Should we treat mild subclinical/mild hyperthyroidism? Yes

WiersingaAbstract:Abstract Whether or not subclinical hyperthyroidism (SCH) should be treated, remains a matter of debate because there are no randomized clinical trials answering the question if treatment improves long-term health outcomes. Cross-sectional and longitudinal population-based sudies demonstrate clear associations between SCH and risk on atrial fibrillation and fractures; associations with cardiovascular and overall mortality and mental health are less consistent. Associations with adverse outcomes of cardiovascular and bone health are in all likelihood causally related to SCH in view of an observed dose–response relationship and the existence of a plausible biologic mechanism. Short-term intervention studies show improvement of surrogate outcome measurements. Against this background treatment of both endogenous and exogenous SCH is recommended when TSH is 0.1 mU/l or lower. At TSH values > 0.1– 65 years, Postmenopause, Osteoporosis, cardiac disease).

2013Co-Authors: Artesya MeidiyanaAbstract:Latar Belakang: Radiografik panoramik digunakan untuk mengidentifikasi kelainan struktur tulang rahang dan berbagai penyakit rongga mulut serta manifestasi penyakit sistemik di rongga mulut. Ketebalan korteks mandibula dapat diukur dengan menggunakan metode morphometric yaitu Antegonial Index (AI). AI didapat dengan mengukur ketebalan korteks mandibula pada garis tegak lurus terhadap kortikal mandibula dengan garis singgung perbatasan anterior ramus. Penurunan ketebalan korteks mandibula merupakan indikasi adanya penurunan massa tulang wanita Postmenopause dengan Osteoporosis. Tujuan: Studi ini bertujuan untuk mengetahui besar AI wanita Postmenopause pada radiografik panoramik. Metode: Pada penelitian ini, dilakukan DXA pada 36 wanita Postmenopause dan didapatkan 18 subjek wanita Postmenopause Osteoporosis dan 18 subjek wanita Postmenopause tanpa Osteoporosis. Kemudian seluruh subjek difoto panoramik dan dilakukan pengukuran AI. Hasil: Besar ketebalan korteks mandibula wanita Postmenopause dengan Osteoporosis adalah 3,48 mm dan wanita Postmenopause tanpa Osteoporosis adalah 4,2 mm. Kesimpulan: Wanita Postmenopause dengan Osteoporosis memiliki nilai AI yang lebih kecil daripada wanita Postmenopause tanpa Osteoporosis.

Karena biaya dan alat pemeriksaan DXA mahal dan terbatas di Indonesia, dicari pemeriksaan alternatif untuk mendeteksi Osteoporosis dini. Penelitian dengan menggunakan mental index (MI) telah dilakukan dan terbukti signifikan untuk mendeteksi Osteoporosis dini. Penelitian mengenai pengukuran mental index pada wanita Postmenopause dengan Osteoporosis di Indonesia belum pernah dilakukan. Tujuan : Mengetahui besar pengukuran mental index pada wanita Postmenopause dengan radiografik panoramik. Metode : 32 foto radiografik panoramik dari wanita Postmenopause dengan Osteoporosis dan tanpa Osteoporosis diamati. Ketebalan korteks mandibula dihitung menggunakan mental index pada kedua sisi mandibula. Rata-rata nilai mental index pada kedua grup tersebut dianalisa dengan menggunakan Tes Kolmogrov Smirnov, Oneway Anova dan Independent-T. Hasil : Terdapat perbedaan signifikan antara nilai MI pada wanita Postmenopause Osteoporosis dengan tanpa Osteoporosis (p<0.05). Kesimpulan : Nilai Mental index pada wanita Postmenopause Osteoporosis adalah 4,2 mm dan tanpa Osteoporosis adalah 4,9 mm.

Chinese Journal of Osteoporosis, 2002Co-Authors: Liu ZhonghouAbstract:China has about one fifth of the world population and this is a large number.The year 2000 census revealed that the total population in China was 1,295,330,000.People over 65 years old reached 88,110,000,6.96 per cent of the total population.To know this large population number is very important to the bone and mineral research and the prevention of Osteoporosis.With reference to the Osteoporosis diagnostic criteria for white women by World Health Organization(WHO)in 1994 and the Osteoporosis diagnostic guidelines by Japanese Bone Mineral Society,and based on the abundant data of average bone density in Hip neck and L 2-4 measured by DEXA in recent 5 years and more than 40,000 data of bone density in forearm 1/3 by SPA in 1980's,the diagnostic guideline for our Chinese people are suggested as follows:Compared with the local peak bone density of the same sex,bone loss rate between 1%-12% is normal,13%-24% is osteopenia,over 25% is Osteoporosis and over 37% is severe Osteoporosis.According to DEXA results in connection with population census in 2000,patients with primary Osteoporosis account for approximately 6.97% of the total population.Women over 60 years old have either Postmenopause Osteoporosis or senile Osteoporosis whereas men over 75 have only senile Osteoporosis.

Ovarian Aging and Osteoporosis.
Advances in experimental medicine and biology, 2018Co-Authors: Zhao WangAbstract:Osteoporosis is the most common bone metabolic disease with a very high morbidity, and women usually got a higher risk of Osteoporosis than men. The high incidence rate of Osteoporosis in women was mainly caused by (1) women having fewer skeletons and bone mass, (2) pregnancy consuming a large amount of calcium and other nutrients, and most importantly (3) the cease of estrogen secretion by ovaries after menopause. Along with ovarian aging, the follicle pool gradually declines and the oocyte quality reduced, accompanied with decline in serum estrogen. Estrogen deficiency plays an important role in the pathogenesis of postmenopausal Osteoporosis; it is mainly a result of the recognition that estrogen regulates bone remodeling by modulating the production of cytokines and growth factors from bone marrow and bone cells. This review will summarize current knowledge concerning ovarian aging and Postmenopause Osteoporosis and also discuss clinical treatment and new ideas of drug development for Osteoporosis.
