    Abstract:

    Background Colloid cyst is the most common neoplastic lesion of the Third Ventricle, and the occurrence of 2 colloid cysts in the ventricular system is rare. Case Report We present a 35 year old female patient with paired colloid cysts of the Third Ventricle who presented with obstructive hydrocephalus and were treated surgically with the transcallosal interhemispheric transforaminal approach. Until now only 3 patients of dual or paired colloid cysts of different Ventricles were reported. To the best of our knowledge, this is the first case with both the colloid cysts present in the Third Ventricle. Conclusion This report illustrates the importance of proper radiologic study in a case of colloid cyst and preparing preoperatively and excision of both the colloid cyst whenever present, ​to prevent the recurrence.

  • Infratentorial supracerebellar approach to the colloid cysts of the Third Ventricle.

    Abstract:

    Objective The transcallosal and the frontal transcortical approaches are the most widely used methods in surgery of Third Ventricle colloid cysts. However, these approaches require traction of the frontal lobe and dissection of the corpus callosum or corticotomy and involve some postoperative consequences. The rationale of the proposed method is to remove the colloid cyst by the infratentorial supracerebellar approach and the posterior wall of the Third Ventricle without dissection of any neural structures. Methods Five patients with a colloid cyst of the Third Ventricle were operated on by the proposed method. The first patient presented with several months' history of symptoms that included increased intracranial pressure and right-sided cerebellar signs, caused by a metastatic tumor of the right cerebellar hemisphere. The other four patients had symptoms including intracranial hypertension for an extended period of time without any other neurological deficits. In all patients, magnetic resonance imaging revealed a colloid cyst of the Third Ventricle without hydrocephalus. Technique With the infratentorial supracerebellar approach, the arachnoid of the quadrigeminal cistern is dissected. The pineal body is separated and displaced from the internal vein medially, and the posterior velum interpositum is opened. Perforation of the inferior layer of the tela choroidea just above the suprapineal recess allows opening of the Third Ventricle cavity. A foraminal region is exposed after a slight lateral displacement of medial surfaces of the thalamus along the Third Ventricle roof. Conclusion The proposed approach through the infratentorial supracerebellar space and the posterior wall of the Third Ventricle may be used for removal of colloid cysts, especially in patients in whom the lateral Ventricles are not enlarged.

  • Endoscope-assisted infratentorial-supracerebellar approach to the Third Ventricle: an anatomical study.

    Abstract:

    The authors studied the microsurgical anatomy and endoscopic features of the pineal region and Third Ventricle to describe a combined microsurgical-endoscopic infratentorial-supracerebellar approach to the posterior Third Ventricle. Such an approach exposes the pineal gland and its complex neurovascular structures so that the Third Ventricle can be reached through a minimally invasive parapineal incision. The approach was studied in 10 adult cadaveric heads, six fresh and four formalin fixed, by using an operative microscope with a magnification level of 6 to 40 and the assistance of an endoscope. The endoscope-assisted infratentorial-supracerebellar approach affords a complete view of the Third Ventricle from a posterior perspective. The Third Ventricle is entered through a parapineal incision using the natural space between the internal cerebral vein and the vein of Rosenthal located above the superior colliculi. The infratentorial-supracerebellar approach to the Third Ventricle follows a natural corridor and requires minimal retraction and resection of critical neural structures. With the use of the endoscope, an unsurpassed view into the Third Ventricle from a posterior perspective is obtained.
