    Abstract:

    OBJECTIVE Ethnicity has been suggested to be a significant risk factor for pelvic organ prolapse (POP); yet, pelvic organ descent in different ethnic groups, especially in Asian populations, is not well studied. The aim of this study was to compare prolapse stages, pelvic organ descent and hiatal dimensions between East Asian and Caucasian women presenting with symptoms of POP. METHODS This was a prospective observational study of East Asian and Caucasian women presenting with symptoms of POP to a tertiary urogynecology clinic in, respectively, Hong Kong and Sydney. Demographic data, prolapse symptoms and prolapse stage were assessed. Physical examination was performed using the pelvic organ prolapse quantification (POP-Q) system. All women underwent transperineal ultrasound using Voluson systems. Offline analysis of four-dimensional ultrasound volume data was performed at a later date, by one operator blinded to all clinical data, to ascertain pelvic organ descent and hiatal dimensions on Valsalva maneuver. Levator muscle avulsion was assessed in volumes obtained on pelvic floor muscle contraction. Multiple logistic regression analysis was performed to assess factors associated with prolapse on clinical and ultrasound examinations. RESULTS A total of 225 East Asian women were included between July 2012 and February 2014 from the Hong Kong clinic and 206 Caucasian women between January 2015 and July 2016 from the Sydney clinic. There was no significant difference in the overall staging of prolapse. However, in East Asian women, compared with Caucasians, apical compartment prolapse was more common (99.6% vs 71.8%, P < 0.001) and posterior compartment prolapse less common (16.9% vs 48.5%, P < 0.001) on POP-Q examination. On Valsalva maneuver, the position of the uterus was lower in East Asian than in Caucasian women (-11.3 vs 1.35 mm, P < 0.001), while the Rectal Ampulla position was lower in Caucasians than in East Asians (-10.6 vs - 4.1 mm, P < 0.001). On multiple regression analysis, Caucasian ethnicity was a significant factor for lower risk of apical compartment prolapse on clinical assessment (odds ratio (OR), 0.01; P < 0.001) and on ultrasound (OR, 0.13; P < 0.001), and for a higher risk of posterior compartment prolapse on clinical assessment (OR, 4.36; P < 0.001) and of true rectocele on ultrasound (OR, 8.14; P < 0.001). CONCLUSIONS East Asian women present more commonly with uterine prolapse while Caucasians show more often posterior compartment prolapse.     INTRODUCTION AND HYPOTHESIS: Rectocele is common in parous women but also seen in nulliparae. This study was designed to investigate the association between vaginal parity and descent of the Rectal Ampulla/rectocele depth as determined by translabial ultrasound (TLUS). METHODS: This retrospective observational study involved 1296 women seen in a urogynaecological centre. All had undergone an interview, clinical examination and 4D ultrasound (US) imaging supine and after voiding. Offline analysis of volume data was undertaken blinded against other data. Rectal Ampulla position and rectocele depth were measured on Valsalva. A pocket depth of 10 mm was used as a cutoff to define rectocele on imaging. RESULTS: Most women presented with prolapse (53%, n = 686); 810 (63%) complained of obstructed defecation (OD). Clinically, 53% (n = 690) had posterior-compartment prolapse with a mean Bp of -1 [standard deviation (SD)1.5; -3 to 9 cm]. Mean descent of the Rectal Ampulla was 10 mm below the symphysis (SD 15.8; -50 to 41). A rectocele on imaging was found in 48% (n = 618). On univariate analysis, OD symptoms were strongly associated with Rectal descent, rectocele depth and rectocele on imaging (all P < 0.001). The prevalence of a rectocele seen on imaging increased with vaginal parity (P < 0.001). One-way analysis of variance (ANOVA) of vaginal parity against Rectal descent and rectocele depth showed a dose-response relationship (both P < 0.001). CONCLUSIONS: Vaginal parity was strongly associated with descent of the Rectal Ampulla and rectocele depth. This relationship approximated dose-response characteristics, with the greatest effect due to the first vaginal delivery.

    Abstract:

    The objective was to evaluate the diagnostic potential of digital Rectal examination in the identification of a true rectocele. This is a retrospective observational study utilising 187 archived data sets of women presenting with lower urinary tract symptoms and/or pelvic organ prolapse between August 2012 and November 2013. Evaluation included a standardised interview, ICS-POPQ, Rectal examination and 4D translabial ultrasound. The main outcome measure was the diagnosis of rectocele by digital Rectal palpation on Valsalva manoeuvre. This diagnosis correlated with the sonographic diagnosis of rectocele to determine agreement between digital examination and ultrasound findings. Complete data sets were available for 180 participants. On imaging, the mean position of the Rectal Ampulla was 11.07 (−36.3 to 44.3) mm below the symphysis pubis; 42.8% (77) had a rectocele of a depth of ≥10 mm. On palpation, a rectocele was detected in 60 women (33%). Agreement between palpation and imaging was observed in 77%; the kappa was 0.52 (CI 0.39–0.65). On receiver operator characteristic analysis, the area under the curve was 0.854 for the relationship between rectocele pocket depth and the detection of rectocele on palpation. Moderate agreement was found between digital Rectal examination for rectocele and translabial ultrasound findings of a “true rectocele”. Digital Rectal examination may be used to identify these defects in clinical practice. Extending the clinical examination of prolapse to include Rectal examination to palpate defects in the rectovaginal septum may reduce the need for defecatory proctograms for the assessment of obstructive defecation and may help triage patients in the management of posterior compartment prolapse.

