    Abstract:

    Abstract Patients complaining of pain or fatigue in the absence of known physical Diseases constitute a high percentage of those seeking general medical care. Depending upon the type of physician/specialist consulted, those individuals may receive Disease labels that range from an implied psychological origin such as somatoform or Psychosomatic Disease, or to a presumed physical Disease such as fibromyalgia. Although all these conditions are regularly associated with fatigue, we have provided a new label suggesting another Disease category, "systemic exertion intolerance Disease," which replaces the previous "chronic fatigue syndrome." All these conditions have common, overlapping features that usually consist of both fatigue and pain, and, in the absence of definitive objective confirmation, might be best classified under one heading such as somatic symptom disorder. Management of these disorders is challenging, but suggestions for proper identification and treatment are presented.

    The diploma work called The Phenomenon of Tarantism and the Traditional Musical and Dancing style of Pizzica in the South-Italian Region of Puglia by BcA. Irena N. Kobyláková analyses two basic cultural topics typical for the South-Italian region of Puglia from ethnological point of view, tarantism and pizzica, which complement each other. In the first part the author treats the process of tarantism, i.e. of a Psychosomatic Disease which was healed by musical and dance therapy in a frenetic rhythm of pizzica created by drumming on a tambourine. The work analyses the causes of the Disease, which are symbolically substituted by a poisonous spider, reveals the process and the therapy of the Disease, the relationship of an afflicted person to the mythical spider, as well as to Saint Paul, the patron of the poisoned. The second half of the work involves a musical and dance analysis of tarantella of Salento called pizzica, namely its three kinds, the pizzica tarantata, the passionate pizzica and the gesticulated pizzica. This part is complemented by a report about the contemporary happenning, popularity and use of pizzica in Puglia. A report of own field research in the region, which took place during years 2003 - 2009 is part of the thesis as well. The purpose of the diploma work was to bring forth an issue..

    Diplomová práce s názvem Fenomén tarantismu a tradiční hudební a taneční styl pizzica v jihoitalském kraji Apulie autorky BcA. Ireny N. Kobylákové rozebírá z etnologického hlediska dvě základní kulturní témata příznačná pro jihoitalský region Apulie, tedy fenomény tarantismu a pizziky, jež byly navzájem propojeny. V první části se autorka zabývá procesem tarantismu, tedy Psychosomatické choroby léčené hudební a taneční terapií ve frenetickém rytmu pizziky, ztvárněném především bubnováním na tamburínu. Práce analyzuje příčiny choroby, které jsou symbolicky zastoupeny jedovatým pavoukem, dále průběh choroby a terapie, vztah postiženého k mytickému pavoukovi a k svatému Pavlovi jako patronu otrávených. Je pojednáno také o historii odborných zpráv o tarantismu. Druhou část diplomové práce zahrnuje hudební a taneční analýzu salentinské tarantely, resp. pizziky, konkrétně jejích tří druhů, tedy tarantované pizziky, svádivé pizziky a šermované pizziky. Tuto část doplňuje zpráva o současném dění, oblibě a užití této hudby a tanců v Apulii. Součástí diplomové práce je také zpráva z terénního výzkumu v daném regionu v období let 2003 až 2009.     Background The effect of nature-based interventions on self-reported mental well-being in patients with physical Disease is gaining increasing attention. However, there is a lack of randomized controlled trials investigating this area. Due to the massive costs in health care systems, there is a need for new strategies to address these issues and an urgent need for attention to this field. Nature-based interventions are low cost, easy to implement, and should get attention within the health care field. Therefore, the objective was to find the impact of nature interventions on mental well-being in humans with a physical Disease. Methods In four major databases (PubMed, Cinahl, PsycINFO, and Cochrane Library), a systematic review of quantitative studies of nature’s impact on self-reported mental health in patients with physical Disease was performed. A total of 1909 articles were retrieved but only five met the inclusion criteria and were summarized. Results All five studies were quantitative, with a control group and a nature-based intervention. A source of heterogeneity was identified: the patients in one of the five studies were Psychosomatic. In the four studies with somatic patients, significant benefit of nature on self-reported mental health outcomes was found; the only study that failed to show a significant benefit was the one with Psychosomatic patients. Conclusion A significant effect of nature on mental well-being of patients with somatic Disease was found. The result in patients with Psychosomatic Disease is inconclusive, and more studies in this category are needed. Further research on the effect of nature on mental health is merited, with special attention to standardizing intervention type and dose as well as outcome measures within each medical discipline.

