Eleven young rabbits were divided into two groups as solid Diet (control group; n = 6) or Soft Diet (Soft-Diet group; n = 5) groups. After 6 months, muscle fibers from the medial and the two heads of the lateral pterygoid muscles were histochemically defined. In the medial pterygoid muscle, the percentage of the type 1 fiber cross-sectional area to total area was 10.1 ± 2.4 % in the control group and 8.3 ± 3.0 % in the Soft-Diet group, respectively. In the Soft-Diet group, there was a trend toward an increase in the number of type 2A fibers, and toward a decrease in the numbers of type 2B fibers in comparison with the controls. In the two heads of the lateral pterygoid muscle, the percentage of the type 1 fiber cross-sectional area to total area was 8.4 ± 7.5 and 3.3 ± 2.7 %, respectively. Compared to that of the control group, the two heads in the Soft-Diet group showed a trend toward a decrease in the number of type 2A fibers. In addition, type 2B had a tendency to decrease in the number in the inferior head. In conclusion, this study suggests that long-term Soft Diet leads to adaptations of the pterygoid muscles. Two heads of the lateral pterygoid muscle revealed different adaptation from jaw-closing muscles under Soft-Diet conditions.

The aim of the present study was to investigate the histochemical effects of long-term Soft Diet on the masseter muscle in growing rabbits. Materials and methods: Twelve young male Japanese white rabbits were divided into two groups (n = 6 each) at weaning (1 month after birth) and fed a solid Diet (control group) or a powder Diet (Soft-Diet group). The duration of the experimental period was 6 months. Masseter fibers from the superficial and the deep portions were histochemically defined as type 1, 2A, 2B, or 2C fibers. Results: As compared with that of the control, the deep masseter of the Soft-Diet group showed a significantly lower ratio of type 1 fiber cross-sectional area to total area (6.3 and 10.1% for the Soft-Diet and control group, respectively), significantly more type 2A fibers (74.0%vs 50.3%) and significantly fewer type 2B fibers (4.3%vs 12.5%). However, fiber size did not differ between the two groups. NADH-tetrazolium-reductase (NADH-TR) of the masseter was less reactive in the Soft-Diet group, reflecting a lower oxidative capacity. Conclusions: These findings indicate that the alteration of the functional activities contributed to selective disuse influences on the type 1 and type 2B fibers, and a resultant increase in type 2A fibers. This study suggests that long-term alteration of jaw function induced by a Soft Diet can lead to adaptations of the masseter muscle.

ReddyAbstract:Summary Background In mild acute pancreatitis, traditional treatment has been initial fasting and oral refeeding with clear liquids to prevent adverse gastrointestinal events such as pain. The Diet is gradually progressed to Soft solids and hospital discharge is planned based on patients’ tolerance to a solid Diet. Aim To determine the length of hospitalization and tolerance to oral refeeding when initiated on a Soft Diet as compared to a clear liquid Diet. Methods One hundred and one patients with mild acute pancreatitis were randomized to receive either a clear liquid Diet or Soft Diet when oral feeding was initiated. Frequency of pain, total and postrefeeding length of hospitalization, and Dietary intake were monitored. Hospital discharge was decided by the medical team without input from the study coordinators. Results A statistically significant decrease in the length of hospitalization (total and postrefeeding) of a median of 2 days was seen in patients receiving a Soft Diet (P < 0.001). No significant difference in the need for cessation of Diet because of pain was observed between the two groups. Patients initiated on a Soft Diet consumed significantly more calories and fats on study day 1 (P < 0.001). Conclusion Oral refeeding with a Soft Diet in patients with mild acute pancreatitis can be considered safe and can result in shorter length of hospitalization.

single free anterolateral thigh flap for simultaneous reconstruction of composite hypopharyngeal and external neck skin defect after head and neck cancer ablation

Herein, we present five patients, with advanced hypopharyngeal cancer and anterior neck skin invasion, which received a single anterolateral thigh (ALT) fasciocutaneous flap for composite inner pharyngeal and outer skin defect reconstruction after wide composite resection. Two ALT flaps were divided into two distinct paddles supplied by two or more separate perforators, one part for reconstructing the inner pharyngeal defect and another for neck skin coverage. Three ALT flaps only supplied by one sizable perforator could not be divided and de-epithelization of mid-part had to be done to reconstruct both defects with the single flap. The results revealed survival of all flaps. There were no flap loss, fistulas, or bleeding complications. All patients recovered uneventfully and could eat a Soft Diet to regular Diet postoperatively. In conclusion, one-staged reconstruction of complex pharyngoesophageal and external skin defects after extensive oncological resection is feasible using a single ALT fasciocutaneous free flap. © 2011 Wiley-Liss, Inc. laparoscopic primary repair with omentopexy for duodenal ulcer perforation a single institution experience of 21 cases
Journal of Gastric Cancer, 2012Co-Authors: Chung Hyeun, Min Gyu KimAbstract:Purpose: Despite the great advances in laparoscopic techniques, most active general surgeons do not apply laparoscopic surgery in the treatment of duodenal ulcer perforation when facing a real-life emergency. Therefore, our study was designed to evaluate the feasibility of laparoscopic surgery in duodenal ulcer perforation, and provide a step-by-step protocol with tips and recommendations for less experienced surgeons. Materials and Methods: Between March, 2011 and May, 2012, 21 patients presenting with duodenal ulcer perforation underwent laparoscopic primary repair with omentopexy. There were no contraindications to perform laparoscopic surgery, and the choice of primary repair was decided according to the size of the perforation. The procedure for laparoscopic primary repair with omentopexy consisted of peritoneal lavage, primary suture, and omentopexy using a knot pusher. Results: During the operation, no conversion to open surgery or intra-operative events occurred. The median operation time was 45.0 minutes (20~80 minutes). Median day of commencement of a Soft Diet was day 6 (4~17 days). After surgery, the median hospital stay was 8.0 days (5~27 days). Postoperative complications occurred in one patient, which included a minor leakage. This complication was resolved by conservative management. Conclusions: Although our study was carried out on a small number of patients at a single institution, we conclude that laparoscopic primary repair can be an effective surgical method in the treatment of duodenal ulcer perforation. We believe that the detailed explanation of our procedure will help beginners to perform laparoscopic primary repair more easily.

the applicability of laparoscopic gastrectomy in the surgical treatment of giant duodenal ulcer perforation

METHODS: Between July 2010 and April 2011, laparoscopic distal gastrectomy with ROUX-EN-Y gastrojejunostomy and truncal vagotomy was performed in consecutive 5 patients with giant duodenal ulcer perforation. RESULTS: There was no conversion to open surgery. There was no severe postoperative complication. The days of normalization of leukocytosis were 3, 1, 2, 2, and 5, respectively. The times to first flatus were postoperative days 2, 3, 5, 2, and 3. The days of commencement of a Soft Diet were postoperative days 5, 5, 6, 5, and 5. They were discharged on postoperative days 9, 11, 20, 10, and 11. CONCLUSIONS: We suggest that laparoscopic surgery may be a good surgical method to improve surgical outcomes and is worth a try in experts.