    Background Rectocele is a herniation of the anterior wall of the Rectal Ampulla through a defect in the rectovaginal septum causing protrusion of the posterior vaginal wall. Common symptoms include symptoms of prolapse and obstructed defecation. Aims To describe subjective, anatomical and functional results of defect-specific rectocele repair. Materials and Methods This is an internal audit of 137 women who underwent defect-specific rectocele repair. Pre- and post-operative assessment included a standardised interview, clinical examination and 3D/4D transperineal ultrasound. Outcome measures were symptoms of obstructed defecation, recurrent prolapse symptoms, clinical posterior compartment recurrence and rectocele recurrence on ultrasound. Results At a mean follow-up of 1.4 years, 117 (85%) of women considered themselves cured or improved. Thirty-four (25%) complained of recurrent prolapse symptoms and 47 (34%) symptoms of obstructed defecation, a significant reduction (P < 0.0001). Clinical recurrence (Bp ≥ −1) was seen in 19 women (14%) and recurrence on ultrasound in 27 (20%). The mean depth of recurrence was 16.6 mm (10.3–25.1). We tested multiple potential predictors of recurrence, including age, BMI, vaginal parity, previous hysterectomy and/or prolapse surgery, follow-up time, pre-operative clinical and ultrasound findings. Only hiatal area on Valsalva (OR 0.95 for sonographic recurrence, P = 0.01) and enterocele (for clinical and sonographic recurrence, OR 4.03, P = 0.01 and OR 2.72, P = 0.02, respectively) reached significance. Conclusion Defect-specific rectocele repair is effective both in restitution of normal anatomy and in resolving prolapse and obstructed defecation symptoms at a mean follow-up of 1.4 years.

    Abstract:

    Objective  Clinically, rectocele is common in parous women and assumed to be due to distension or tearing of the rectovaginal septum in labour. In a prospective study, we examined the prevalence of such defects in primiparae before and after childbearing in order to define the role of childbearing in the aetiology of rectocele. Design  Prospective observational study. Setting  Tertiary urogynaecological clinic. Population  A total of 68 nulliparous women between 35 + 6 and 40 + 1 weeks of gestation. Methods  Participants underwent a standardised interview and were assessed by translabial ultrasound. Presence and depth of a rectocele was determined on maximal Valsalva, as was descent of the Rectal Ampulla. Fifty-two women were reassessed 2–6 months postpartum. Main outcome measures  Presence of a true rectocele, Rectal descent. Results  True rectoceles were identified in 2 of the 68 women before childbirth and in 8 of the 52 women after childbirth (P= 0.02). After childbirth, the Ampulla descended >22 mm further than before (P < 0.0001 on paired t test). Symptoms such as digitation (n= 2), straining at stool (n= 10) and incomplete emptying (n= 11) were not uncommon 2–6 months postpartum; but out of eight rectoceles, four were asymptomatic. Conclusions  True rectoceles occur in young nulliparae. However, childbirth is associated with an increase in prevalence and size of such defects.

The production and storage of a sex pheromone in glands associated with the rectum, described for the first time by FLETCHER (1968) in the male of Dacus tryoni (Froggatt), h a s b e e n h y p o t h e s i z e d (SCHUL TZ a n d B u s H , 1 9 7 1 ; EcoNOMOPOULUS, GrANNAKAKIS, TzANAKAKIS, VoYADJOGLOU, 1971) also for Dacus oleae Gmel. This paper proves that the said hypothesis is correct on anatomica!, histological, . ultrastructural and physiological ground, together with complementary ethological observations. In particular, the results of the anatomica!, histological, ultrastructural and physiological observations on D. oleae rectum bring to light: i) a functional organization of the rec.tal muscular coat, particolarly suitable for accomplishing graduai (intermittent) but vigorous expulsion of some substances, that (like sex pheromones) must be diffused as widely as possible, in the environment air; ii) four morphologically and physiologically different types of epithelium: a typical ab- sorbent epithelium, from the Rectal valve to the opening of the Rectal Ampulla; an epi- thelium « of transition », among the bases of the Rectal papillae; a squamous epithelium, in the fore third (except the opening) and along a dorsal strip of the Rectal Ampulla, and in the anal tube; and a typical glandular epithelium, which lines the mid and hind parts of the Rectal Ampulla. In sexually immature males, the glandular epithelium secretes an « oily substance » without any odor, while in sexually mature males, the said epithelium secretes together with the «oily substance», other substances with an odor (vaguely ammoniaca!) well- known as « sex odor » of the D. oleae males. The ethological observations, particularly the olfactometric ones, prove that D. oleae males strongly attract females by a sex pheromone, at the time of the day when the species is sexually active. Only hiatal area on Valsalva (OR 0.95 for sonographic recurrence, P = 0.01) and enterocele (for clinical and sonographic recurrence, OR 4.03, P = 0.01 and OR 2.72, P = 0.02, respectively) reached significance. Conclusion Defect-specific rectocele repair is effective both in restitution of normal anatomy and in resolving prolapse and obstructed defecation symptoms at a mean follow-up of 1.4 years.

The production and storage of a sex pheromone in glands associated with the rectum, described for the first time by FLETCHER (1968) in the male of Dacus tryoni (Froggatt), h a s b e e n h y p o t h e s i z e d (SCHUL TZ a n d B u s H , 1 9 7 1 ; EcoNOMOPOULUS, GrANNAKAKIS, TzANAKAKIS, VoYADJOGLOU, 1971) also for Dacus oleae Gmel. In sexually immature males, the glandular epithelium secretes an « oily substance » without any odor, while in sexually mature males, the said epithelium secretes together with the «oily substance», other substances with an odor (vaguely ammoniaca!) well- known as « sex odor » of the D. oleae males. The ethological observations, particularly the olfactometric ones, prove that D. oleae males strongly attract females by a sex pheromone, at the time of the day when the species is sexually active.
