    Abstract:

    Objectives Transfusion of allogeneic blood is still common in orthopedic surgery. This analysis evaluates from the perspective of a German hospital the Potential Cost Savings of Epoetin alfa (EPO) compared to predonated autologous blood transfusions or to a nobloodconservationstrategy (allogeneic blood transfusion strategy)during elective hip and knee replacement surgery. Methods Individual patients (N = 50,000) were simulated based on data from controlled trials, the German DRG institute (InEK) and various publications and entered into a stochastic model (Monte-Carlo) of three treatment arms: EPO, preoperative autologous donation and nobloodconservationstrategy. All three strategies lead to a different risk for an allogeneic blood transfusion. The model focused on the Costs and events of the three different procedures. The Costs were obtained from clinical trial databases, the German DRG system, patient records and medical publications: transfusion (allogeneic red blood cells: €320/unit and autologous red blood cells: €250/unit), pneumonia treatment (€5,000), and length of stay (€300/day). Probabilistic sensitivity analyses were performed to determine which factors had an influence on the model's clinical and Cost outcomes. Results At acquisition Costs of €200/40,000 IU EPO is Cost Saving compared to autologous blood donation, and Cost-effective compared to a nobloodconservationstrategy. The results were most sensitive to the Cost of EPO, blood units and hospital days. Conclusions EPO might become an attractive blood conservation strategy for anemic patients at reasonable Costs due to the reduction in allogeneic blood transfusions, in the modeled incidence of transfusion-associated pneumonia andthe prolongedlength of stay.

    This paper will evaluate Ground Penetrating Radar (GPR) as a Non Destructive Test (NDT) technique for the assessment of railway track bed ballast. Ballast and tracked deterioration is responsible for the majority of differential settlement of tracks, resulting in poor track geometry. Traditionally ballast is only replaced when it has visually deteriorated, resulting in the planning and programming of maintenance and renewals currently being condition driven. There is a significant Potential Cost Saving if ballast deterioration can be detected earlier. GPR can survey large lengths of track ballast in a relatively short space of time. This paper will show how GPR can be used to quickly identify the degree of track bed ballast deterioration and detect the ballast/formation interface.

  • An Approach to the Integrated Design of PCM-Air Heat Exchangers Based on Numerical Simulation: A Solar Cooling Case Study

    Abstract:

    A novel technique of design of experiments applied to numerical simulations is proposed in this paper as a methodology for the sizing and design of thermal storage equipment integrated in any specific application. The technique is carried out through the response surfaces in order to limit the number of simulation runs required to achieve an appropriate solution. Thus, there are significant Savings on the time spent on the design as well as a Potential Cost Saving on the experimentation if similarity relationships between the prototype and the model are met. The technique is applied here to a previously developed and validated numerical model that simulates the thermal behavior of a phase change material-air heat exchanger. The incorporation of the thermal energy storage unit is analyzed in the case of a solar cooling application, improving the system coefficient of performance. The economic viability is mainly conditioned by the price of the macroencapsulated phase change material.

    Abstract:

    Objectives. This study was designed to determine the effectiveness, safety and Costs associated with reuse of angioplasty catheters and to compare these results with those of a contemporary center that employed a single-use strategy. Background. Coronary angioplasty is an important but expensive procedure. To overcome the financial constraints of the Canadian health care system, reuse of angioplasty catheters is routinely practiced in some institutions. Methods. In a prospective observational study, data forms were completed after each angioplasty procedure and before patient discharge over a 10-month period. Results. A total of 693 patients underwent coronary angioplasty in the two centers. Clinical and lesion characteristics were similar except for a higher incidence of unstable angina at the reuse center (p < 0.005). The angiographic success rale was identical (88%) al both centers. The reuse center utilized more balloon catheters/lesion (mean ± SD 2.4 ± 1.5 vs. 1.2 ± 0.5, p < 0.00001) and had a higher incidence of initial balloon failure (10.2% vs. 3.3%, p < 0.0001). Significant prolongation of the procedure time (81 ± 41 vs. 68 ± 32 min, p < 0.0001) and increased volume of contrast medium (201 ± 86 vs. 165 ± 61 ml, p < 0.0001) were seen in the reuse center. A higher rate of adverse clinical events (7.8% vs. 3.8%, p < 0.025) was observed in the reuse center, especially in patients with unstable angina. Conclusions. The reuse strategy was associated with a higher rate of adverse events, prolonged procedure time and increased use of contrast medium, especially in lesions that were not crossed by the initial balloon and in patients with unstable angina. Whether these differences are related to the reuse strategy or to differences in patient groups cannot be ascertained by this observational study. A multicenter randomized trial is required to further assess the safety and the Cost/benefit ratio of this strategy.

    Sequence Time Domain Reflectometry (STDR) have been demonstrated to be a powerful technique for detecting the length of cable or length of open circuit or short circuit cables. Using this method along with using smart meter on the main electrical panel board to monitor consumption if load at each circuit, enable user to monitor power consumption at each node (power outlet) only by operating a smart digital meter and an STDR circuitry on each circuit at the main electrical panel board. This paper introduces this method and examines it on dead-wire and energized wire with a load connected across it. Experimental results are demonstrated for both types. Test result show the Potential application of this approach to provide consumption information and Potential Cost Saving via feedback for users.
