 Physical Tolerance to higher acute loads and resilience to injury risk.

USE OF MELDONIUM IN THE TREATMENT OF PATIENTS WITH CORONARY ARTERY DISEASE AND CONCOMITANT ARTERIAL HYPERTENSION
EUREKA: Health Sciences, 2019Co-Authors: Denys A Volynskyi, Ihor VakaliukAbstract:Coronary artery disease (CAD) remains one of the leading causes of mortality and disability in Ukraine. Arterial hypertension (AH) is one of the most common diseases and a leading risk factor for coronary artery disease. The aim of the work is to evaluate the antianginal activity of meldonium in the complex therapy in patients with CAD with stable angina and concomitant AH. Materials and methods. The study included 82 patients with CAD, stable angina pectoris II–III functional class, including 52 patients with concomitant AH stage II. The patients were divided into 2 groups. Patients in group 1 were prescribed meldonium at a dose of 750 mg/d for 2 months in addition to basic therapy for the underlying disease. Patients in group 2 continued basic antianginal, disaggregant, hypolipidemic therapy. Results . The use of meldonium led to a decrease in the frequency of angina attacks and the need for nitroglycerin. From the 1st month of therapy and up to 2 months treatment decreased it consumption by 63 and 82.3 % respectively. Adding meldonium to basic therapy led to a likely reduction in shortness of breath, episodes of palpitations, tinnitus, and headache. In all patients, after the treatment, an increase in exercise Tolerance was observed, which was more pronounced in the group where patients were receiving meldonium. In the group of patients receiving meldonium, normalisation of blood pressure was faster and more pronounced. Conclusions . Meldonium has antianginal activity, which is manifested by an increase in the Physical Tolerance of patients, a decrease in the frequency of angina attacks, the need for sublingual nitroglycerin intake and improvement in the well-being of patients. Additional use of meldonium promotes faster and better normalization of blood pressure. The use of meldonium in the complex therapy of patients with stable angina and concomitant AH allows to increase the effectiveness of traditional antianginal therapy and to improve the quality of life of such patients.

Acta Cardiologica, 2007Co-Authors: Hamayak Sisakian, Adrina Torgomyan, Anush BarkhudaryanAbstract:OBJECTIVE Trimetazidine (TMZ) is the first of novel antianginal drugs with a cardioprotective effect, selectively inhibiting mitochondrial long-chain 3-ketoacyl coenzyme A thiolase. This study tested the hypothesis that the cytoprotective beneficial effect of this agent can lead to the improvement of left ventricular (LV) systolic function and Tolerance to Physical activity in patients with ischaemic cardiomyopathy. METHODS AND RESULTS In 82 consecutive patients with ischaemic cardiomyopathy, a subgroup of patients (n = 42) was assigned to receive a modified form of TMZ (35 mg twice daily) in addition to the conventional therapy for the duration of three months. All patients underwent clinical, echocardiographic examination and a six-minute walk test at baseline and after a three-month treatment. The therapy with TMZ significantly improved the functional class in these patients. Left ventricular ejection fraction (LVEF) increased by 3.5 +/- 6.72% (from 34.5 +/- 3.8% to 38.0 +/- 4.8%) in the TMZ group vs. 0.8 +/- 8.06% (from 32.4 +/- 5.6% to 33.2 +/- 5.8%) in the control group (P = 0.05). The Tolerance to Physical activity improved by 30.0 +/- 20.7 m in the TMZ group (from 215 +/- 17.5 m to 245 +/- 20.7 m) vs. 2.0 +/- 18.85 m (from 208.2 +/- 12.4 m to 210.2 +/- 14.2 m) in the control group (P < 0.001). CONCLUSIONS A therapeutic intervention with TMZ in conjunction with the standard therapy, over a three-month period, is associated with an increase in LVEF and improved Tolerance to Physical activity in patients with ischaemic cardiomyopathy.
