The Relationship Between Maxillary and Mandibular Base Lengths and Dental Crowding in Patients with True Class II Malocclusions

MATERIALS AND METHOD: A sample of 40 orthodontic patients with complete bilateral Class II malocclusions in the permanent dentition (25 males, 15 females) who were divided into two groups based on severity of pre-treatment mandibular anterior dental crowding. The maxillary and mandibular effective lengths and Tooth-Arch size discrepancies were measured on the pre-treatment Lateral cephalograms and initial casts, respectively. Intergroup comparisons of apical base lengths were assessed with independent t-tests. Correlation between effective length and dental crowding was assessed by Pearson's correlation coefficient (P <.05).RESULTS: Subjects with Class II malocclusion and moderate to severe crowding had significantly smaller maxillary and mandibular effective lengths compared with subjects without crowding and with minimal dental crowding. A weak inverse correlation was also found between maxillary and mandibular effective lengths and the severity of dental crowding. CONCLUSION: In patients with complete Class II malocclusion, decreased maxillary and mandibular effective lengths constitute a significant factor associated with dental crowding

occlusion and Tooth Arch dimensions in the primary dentition of preschool jordanian children

To assess Tooth and Arch dimensions, occlusal relationships and the presence of spacing or crowding in primary dentition of Jordanian children. Study design. A list of 10 kindergartens was randomly selected in the governate of Irbid, Jordan. A total of 1048 children (2·5–6 years old) with complete primary dentitions were examined for occlusal relationships in three planes, and the presence of spacing or crowding. Of those, study casts were taken for 87 randomly selected 4–5-year-old children (39 girls, 48 boys). Measurements of Tooth and Arch dimensions were made using electronic digital sliding callipers. Results. In general, males had larger mesiodistal Tooth width and larger Arch dimensions. Bilateral mesial step molar relationship was found in 47·7% of children followed by bilateral flush terminal molar relationship in 37% and bilateral distal step in 3·7%. Asymmetric right/left molar relationship was found in 11·6% of children examined. Class I canine relationship was found in 57% of children, followed by Class II canines in 29% and Class III canines in 3·7%. Fifty per cent of children examined had Class I incisors, 24·7% had Class II Division 1, 13·5% had Class II Division 2 and 11·8% showed reversed overjet. Ideal overbite was seen in 44·3% of children, reduced overbite in 21·8%, increased overbite in 28·2% and 5·7% had anterior openbite. Buccal crossbite was seen in 7% of the sample. Generalized spacing was found in 61·8% and 61·1% of children in the upper and lower Arches, respectively. Anthropoid spaces were found in 70% of the upper Arches and in 51% of the lower Arches. Conclusions. Males had larger Tooth/Arch dimension than that of females in the primary dentition stage. Mesial step molar relationship was found in 47·7% of the subjects followed by flush terminal molar relationship in 37%. The majority of children examined had spaced primary dentition.

However, incisor crowding is not merely a Tooth-Arch size discrepancy. Many variables such as direction of mandibular growth, early loss of deciduous molars, the oral and perioral musculature and incisor and molar inclination can be associated with crowding. Only few studies evaluated the relationship between mandibular anterior crowding and cephalometric measurements in the early mixed dentition. It was the aim of this study to seArch for dentofacial factors that might be associated with mandibular crowding in the early mixed dentition. Lateral cephalograms and dental casts of 60 children (33 girls, 27 boys) were evaluated. It was determined that patients with crowding had smaller lower incisor to NB angles, maxillary skeletal lengths, mandibular skeletal length, and mandibular dental measurements. They also had greater interincisal angles, overjet, overbite, and Wits appraisal measurements. Significant inverse correlations were found between crowding and SNB, lower incisor to NB angle, anterior cranial length, mandibular length, maxillary length, mandibular dental measurement and direct correlations between crowding and interincisal angle, overjet, overbite, and FMIA. According to these results, we conclude that crowding of the mandibular incisors is not only a Tooth-Arch size discrepancy. Dentofacial characteristics also contribute to this misalignment.

Rapid palatal expander: Maxillary and mandibular measurements using manual and digital methods

FrancettiAbstract:OBJECTIVES To evaluate the existence of a cor-relation between rapid palatal expansion and changes in width, length and perimeter of the upper Arch, as well as changes in width, length and perimeter of the lower Arch, as a natural consequence of those of the maxillary complex over time. MATERIALS AND METHODS This study was conducted on a sample of 19 patients treated with RPE, rapid palatal expansion. The entire group was divided in two samples: in the first sample data were recorded before the start of treatment for rapid palatal expansion and at 6 months follow-up from the end of the activation period; in the second group information was acquired at the start and at 3 month follow-up. Each patient was treated with an orthodontic-ortho-pedic expansion device Hyrax type, with a central screw, with an activa-tion protocol of a quarter turn of the screw performed two times a day for the first three days and once a day until the control appointment. After the control appointment it has been prescribed to proceed with the daily activations of a quarter turn personalized for every single patient, until the planned expansion is reached. The data were examined with two different methods, then a comparison between them was made: the measurements were taken manually, directly on the cast models, with a caliber and then with digitally using intraoral scanner GC Aadva IOS (GC Europe N.V., Leu-ven, Belgium). For each group the following data were recorded at time zero and at the post-expan-sion follow-up time at maxillary and mandibular level: inter-canine width, inter-joint width, inter-mo-lar width, antero-posterior length, perimeter of the Arch. Finally, the correlation index be-tween transverse expansion of the palate and variation of the Arch perimeter following ortho-dontic-orthopedic therapy is evaluated. RESULTS AND CONCLUSIONS Manual and digital acquisitions showed comparable results. The data show that there are no statistically significant differences between the two groups at time zero, for both Arches. The results show a greater variation for all the variables examined in favor of the group with a six month follow-up, due to the greater possibility of growth of the jaw, directly proportional to the greater period of time elapsed between evaluations; by the statistical point of view, however, there are values that are not significantly different as inter-canine, inter-premolar and Arch pe-rimeter parameters, opposed to inter-molar and antero-posterior data that are statistically signifi-cant. CLINICAL SIGNIFICANCE The examination of two groups with different follow-up showed non-homogeneous variations for all the dental elements, in particular at molar level a longer period of time is necessary for the achievement of an optimal expansion. The results of this study can be exploited by the clinician to correct unilateral or bilateral posterior cross bites, dental crowding, especially in patients with unfavorable Tooth-Arch relationships, thus allowing alignment, restoring correct occlu-sal shape and function, correcting asymmetries, as well as planning an orthodontic treatment
