Avaliação do padrão de realização do papanicolau para rastreamento de câncer de colo uterino em uma unidade básica de saúde de Lagarto – SE

Objective: To identify the standard execution of Papanicolaou tests realized in Dr. Davi Marcos de Lima’s Health Care Center, placed in the city of Lagarto-SE, in order to know its profile of frequency, its causes and implications. Methods: It is a cross-sectional study which consisted of the assessment by a questionnaire about Papanicolaou test execution applied to 161 users of Dr. Davi Marcos de Lima’s Health Care Center who had previously started their sexual lives, followed by the comparison between the data obtained and what is stablished by Brazilian Ministry for Health. Results and Discussion: It was found 87.5% of the interviewed patients had already undergone a Papanicolau test, considering that 78% started undergoing that examination before the age of 25 (after their sexual life's beginning). About the periodicity, they undergo: 34.8% said once a year; 27.7% do not have a Regular Time Interval; 18.4% had undergone the test once; 17%, once a semester; and 2.1% once two years. It is noticed 53.9% of the patients undergo the test in a frequency bigger than the one stablished by Brazilian Ministry for Health. When they were asked about the reasons for that behavior of doing the Smear Test, 19.8% stated it was recommended by a health professional, 17% believed it is safer; 15.6% believed it is correct; 14.8% only do if symptoms are present; and 11.3% are shamed on the test.Conclusion: It is noticed there is an inadequate standard execution of the Smear Test in Dr. Davi Marcos de Lima’s Health Care Center, happening in a frequency more elevated than the one stablished by Brazilian Ministry for Health due to an equivocated orientation of some health professionals, followed by the fact the user believes the way she is doing is the safest or the most correct. Despite of existing factors that lead the users to a lower search for the test (shame or lack of symptoms, for instance), they still undergo in a bigger frequency than the one recommended. Before that, the necessity for proposing means to change that reality exists, such as the implementation of the Women's Health Card.Introdução: O rastreamento de lesões precursoras do câncer (CA) de colo uterino deve ser realizado com uma frequência adequada evitando-se a sua baixa execução e os excessos, pelos impactos consequentes à saúde da mulher e ao Sistema Único de Saúde (SUS). Objetivo: Identificar o padrão de realização do exame preventivo na Clínica de Saúde da Família (CSF) Dr. Davi Marcos de Lima em Lagarto – SE para conhecer o perfil de frequência, suas causas e implicações. Método: Trata-se de um estudo transversal, realizado com base na avaliação de um questionário sobre a realização do exame de lâmina aplicado a 161 usuárias da CSF Dr. Davi Marcos de Lima que já iniciaram atividade sexual, seguida da comparação entre os resultados obtidos e o que é preconizado pelo Ministério da Saúde (MS). Resultados e discussões: Constatou-se que 87,5% das entrevistadas já realizaram o exame de lâmina, sendo que 78,0% iniciaram a realização com menos de 25 anos (após início da vida sexual). Sobre a periodicidade em que realizam: 34,8% referiram ser anualmente; 27,7% não têm Intervalo de tempo Regular; 18,4% só realizaram uma vez; 17,0% a cada 6 meses; e 2,1% a cada 2 anos. Nota – se que 53,9% das entrevistadas se submetem ao exame numa frequência maior que a preconizada pelo MS. Quando questionadas sobre os motivos que levam ao padrão que realizam o exame de lâmina, 19,8% afirmaram ser por recomendação de algum profissional de saúde, 17,0% acreditam ser mais seguro; 15,6% acreditam ser o correto; 14,8% só realizam na vigência de sintomas; e 11,3% têm vergonha do procedimento. Conclusões: Percebemos que existe um padrão inadequado de realização do exame de lâmina na CSF Dr. Davi Marcos de Lima, ocorrendo numa frequência mais elevada que a recomendada pelo MS devido a orientação equivocada de algum profissional de saúde, seguido pelo fato da usuária acreditar que a forma que está realizando o exame seja a mais segura ou a mais correta. Apesar de haver fatores que levem à usuária a menor busca pelo exame (como vergonha ou estar assintomática), esta ainda o realiza numa frequência maior que a recomendada. Diante disso, sentimos a necessidade de propor meios para mudar essa realidade, como a implantação do cartão de saúde da mulher.Lagarto - S

The possibility of the use of rapidly acting pressor agents which can effectively maintain the blood pressure and peripheral perfusion (without volume resuscitation) and prevent the progression of shock and providing in Regular Time Interval for volume replacement has not been looked into. In the present investigation, two pressor agents, dopamine and a long acting carboxylic ionophore monensin, were examined for their potential role in hemorrhagic hypotension produced experimentally in dogs. METHODS: Two groups of animals were studied: (a) In the first group (n=10) 20% of the total blood volume i.e. moderate hemorrhage was induced. (b) In the second group (n=10) 35% of the total blood volume i.e. severe hemorrhage was induced. After 30 min of induction of hemorrhage both the groups were given intravenous infusion of dopamine (20 microgm/kg/min) for 10 min. Then after 30 min of stopping dopamine infusion, both the groups were given intravenously a single bolus dose (50 microgm/kg) of monensin. RESULTS: In both moderate and severe hemorrhage monensin injection produced significant increases in systolic arterial blood pressure, mean arterial blood pressure, cardiac output and stroke volume. The effects persisted for 1 h of administration. In severe hemorrhage dopamine infusion produced significant decrease in mean arterial blood pressure, diastolic blood pressure and pH. Dopamine produced a significant rise in heart rate also while monensin did not have such an effect. CONCLUSION: Monensin produced a rapid and sustained recovery of the arterial blood pressure without requiring adequate preload. Dopamine failed to produce such an effect because it requires adequate circulating blood volume for its pressor action. Thus monensin has the potential to prevent progression of shock and improving the prognosis in management of hemorrhagic hypotension.

Dopamine produced a significant rise in heart rate also while monensin did not have such an effect. CONCLUSION: Monensin produced a rapid and sustained recovery of the arterial blood pressure without requiring adequate preload. Dopamine failed to produce such an effect because it requires adequate circulating blood volume for its pressor action. Thus monensin has the potential to prevent progression of shock and improving the prognosis in management of hemorrhagic hypotension.
