Tumor thickness and paralingual distance of coronal mr imaging predicts cervical node metastases in oral Tongue carcinoma

The accurate preoperative assessment is essential for treatment. Several studies have suggested that histologic Tumor thickness is related to the metastases. The aim of this study was to determine whether MR images of oral Tongue Tumor have the potential to predict cervical lymph node metastases. MATERIALS AND METHODS: A total of 43 patients with squamous cell carcinoma of the oral Tongue were investigated. Tumor thickness, sublingual distance between Tumor and sublingual space, and paralingual distance between Tumor and paralingual space, as determined from coronal MR imaging, were preoperatively estimated. Logistic regression analysis was used to identify independent predictors of lymph node metastases. RESULTS: Univariate logistic regression analysis showed that T classification, N classification, and 3 measured MR imaging distances (millimeters) were significantly associated with lymph node metastases. Multivariate logistic regression analysis showed that Tumor thickness (odds ratio, 1.34; 95% confidence interval [CI], 1.11–1.63; P P 9.7 mm and paralingual distance of CONCLUSION: MR images provide satisfactory accuracy for the preoperative estimation of the Tumor thickness and the paralingual distance, which are valuable for predicting cervical lymph node metastases.

Investigation of CXCR4 in squamous cell carcinoma of the Tongue.

Recent studies on metastasis have focused on chemoattraction, particularly on the role of chemokines. Therefore, in this study we investigated whether CXCR4 (receptor for stromal cell-derived factor-1) is expressed in squamous cell carcinoma of the Tongue. For this purpose, immunohistochemical staining was performed on 26 sections obtained from 23 patients (15 patients having Tongue Tumor without lymph node metastasis and eight patients having lymph node metastasis). All Tumor cells expressed CXCR4, whereas normal mucosa of the Tongue had no or faint expression CXCR4. Metastatic Tumor cells in lymph nodes had stronger expression than primary Tongue Tumor cells. Our data raises the possibility that CXCR4 could be involved in lymph node metastasis of oral squamous cell carcinoma.

MRI in differentiation of benign and malignant Tongue Tumors.

Magnetic resonance imaging (MRI) is the preferred modality for the evaluation of Tongue carcinomas. Dynamic contrast-enhanced (DCE)-MRI can reflect the density, integrity, and leakiness of Tumor vasculature, and the time-intensity curve (TIC) patterns derived from DCE-MRI results can differentiate benign from malignant Tumors based on differences in vascular structure. Diffusion-weighted (DW)-MRI is based on the random thermal motion of water molecules and can provide information on the cellular and tissue microstructure of the Tumor. A low apparent diffusion coefficient (ADC) derived from DW-MRIs may indicate a malignant Tumor. Thus, ADC values and TIC parameters yield complementary information on Tumors that may improve diagnostic accuracy. Indeed, the combination of DCE-MRI and DW-MRI is a comprehensive reflection of the pathological status of the Tongue Tumor, so utilization of these MRI modalities may facilitate the diagnostic differentiation of benign from malignant Tumors of the Tongue.

oral Tongue cancer gene expression profiling identification of novel potential prognosticators by oligonucleotide microarray analysis

The gene expression profile of patients (n=37) with oral Tongue SCC were analyzed using Affymetrix HG_U95Av2 high-density oligonucleotide arrays. Patients (n=20) from which there were available Tumor and matched normal mucosa were grouped into stage (early vs. late) and nodal disease (node positive vs. node negative) subgroups and genes differentially expressed in Tumor vs. normal and between the subgroups were identified. Three genes, GLUT3, HSAL2, and PACE4, were selected for their potential biological significance in a larger cohort of 49 patients via quantitative real-time RT-PCR. Hierarchical clustering analyses failed to show significant segregation of patients. In patients (n=20) with available Tumor and matched normal mucosa, 77 genes were found to be differentially expressed (P< 0.05) in the Tongue Tumor samples compared to their matched normal controls. Among the 45 over-expressed genes, MMP-1 encoding interstitial collagenase showed the highest level of increase (average: 34.18 folds). Using the criterion of two-fold or greater as overexpression, 30.6%, 24.5% and 26.5% of patients showed high levels of GLUT3, HSAL2 and PACE4, respectively. Univariate analyses demonstrated that GLUT3 over-expression correlated with depth of invasion (P<0.0001), Tumor size (P=0.024), pathological stage (P=0.009) and recurrence (P=0.038). HSAL2 was positively associated with depth of invasion (P=0.015) and advanced T stage (P=0.047). In survival studies, only GLUT3 showed a prognostic value with disease-free (P=0.049), relapse-free (P=0.002) and overall survival (P=0.003). PACE4mRNA expression failed to show correlation with any of the relevant parameters. The characterization of genes identified to be significant predictors of prognosis by oligonucleotide microarray and further validation by real-time RT-PCR offers a powerful strategy for identification of novel targets for prognostication and treatment of oral Tongue carcinoma.
