    Abstract:

    Digital initiatives such as big data analytics at Progressive Insurance can drive competitive advantage. However, digital initiatives often fail to meet their objectives in terms of budget, time, or performance. Why is this the case and what can be done about it? Complementing existing studies of software Development methodologies, project management techniques, and the role of the CIO, we focus on the role of the non-CIO organizational leader in Successful Development of digital initiatives. Synthesizing our own original research with existing scholarship, we introduce four principles for Successful Development of digital initiatives: generate business value, maintain strategic alignment with organizational and business strategies, leverage technology trajectories, and apply digital economics. Understanding the four principles raises the digital fitness of organizational leaders, thereby reducing risk and raising the chances of success in digital initiatives.

    In a project which has lasted 3 years, a vision guidance system has been developed to the stage where commercial availability is imminent. Six prototypes have been field tested by farmers and two more are on trial in the United States. There have been several changes of technology but the fundamental principles have remained consistent. The system derived its guidance signal from a video camera image of the rows of a crop such as cotton. It is relatively insensitive to additional visual 'noise' from weeds, while tolerating the fading out of one or more rows in a barren patch of the field. The software integrates data from several crop rows, and tests each row for image quality. Colour components of the image signal can be selected to improve discrimination between crop and detritus. Experimental results are presented showing that the system is capable of maintaining an accuracy of 2 cm. Some farmer responses from the extensive field trials are also included.

    Abstract:

    Summary In recent years, a number of reviews have generated evidence on the potential of community health insurance (CHI) to increase access to care and offer financial protection against the cost of illness for poor people excluded from formal insurance systems. Field experience, however, shows that in sub-Saharan Africa (SSA), a series of operational difficulties still hampers the Successful Development of CHI, yielding negative effects on potential progress towards increased access to care and improved financial protection. Through a careful assessment of the existing literature, including peer-reviewed articles, books, consultancy reports, and manuscripts from international organizations, we produce an analytical review of such difficulties. Our aim is to provide policy makers with the necessary knowledge on the problems at stake and with policy propositions to offset such problems, strengthening CHI and enhancing its role within SSA health systems. Our review of the literature reveals that the major difficulties currently faced by CHI in SSA are operational in nature and cluster around five areas: (i) lack of clear legislative and regulatory framework; (ii) low enrolment rates; (iii) insufficient risk management measures; (iv) weak managerial capacity; and (v) high overhead costs. Consequently, our review calls for appropriate policy interventions, specifically: (i) greater commitment towards the Development of adequate legislation in support of CHI; (ii) increasing uptake of measures to expand equitable enrolment; (iii) the adoption of adequate risk management measures in all schemes; (iv) substantial investments from host countries as well as from sponsoring agencies to improve managerial capacity; and (v) collective efforts to contain overhead costs. Au cours des dernieres annees, un certain nombre de revues ont genere des preuves sur le potentiel de l’assurance en sante communautaire (ASC) d’accroitre l’acces aux soins et d’offrir une protection financiere contre le cout de la maladie, pour les personnes pauvres exclues des systemes formels d’assurance. L’experience de terrain montre cependant qu’en Afrique subsaharienne (ASS), une serie de difficultes operationnelles constituent encore des obstacles au succes du developpement de l’ASC, produisant des effets negatifs sur le potentiel du progres vers l’amelioration de l’acces aux soins et l’amelioration de la protection financiere. Par une evaluation minutieuse de la documentation existante, comprenant des articles publies, des livres, des rapports d’experts et des manuscrits provenant d’organisations internationales, nous avons effectue une analyse de ces difficultes. Notre but est de fournir aux decideurs, les connaissances necessaires sur les problemes en jeu et avec des propositions de politique, de pallier ces problemes, ce qui renforceraient l’ASC et son role au sein des systemes de sante de l’ASS. Notre revue de la litterature revele que les principales difficultes rencontrees par l’ASC en ASS sont de nature operationnelle et regroupent cinq domaines: a) absence d’une structure legislative et reglementaire claire, b) faible taux d’enrolement, c) insuffisance des mesures de gestion des risques, d) faible capacite de gestion; e) frais generaux eleves. Par consequent, notre revue fait appel a des interventions de politiques, plus specifiquement: a) un engagement plus important en vue de l’elaboration d’une legislation appropriee a l’appui de l’ASC, b) une augmentation de l’adhesion aux mesures visant a accroitre l’enrolement equitable, c) l’adoption de mesures adequates de gestion des risques dans tous les schemas, d) des investissements substantiels de la part des pays d’accueil ainsi que des organismes sponsors afin d’ameliorer les capacites de gestion; e) des efforts collectifs en vue de contenir les frais generaux. En anos recientes, un numero de revisiones han generado evidencia sobre el potencial del seguro comunitario de salud (SCS) a la hora de aumentar el acceso a cuidados sanitarios y ofrecer proteccion financiera frente al coste de la enfermedad entre personas pobres excluidas del sistema de seguros sanitarios formales. Sin embargo, la experiencia en el terreno muestra que en Africa sub-Sahariana (ASS) existen una serie de dificultades operacionales que obstaculizan el desarrollo exitoso del SCS, afectando negativamente el progreso potencial hacia una mejora de acceso sanitario y proteccion financiera. Mediante una evaluacion cuidadosa de la literatura existente, que incluyo articulos arbitrados, libros, informes de consultores y manuscritos de organizaciones internacionales, hemos producido una revision analitica sobre dichas dificultades. Nuestro objetivo es proveer a los politicos con los conocimientos necesarios sobre los problemas en juego y con propuestas de politicas que les den una solucion, fortaleciendo el SCS y mejorando su rol dentro de los sistemas sanitarios de ASS. Nuestra revision revela que las principales dificultades que actualmente tienen los SCS en ASS son operacionales y se agrupan alrededor de cinco areas: a) la falta de un marco claro a nivel legislativo y regulatorio; b) unas bajas tasas de inscripcion; c) insuficientes medidas para el manejo de riesgos; d) una capacidad directiva debil; e) y costes indirectos altos. En consecuencia, nuestra revision insiste en la necesidad de intervenciones politicas adecuadas, especificamente: a) un mayor compromiso con el desarrollo de una legislacion adecuada que apoye los SCS; b) medidas que aumenten la inscripcion equitativa; c) la adopcion de medidas adecuadas para el manejo de riesgos en todos los planes; d) una inversion sustancial por parte de paises huespedes, asi como de las agencias patrocinadoras que mejoren la capacidad directiva; e) un esfuerzo colectivo que contenga los costes indirectos.
