    A presente dissertação propõe-se a verificar se fantasias ordálicas infantis se atualizam no contexto que leva à irrupção de uma crise psicótica. Neste trabalho, pensaremos em ordálio como prova de linhagem, remetendo ao campo originário da pessoa. A prova ordálica muitas vezes constitui um ritual de passagem, o qual busca marcar de forma simbólica a passagem de um estado para um outro. Buscamos compreender como se dá a vivência de pessoas psicóticas no que tange a mudança de papéis sociais. Para tanto, realizaremos o estudo de dois casos clínicos de psicose: um caso de esquizofrenia e outro de paranoia, os quais nos permitirão levar em conta especificidades dos quadros na análise. A partir desta investigação, refletimos sobre como se deu a vivência do complexo de Édipo e de castração nestes casos de psicose, buscando verificar se houve a ocorrência ou não de provas ordálicas na infância dos sujeitos. Questionamos se o psicótico foi submetido a certas idealizações em sua infância as quais, posteriormente, se tornam fantasias ordálicas que se atualizam no contexto que precede uma crise psicótica. Após a análise dos casos clínicos, pudemos verificar que em ambos os casos houve a vivência de provas ordálicas no momento que antecedeu o surto psicótico. Ficou inconclusiva a questão da ocorrência de um ordálio real na infância de futuros psicóticos. Verificamos em ambos os casos, como constatado no caso Schreber, que as fantasias ordálicas atualizadas no momento da crise remetem ao complexo paterno, devido a não identificação com a figura do pai. Também verificamos que o delírio constituído posteriormente tem como papel uma tentativa de cura ao narcisismo ferido devido ao fracasso da prova ordálica. _________________________________________________________________________________ ABSTRACTThis study aims to verify whether ordalic fantasies from childhood are updated in the context that leads to the irruption of a Psychotic Break. In this work, we will think of ordeal as proof of lineage, leading us back to the original backgrounds of a person. An ordealic proof often constitutes a rite of passage, which seeks to symbolically mark the passage from one state to another. We seek to understand how the experience of a Psychotic person is when it comes to shifting social roles. For this aim, we will accomplish two case studies of psychosis: a case of schizophrenia and another of paranoia, which will allow us to take into account specificities of the disorders in the analysis. From this investigation, we reflect on how was the experience of the Oedipus complex and castration in these cases of psychosis seeking to verify if there was the presence or absence of ordalic experiences in the person’s childhood. We question whether the Psychotic underwent certain idealizations in its infancy, which subsequently become ordalic fantasies that are updated in the context that precedes a Psychotic Break. After the analysis of clinical cases, we observed that in both cases there was evidence of ordalic experience on the moment that preceded the Psychotic Break. It was inconclusive the question of the occurrence of a real ordeal in the childhood of the future Psychotic. We verified in both cases, as found in the Schreber case, that the ordalic fantasies updated at the moment of the crisis refer to the father complex, due to non-identification with the father figure. We also verified that the delusion that emerges after the Psychotic Break is an attempt to cure the wounded narcissism due to the failure of the ordalic proof

    Introduction First Psychotic Break represents a challenging differential diagnosis for psychiatrists as the clinical outcome depends on an aetiology-directed approach. There is growing focus on detection of treatable causes of psychosis because treatment can alter the natural history of disease. Anti-NMDA-receptor encephalitis is a recently described treatable neuropsychiatric disease presenting with psychiatric symptoms. Objectives Review of epidemiology, clinical presentation, diagnostic tests and differential diagnosis, treatment and prognosis of anti-NMDAr encephalitis. Methods Case report of a patient diagnosed with anti-NMDAr encephalitis. Review of relevant literature collected from medical internet databases. Results Male 15 year-old patient with a previous episode of involuntary movements of the limbs and loss of conscience, presented with distonic contact, behaviour disturbances, sexual stereotypies, agitation and disturbed speech, that evolved with generalized tonic-clonic seizures, global aphasia and sinking of consciousness. A diagnose of anti-NMDAr encephalitis was made and partial recovery was obtained after treatment. Discussion Anti-NMDAr encephalitis is an auto-immune process against synaptic receptors, paraneoplasic or not, detectable by anti-body mesurement in CSF. Most patients develop a multistage illness initially presenting with psychiatric manifestations, progressing to seizures, language disintegration and catatonic states associated with abnormal movements. As patients are frequently treated with antiPsychotics, motor abnormalities and catatonia are often interpreted as extrapyramidal effects or neuroleptic malignant syndrome, confounding the diagnosis. Conclusion Early recognition and treatment of anti-NMDAr encephalitis improve the outcome. Psychiatrists working in emergency departments and first Psychotic Break units should be aware of this severe disorder in order to start adequate treatment and prevent negative outcomes

    Although the full intricacy of his accomplishment has not yet been recognized, Vian interweaves three codes in this play: the absurdist, the anti-colonial, and the psychoanalytical. A bourgeois family periodically retreats from a terrifying noise, moving upstairs into a series of ever-smaller apartments. In each they find already established the "schmürz"—a battered, silent scapegoat figure—and at each new level they lose a member of the family, until finally only le Père and the schmürz remain. Presumably they perish at the end. In 1959, the shrinking space suggested the shrinking French overseas empire, and the schmürz, its colonized victims. The disintegrating family figures the fragmentation of the individual into dissociated super ego, and id, incapable of mutual communication, a drama ending in a Psychotic Break. The absurdist features of repetition and non sequitur cannot totally conceal an underlying dramatic structure and the gradual emergence of the anti-colonialist, bourgeois critique code. The patriarchy cannot triumph without destroying itself. Its absolute reign entails total alienation. But Vian's denouement, a leurre , preserves absurdist indeterminacy
