Verrugas Plantares: caracterización de los virus causales y aplicación del láser 1064 nm a su tratamiento

Se han descrito diferentes genotipos asociados a estas lesiones si bien existen pocos estudios centrados exclusivamente en los genotipos de las verrugas Plantares. El diagnóstico generalmente es clínico, aunque actualmente existen métodos diagnósticos basados en la detección del ADN viral como es el caso de la PCR (polymerase chain reaction). Las modalidades de tratamiento de las verrugas son muchas. Incluyen tratamientos tópicos, crioterapia, quirúrgicos, infiltraciones intralesionales de citotóxicos y la posibilidad de tratamientos combinados. El tratamiento de las verrugas mediante láser se basa en la elevación exógena de la temperatura del tejido (hipertermia local), ya que está comprobado que el tejido infectado es más sensible a los efectos de la temperatura elevada que el tejido normal. En esta tesis se identifican los genotipos del VPH de las verrugas Plantares, y se describe su relación con las características demográficas y clínicas de los pacientes. También se evalúa la eficacia de su tratamiento mediante el láser de 1064 nm de longitud de onda, valorando su tiempo de curación así como la tolerabilidad del tratamiento en estudio. MATERIAL Y MÉTODOS: La población de estudio fueron 72 pacientes diagnosticados de verrugas Plantares de la Unitat de Láser del Hospital Podològico de la Universitat de Barcelona. Las muestras fueron recogidas previo corte laminar de la hiperqueratosis profunda de la verruga Plantar, mediante bisturí para realizar la extracción del DNA. Se realizó la amplifición de una región conservada del gen L1 de VPH mediante La reacción en cadena de la polimerasa para secuenciar los amplicones del DNA e identificar los tipos de VPH. Se determinaron unos parámetros finales de pulso, pausa, potencia y fluencia del láser finalmente que fueron objeto de estudio en la valoración de la efectividad de tratamiento óptima y evitando el dolor para el paciente en el tratamiento de las verrugas Plantares. RESULTADOS: Los genotipos más prevalentes entre las 105 muestras de verrugas Plantares analizadas fueron VPH- 57 (37.1%), VPH-27 (23.8%), VPH-1a (20.9%), VPH-2 (15.2%) y VPH-65 (2.8%). La mayoría de los pacientes (78%) presentaron solo una verruga Plantar, mientras que en el 22,2% de los pacientes se detectaron múltiples verrugas. Un paciente con múltiples verrugas presentava dos generos de VPH diferentes, lo que sugiere la diseminación de las verrugas por autoinoculación además de la infección de novo. No se hallaron diferencias significativas entre el número de verrugas en dedos, mediopie y retropie. El género más prevalente en todas lás zonas fue el género alfa. En el tratamiento con láser de 1064 nm de longitud de onda, aplicando los parámetros finales, el 26,66% de las verrugas con un diámetro igual o inferior a 4 mms sólo precisó de una sesión de láser para su eliminación, el mismo porcentaje precisó de dos sesiones, el 40% de estas verrugas precisaron de 3 sesiones de láser y el 6,68% de cuatro sesiones. El 11,53% de las verrugas con un diámetro superior a 4 mm sólo precisó de una sesión de láser para su eliminación, el 23,07% de estas verrugas precisaron de dos sesiones, el mismo porcentaje precisó de tres sesiones y el 19,23% de cinco sesiones. CONCLUSIONES: La determinación de las secuencias de los fragmentos amplificados mostró que el género alfa, con el genotipo 27 y 57 fue el más prevalente en las verrugas Plantares. Los individuos con múltiples verrugas Plantares pueden presentar vía de transmisión tanto por autoinoculación como por infección ex-novo. La localización de las verrugas en la planta del pie es coincidente con los puntos de máxima carga durante la deambulación, tanto en superficie como en tiempo de apoyo. El género alfa, es el más coincidente con este patrón de apoyo. One patient with multiple Warts presented HPV types from two different genera, suggesting the spread of Warts by self-inoculation as well as by de novo infection. No significant differences between the number of Warts in toes, midfoot and heel were found. The most prevalent HPV types detected in all areas belonged to the alpha genus. Laser treatment was applied using optimized