  • Pilonidal Sinus Carcinoma found in a Pilonidal Sinus – a surprise diagnosis from the pathologist

    Abstract:

    Introduction: Malignancy arising in long standing chronic infection is known in ulcerative colitis and non healing burns scars. It is exquisitely rare in long standing Pilonidal Sinus infection. We report a male case with the incidental finding of a SCC in a 20 year old untreated Pilonidal Sinus. Methods: Decription of the diagnostic and therapeutic course till his demise of reported world case #125. Results: A 59 yr. old male presented with extensive fistulous disease in the presacral area with induration and reddening. On surgery, tracts were found to extend into the perianal up to the perineum. Pathology specimen analysis revealed SCC with involvement of the PSD tracts with R1 situation (pT2pN0M0G2R1). Despite consecutive wide excision and tumor board advised radiochemotherapy the patient recurred twice locally, in lymph nodes and in the ischorectal fossa and demised 3 years later. Conclusion: Despite best surgical and oncological practice, the patient demised due to a – if defined by the skin SCC classification – low TNM stage carcinoma. Pilonidal carcinoma is a exquisitely rare disease which most surgeons have not experienced in their professional lifetime. As earlier carcinomas may arise within the Pilonidal Sinus system, all histopathological specimen should be either opened and meticulously inspected by the surgeon himself, or sent for routine histopathological examination. Despite early diagnosis, aggressive therapy may change the otherwise dismal course of disease.

    Abstract:

    PURPOSE: To study potential benefits of postoperative epilation after Pilonidal Sinus surgery, the long-term effect of hair removal on Pilonidal recurrence was investigated. METHODS: A total of 1,960 patients with Pilonidal Sinus treated surgically from 1980 to 1996 in three hospitals of the Deutsche Bundeswehr were eligible for the study. Regular hair removal with a razor was recommended for all patients after surgery. A randomly selected sample of 504 patients was contacted for a follow-up telephone interview. RESULTS: The mean follow-up time was 11.3 (standard deviation, 6.4) years. Overall, Pilonidal Sinus disease recurred in 111 (22 percent) of the 504 patients. A total of 113 patients followed the recommendation to perform epilation (mean duration, 7.5 months), and 391 patients did not. Recurrence was observed in 30.1 percent (34/113) of patients who performed postoperative epilation and in 19.7 percent (77/391) of patients who did not perform postoperative epilation (P = 0.01). CONCLUSIONS: Razor hair removal increases the rate of long-term recurrence after surgery for Pilonidal Sinus disease and therefore should not be recommended. However, the rationale for hair removal in Pilonidal Sinus disease is compelling. Other epilation techniques such as laser hair removal should be investigated in appropriate studies.

  • Nd-YAG laser treatment of primary and recurrent Pilonidal Sinus

    A Pilonidal Sinus usually contains hair and extensive vascularization due to inflammation, which theoretically gives Nd-YAG laser waves the potential to destroy the deep fistula systems of the Sinus without affecting the overlying skin. We describe our preliminary experience with Nd-YAG laser waves in the treatment of Pilonidal Sinus. A total of 41 consecutive patients with Pilonidal Sinus underwent the intended Nd-YAG laser treatment. Structured telephone interviews were carried out in 37 patients (90%). The median duration of treatment and observation time were 5.1 months and 15.2 months, respectively. Three patients (7%) received additional surgery: two had granulomas removed via a minor excision, and one requested more traditional surgery. At follow-up, 28 out of the 37 patients (75.7%) reported freedom from symptoms without supplementary treatment, and 30 (81%) were symptom-free after supplementary laser treatment and minimal surgery, if needed. We have identified a potential treatment for Pilonidal Sinus that is extremely gentle compared with conventional surgery. We aim to substantially increase the cohort of patients undergoing this treatment and follow up these patients with a register-based technique with a comparable control group. The Nd-YAG laser may prove to be an attractive treatment alternative for Pilonidal Sinus.
