    Accurate and timely clinical data are required for clinical and organisational purposes and is especially important for patient Management, audit of surgical performance and the electronic health record. The recent introduction of computerised Theatre Management systems has enabled real-time (point-of-care) operative procedure coding by clinical staff. However the accuracy of these data is unknown. The aim of this Scottish study was to compare the accuracy of Theatre nurses’ real-time coding on the local Theatre Management system with the central Scottish Morbidity Record (SMR01). Paired procedural codes were recorded, qualitatively graded for precision and compared (n = 1038). In this study, real-time, point-of-care coding by Theatre nurses resulted in significant coding errors compared with the central SMR01 database. Improved collaboration between full-time coders and clinical staff using computerised decision support systems is suggested.

    Abstract:

    This chapter announces Harris’s arrival at Covent Garden Theatre. It recounts the ill-fated purchase of the Theatre by four speculators in 1767 — Harris, Colman, Powell, and Rutherford — and the violent fight for control that eventually led to Harris’s triumph. This chapter explores the culmination of their dispute at the trial in the Court of Chancery, along with the factors and prejudices which influenced its outcome. Newly uncovered manuscripts are employed to recreate the arguments offered by the opposing advocates — Wedderburn and Dunning — as they focused upon the art of Theatre Management and the motivations of the youthfully impetuous Harris. This trial before the Lords Commissioners of the Great Seal was one staging post in Harris’s course towards Pitt’s ministry, as it forced him to confront the nature of public opinion as well as Colman’s ability to marshal it.

    This is the first biography of Thomas Harris (1738-1820). Until now, little has been known about his life. He was most visible as the man who controlled Covent Garden Theatre for nearly five decades, one of only two venues in London allowed by law to perform spoken drama. Harris presided over one of the most eventful periods in the history of the English stage; uncovering his involvement provides new perspectives upon landmark events in London’s history. But this career was only one of many: he became the confidant of George III, a philanthropist, sexual suspect, and a brothel owner in the underworld of Covent Garden. While deeply involved in Pitt the younger’s government, Harris worked as a ‘spin doctor’ to control the release of government news. Only through understanding his career is it possible to appreciate fully the suppression of radicalism in the period. As novelists created elaborate storylines with fictional intriguers lurking in the shadows, Harris was the real thing. Harris’s career intersects many of the hidden worlds of the eighteenth century including the art of Theatre and Theatre Management, the activities of the Secret Service, radical protest, and sexual indulgence. This narrative of detection brings together a hoard of newly discovered manuscripts to construct his numerous lives.

  • Simulation-Based Optimization for Surgery Scheduling in Operation Theatre Management Using Response Surface Method

    Abstract:

    Operation Theatre is one of the most significant assets in a hospital as the greatest source of revenue as well as the largest cost unit. This paper focuses on surgery scheduling optimization, which is one of the most crucial tasks in operation Theatre Management. A combined scheduling policy composed of three simple scheduling rules is proposed to optimize the performance of scheduling operation Theatre. Based on the real-life scenarios, a simulation-based model about surgery scheduling system is built. With two optimization objectives, the response surface method is adopted to search for the optimal weight of simple rules in a combined scheduling policy in the model. Moreover, the weights configuration can be revised to cope with dispatching dynamics according to real-time change at the operation Theatre. Finally, performance comparison between the proposed combined scheduling policy and tabu search algorithm indicates that the combined scheduling policy is capable of sequencing surgery appointments more efficiently.
