An unusual cause of gastric Perforation in childhood: trichobezoar (Rapunzel syndrome). A case report

Rapunzel syndrome is a rare condition in which the presence of giant trichobezoars causes mechanical obstruction. To date, only two cases of Stomach Perforation caused by trichobezoars have been reported among pediatric patients. We report a 14-year-old female patient who experienced nausea, vomiting and severe abdominal pain for 1 month. Physical examination revealed diffuse abdominal distension. Palpation detected a mobile and sensitive mass, 15 × 15 cm , which filled the upper quadrant. Urgent surgery revealed that the Stomach was perforated by the trichobezoar ball. This trichobezoar mass was totally excised by expanding the Perforation area. Conclusion This is the reported third case of gastric Perforation caused by trichobezoar in a pediatric patient. Among acute abdominal cases, gastric Perforation should remain a possibility in differential diagnosis.

Stomach Perforation post cardiopulmonary resuscitation—A case report

This is mainly caused by incorrect management of the airway during CPR performed by non-medical personnel. Presentation of case We report a case of 72 year old female who sustained a Stomach Perforation during prolonged CPR in an out of hospital arrest situation. This was diagnosed on a computed tomography scan of the abdomen requiring midline laparotomy and a primary repair of the Stomach. Discussion The training of medical and non-medical persons in cardiopulmonary resuscitation is to be encouraged. However it should be emphasized that any technique which breaches the normal integrity of the body can itself lead to life-threatening complications. Conclusion A high degree of vigilance is required in patients presenting after a cardiac arrest and CPR.

The Annals of Thoracic Surgery, 2004Co-Authors: Alessandro Bini, M Grazia, Francesco Petrella, Franco Stella, R BazzocchiAbstract:Abstract Gastropleural fistula may occur after pulmonary resection, perforated paraesophageal hernia, perforated malignant gastric ulcer at the fundus, or gastric bypass surgery for morbid obesity. We describe a case of gastropleural fistula after Stomach Perforation by a nasogastric tube in a patient who underwent Billroth II gastric resection for adenocarcinoma. Left biliopneumothorax occurred and was treated by thoracic drainage with –20 cm H 2 O aspiration. As gastropleural fistula persisted, laparotomy was repeated and gastric and diaphragmatic Perforations were sutured. Gastropleural fistula is rare and, to our knowledge, this is the first reported case of gastropleural fistula and biliopneumothorax caused by gastric and diaphragmatic Perforation by a nasogastric tube.