parameters: 26.7% of Warts with a diameter ≤4 mm required only one laser session for removal, 26.7% needed two sessions, 40.0% required 3 laser sessions and 6.7% four sessions; 11.5% of Warts with a diameter >4 mm required only one laser session for removal, 23.1% required two sessions, 23.1% required three sessions and 19.2% five sessions. CONCLUSIONS: This work provides new insight on Plantar Warts and their associated HPV genotypes, and evidences the usefulness and reliability of both the sample collection procedure and the PCR method used for HPV detection and typing. However, it is not known whether or not HPV-60 infection causes the other cutaneous lesions. Observations: Six Plantar Warts were found in five patients. These Warts were found either with concomitant Plantar cysts (four patients) or without an accompanying cyst (one patient). All Warts showed a similar clinical appearance, including a smooth and slightly elevated surface and the retained appearance of the dermal ridges. Their histopathologic features were identical to those found in the wall of the cysts reported previously, namely hyperkeratosis with vacu- olated structure with or without nuclei in the horny layer and amorphous eosinophilic cytoplasmic inclusions in the cells with peripherally located nuclei in the granular and prickle cell layers. By Southern blot analysis, all Warts were found to harbor HPV-60 DNA. In addition, immunohistochemistry and in situ hybridization analysis revealed the presence of viral capsid antigen and viral genome in these lesions, respectively. Conclusion: We conclude that HPV-60 infection on the Plantar skin causes the Warty lesion, designated as ridged Wart , distinct from those infected with other cutaneous human papillomaviruses. A 37-year-old woman with a background of HIV presented with a large verrucous plaque involving her right foot. Human papillomavirus (HPV)-66 was identified in the lesional skin biopsy sample and in scrapings obtained from her cervix. The Wart rapidly responded to topical cidofovir therapy. HPV-66 is a novel HPV type to be associated with verruca vulgaris. Topical cidofovir should be further investigated as an alternative treatment modality for verruca vulgaris.

The effect of patients’ preference on outcome in the evert cryotherapy versus salicylic acid for the treatment of Plantar Warts (verruca) trial

One such bias is the effect of patient\u27s preference on outcome measures. The aims of this study were to examine whether patients\u27 treatment preference affected clearance of Plantar Warts and explore whether there were any associations between patients\u27 treatment preference and baseline variables in the EverT trial. Methods: Two hundred and forty patients were recruited from University podiatry schools, NHS podiatry clinics and primary care. Patients were aged 12 years and over and had at least one Plantar Wart which was suitable for treatment with salicylic acid and cryotherapy. Patients were asked their treatment preference prior to randomisation. The Kruskal-Wallis test was performed to test the association between preference group and continuous baseline variables. The Fisher\u27s exact test was performed to test the association between preference group and categorical baseline variables. A logistic regression analysis was undertaken with verruca clearance (yes or no) as the dependent variable and treatment, age, type of verruca, previous treatment, treatment preference as independent variables. Two analyses were undertaken, one using the health professional reported outcome and one using the patient\u27s self reported outcomes. Data on whether the patient found it necessary to stop the treatment to which they had been allocated and whether they started another treatment were summarised by treatment group. Results: Pre-randomisation preferences were: 10% for salicylic acid; 42% for cryotherapy and 48% no treatment preference. There was no evidence of an association between treatment preference group and either patient (p=0.95) or healthcare professional (p=0.46) reported verruca clearance rates. There was no evidence of an association between preference group and any of the baseline variables except gender, with more females expressing