Simultaneous Bilateral Quadriceps Tendon Rupture following Long-Term Low-Dose Nasal Corticosteroid Application

We report the case of a 61-year-old healthy patient who sustained a simultaneous bilateral Quadriceps Tendon Rupture following minor trauma. Medical history was completely clear except of a long-term nasal corticosteroid medication due to allergic rhinitis. 1

Early diagnosis and surgical repair are important to achieve an excellent functional outcome. Patient concerns We report a case of simultaneous spontaneous bilateral Quadriceps Tendon Rupture associated with secondary hyperparathyroidism. Diagnosis Magnetic resonance imaging showed that the Quadriceps Tendon was completely Ruptured at the osteotendinous junction. We then found bilateral Quadriceps Tendon Rupture during the operation. Interventions The patient underwent successful Tendon repair surgery. Outcomes The 31-year-old female patient regained full active movement of both knee joints and was able to participate in her activities of daily living. Lessons Simultaneous spontaneous bilateral Quadriceps Tendon Rupture in a patient with secondary hyperparathyroidism (undergoing hemodialysis) is a rare orthopedic injury that can be easily overlooked at the initial presentation. Early diagnosis and surgical repair is important to achieve an excellent functional outcome. For patients with secondary hyperparathyroidism receiving hemodialysis, strict systematic treatment of hyperparathyroidism is needed to prevent Rupture or re-Rupture of the Quadriceps Tendon.

To our knowledge, there have been no reports of this condition in a healthy subject. In this report, we present a case of bipartite patella separation complicated with Quadriceps Tendon Rupture in a patient with no underlying disease. Treatment involving suturing the Tendon to the patella and osteosynthesis of the bipartite patella produced a good result.

He underwent simultaneous total knee arthroplasty and extensor mechanism allograft reconstruction. At 4 years postoperatively, the patient had pain-free knee function and active extension. Conclusion The combination of a chronic Quadriceps Tendon Rupture with lack of active knee extension and knee osteoarthritis is a rare and challenging problem. To our knowledge, this combined diagnosis and the treatment described in this case report have not been reported previously in the literature.