    Abstract The purpose of this study was to examine the relationship among alexithymia (as measured by the Toronto Alexithymia Scale—TAS and the scored version of the Archetypal 9 Test—SAT9), the presence or absence of classical Psychosomatic Disease, and the experience and expression of physical signs and symptoms. Subjects included thirty-two physical signs and symptoms. Subjects included thirty- two in-patients on a Psychosomatic medicine unit (somatizing group), thirty-one out-patients who presented to a psychology clinic (psychiatric group), and thirty-four dental patients (comparison group). All subjects completed the TAS, the SAT9, the Hypochondriasis scale of the Minnesota Multiphasic Personality Inventory (MMPI Hs), the Physical Malfunctioning subscale of the MMPI (MMPI Physm), the Somatic Complaints subscale of the MMPI (MMPI Somc), the Hypochondriasis and Denial scales of the Basic Personality Inventory (BPI Hs and BPI Dn), and a demographic questionnaire.     Abstract:

    Background Anorexia nervosa is a severe Psychosomatic Disease with somatic complications in the long-term course and a high mortality rate. Somatic comorbidities independent of anorexia nervosa have rarely been studied, but pose a challenge to clinical practitioners. We investigated somatic comorbidities in an inpatient cohort and compared somatically ill anorexic patients and patients without a somatic comorbidity. In order to evaluate the impact of somatic comorbidity for the long-term course of anorexia nervosa, we monitored survival in a long-term follow-up. Method One hundred and sixty-nine female inpatients with anorexia nervosa were treated at the Charité University Medical Centre, Campus Benjamin Franklin, Berlin, between 1979 and 2011. We conducted retrospective analyses using patient's medical and psychological records. Information on survival and mortality were required through the local registration office and was available for one hundred patients. The mean follow-up interval for this subgroup was m = 20.9 years (sd = 4.7, min = 13.3, max = 31.6, range = 18.3). We conducted survival analysis using cox regression and included somatic comorbidity in a multivariate model. Results N = 41 patients (24.3%) showed a somatic comorbidity, n = 13 patients (7.7%) showed somatic comorbidities related to anorexia nervosa and n = 26 patients (15.4%) showed somatic comorbidities independent of anorexia nervosa, n = 2 patients showed somatic complications related to other psychiatric disorders. Patients with a somatic comorbidity were significantly older (m = 29.5, sd = 10.3 vs m = 25.0, sd = 8.7; p = .006), showed a later anorexia nervosa onset (m = 24.8, sd = 9.9 vs. m = 18.6, sd = 5.1; p < .000) and a longer duration of treatment in our clinic (m = 66.6, sd = 50.3 vs. m = 50.0, sd = 47; p = .05) than inpatients without somatic comorbidity. Out of 100 patients, 9 patients (9%) had died, on average at age of m = 37 years (sd = 9.5). Mortality was more common among inpatients with somatic comorbidity (n = 6, 66.7%) than among inpatients without a somatic Disease (n = 3, 33.3%; p = .03). Somatic comorbidity was a significant coefficient in a multivariate survival model (B = 2.32, p = .04). Conclusion Somatic comorbidity seems to be an important factor for anorexia nervosa outcome and should be included in multivariate analyses on the long-term course of anorexia nervosa as an independent variable. Further investigations are needed in order to understand in which way anorexia nervosa and a somatic Disease can interact.