a preference for salicylic acid (p=0.004). There was no evidence that the number of times salicylic acid was applied was different between the preference groups at one week (p=0.89) or at three weeks (p=0.24). Patient self treatment with 50% salicylic acid (Verrugon) daily up to a maximum of eight weeks. Main outcome measures Complete clearance of all Plantar Warts at 12 weeks. Secondary outcomes were (a) complete clearance of all Plantar Warts at 12 weeks controlling for age, whether the Wart had been treated previously, and type of Wart, (b) patient self reported clearance of Plantar Warts at six months, (c) time to clearance of Plantar Wart, (d) number of Plantar Warts at 12 weeks, and (e) patient satisfaction with the treatment. Results There was no evidence of a difference between the salicylic acid and cryotherapy groups in the proportions of participants with complete clearance of all Plantar Warts at 12 weeks (17/119 (14%) v 15/110 (14%), difference 0.65% (95% CI ¿8.33 to 9.63), P=0.89). The results did not change when the analysis was repeated but with adjustment for age, whether the Wart had been treated previously, and type of Plantar Wart or for patients¿ preferences at baseline. There was no evidence of a difference between the salicylic acid and cryotherapy groups in self reported clearance of Plantar Warts at six months (29/95 (31%) v 33/98 (34%), difference ¿3.15% (¿16.31 to 10.02), P=0.64) or in time to clearance (hazard ratio 0.80 (95% CI 0.51 to 1.25), P=0.33). There was also no evidence of a difference in the number of Plantar Warts at 12 weeks (incident rate ratio 1.08 (0.81 to 1.43), P=0.62). 'Universidade da Coruna', 2016Co-Authors: Sánchez Gómez Rubén, Flores Olavarría MacarenaAbstract:[Resumen] La crioterapia y el láser son dos de las técnicas físicas más usadas actualmente para el tratamiento no quirúrgico del virus del papiloma humano causante de las verrugas Plantares (verruca pedis).El objetivo del presente estudio fue comparar estas dos técnicas para evaluar cuál es la que mejor tasa de curación posee en el tratamiento de la verruga Plantar.Se realizó una búsqueda bibliográfica en el portal científico PubMed, con los términos “Plantar Wart”, “verruca Plantar” y “Plantar papilloma” en combinación con los descriptores “cryotherapy”, y “laser”, que engloban tanto los tratamientos únicos mediante estas dos técnicas como los combinados con otros, tales como el ácido salicílico y la fotodinámica, acotando los años de búsqueda al periodo que comprende desde el 1 de enero de 2005 a octubre de 2015.De los resultados obtenidos, los 19 artículos que recogen tasas de curación muestran la superioridad terapéutica del láser frente a la crioterapia en el tratamiento de la verruga Plantar, siendo las tasas de curación más elevadas en el láser YAG de Erbio combinado con fotodinámica y en el láser de dióxido de carbono.Por tanto, con el presente estudio queda demostrada la mayor tasa de curación del láser frente a la crioterapia para el tratamiento de las verrugas Plantares.[Abstract] Cryotherapy and laser are currently two of the most used techniques for non-surgical treatment of human papilloma virus that causes Plantar Warts.The aim of this study was to compare and evaluate which of the two techniques mentioned above have a higher heal rate in the treatment of verruca Plantar.A bibliographic search was done in the scientific website PubMed, using the terms: “Plantar Wart”, “Plantar verruca” and “Plantar papilloma” combined with the following descriptors: “cryotherapy”, and “laser”. These descriptors encompass both, the unique treatments through these two techniques and the combination with others, such as the salicylic acid and photodynamic, narrowing the research to the period ranging from 1st January 2005 to October 2015.The results obtained from the 19 articles which collect cure rates, show that regarding the treatment of verruca Plantar, laser is therapeutically better compared to cryotherapy. Being Erbium Lasers YAG combined with photodynamic and Carbon dioxide lasers the treatment with the highest rates of cure.Therefore, taking into account the rates of healing, the study shows the potential of laser against cryotherapy for the treatment of verruca planta
