    Analysis of the concurrence of Torus Mandibularis and Torus palatinus among a total of 2010 dental patients, by the test of independence, revealed that the concurrence was statistically significant among women, but not among men. A tentative hypothesis was proposed to explain the sexual dimorphism of the variable, combined occurrence of the two features.

    The prevalence of Torus Mandibularis was assessed in two groups of dental patients, altogether 2010 individuals over 10 yr of age: 1181 individuals native to the Lofoten Islands in North Norway, situated at 68 degrees latitude; and 829 patients indigenous to Gudbrandsdal, an inland district in the southeastern part of the country at 61 degrees latitude. Both groups were supposed to be of the same Caucasian stock and, therefore, to have similar genetic predisposition to Torus on the average. The following observations were found: 1) the prevalence of Torus Mandibularis was much greater in Gudbrandsdal than in Lofoten (P much less than 0.001); 2) the prevalence decreased among persons above 50 yr of age as compared with those of the age classes 10-49 yr (P less than 0.01); and 3) it was smaller among women than men (P less than 0.05), mainly due to such a decrease in Lofoten. In a recent investigation of people living in Gudbrandsdal the fraction of the variation of Torus that was attributable to genetic differences was estimated as about 30%, whereas approximately 70% of the causes seemed to be ascribable to environmental influences in terms of occlusal stress. It is suggested that dietary habits and number of existing teeth seemed to be environmental variables with an influence on the observed variation of Torus prevalence between geographical regions, age classes, and sexes. The question about a possible sexual difference as to the genetic component of liability to Torus Mandibularis was outside the scope of the present study.

    Abstract:

    OBJECTIVE Obstructive sleep apnea (OSA) is a common sleep breathing disorder. Untreated OSA may lead to a number of cardiovascular complications. Dentists may play an important role in OSA detection by conducting careful oral examinations. This study focused on the correlation of oral anatomical features in Thai patients who presented with OSA. METHODS We conducted a prospective comparative study at a sleep/hypertension clinic and a dental clinic at Khon Kaen University in Thailand. Patients with OSA were enrolled in the study, along with age-matched patients with non-OSA (controls). Baseline characteristics, clinical data, and oropharyngeal data of all patients were compared between the two groups. Oropharyngeal measurements included tongue size, Torus Mandibularis, Mallampati classification, palatal space, and lateral pharyngeal wall area. Multivariate logistic regression analysis was used to identify the factors associated with OSA. RESULTS During the study period, there were 156 patients who met the study criteria; 78 were patients with OSA and the other 78 were healthy control subjects. In the OSA group, there were 43 males with a mean age of 53 (standard deviation 12.29) years and a mean BMI of 30.86 kg/mm(2). There were 37 males in the control group with a mean age of 50 (standard deviation 12.04) years and a mean BMI of 24.03 kg/mm(2). According to multivariate logistic analysis, three factors were perfectly associated with OSA, including Torus Mandibularis class 6, narrow lateral pharyngeal wall, and Mallampati class 4. There were two other significant factors associated with having OSA, namely, BMI and Mallampati classification. The adjusted odds ratios (95% confidence interval) of these two factors were 1.445 (1.017, 2.052) and 5.040 (1.655, 15.358), respectively. CONCLUSION Dentists may play an important role in the detection of OSA in patients with high BMI through careful oropharyngeal examination in routine dental treatment. A large Torus Mandibularis, Mallampati class 4, and a narrow lateral pharyngeal wall are important anatomical risk factors for OSA.

    OBJECTIVE Various anatomical structures of upper airway and physical differences are known to be risk factors for obstructive sleep apnoea (OSA). Torus Mandibularis is a structure that can appear on the inside of the mandible. Therefore, it is possible for tori to influence airway volume by occupying the space for tongue and cause sleep apnoea. The purpose of this study is to investigate the effect of Torus Mandibularis on the severity of OSA as one of the craniofacial risk factors. DESIGN Retrospective case-control study. SETTING University-based tertiary medical centre. PARTICIPANTS Adult patients over 19-years-old who visited outpatient clinics with complaints of sleep-disordered breathing symptoms between January 2010 and December 2017 were investigated. MAIN OUTCOME MEASURES The presence of Torus Mandibularis in oral cavity was confirmed by physical examination or CT image. We analysed demographic findings including age, sex, medical history, previous operation history, physical findings of upper airway and result of polysomnography. To evaluate the effect of Torus Mandibularis on OSA, polysomnography data of the two groups according to presence or absence of Torus Mandibularis were compared and analysed. RESULTS Two-hundred and thirty-two OSA patients with BMI <25 were divided into two groups, according to either the presence or absence of Torus Mandibularis. We analysed 138 patients of control group and 94 of Torus Mandibularis group. Apnoea-hypopnoea index (AHI) was 18.8 ± 14.9 in control group and 25.1 ± 18.4 in Torus Mandibularis group (P = 0.006). Respiratory disturbance index (RDI) was 23.1 ± 14.7 in control group and 27.9 ± 18.4 in Torus Mandibularis group (P = 0.035). Supine AHI showed 26.6 ± 20.3 in control group and 32.5 ± 22.6 in Torus Mandibularis group (P = 0.039). Patients with Torus Mandibularis had a trend of increase in proportion according to the severity of sleep apnoea, such as AHI (P = 0.007) or RDI (P = 0.034). CONCLUSIONS We newly found that the presence of Torus Mandibularis affects not only severity of OSA and also position-dependent OSA. These results support the necessity of Torus Mandibularis evaluation in OSA patients, and further study is also required to investigate its consequence in the surgical outcome.

    The factor marginal alveolar bone height and Torus Mandibularis (TM) were studied in 2 groups of dentate patients more than 20 years of age, altogether 571 individuals. Subgroups possessing TM had the higher prevalence of unimpaired bone height as compared with those without the trait (P < 0.001). Different environmental factors considered, such as food habits, exposure to infection, oral hygiene habits, bruxism, access to professional dental care, and habits as to seeking dental treatment, did not seem to account for or to have any influence on the observed phenotypic correlation between TM and more favorable bone height. It was therefore suggested that TM and marginal bone seem to be influenced by common pleiotropic genes. It is hypothesized that future disclosure of the loci that regulate the capacity to develop TM at the same time may throw some light on the genetic contribution and mechanisms that tend to make the marginal alveolar bone more resistant to destructive agents. □ Anatomy; osteology; peri...
