Dilation of tracheal stenosis below Tracheostomy tube with Dolphin percutaneous Tracheostomy Kit

Under endoscopic view, the Dolphin Kit was inserted through the stoma into the stenosis; the balloon was inflated until a sufficient tracheal diameter was obtained; then, a longer Tracheostomy tube was inserted through the stenosis and the distal tip placed near the carina. This procedure was succesfully applied in seven patients.

Awake Tracheostomy in a patient with acute upper airway obstruction: an emergency application of an elective percutaneous procedure.
Open Access Emergency Medicine, 2019Co-Authors: Pierluigi Fusco, Angela Iuorio, Mirco Della Valle, Fausto FerraroAbstract:: Since its introduction in 1985 with Ciaglia, percutaneous Tracheostomy (PT) was contraindicated in emergency settings and obesity. However, several case series in the last 20 years have documented the use of PT in life-threatening airway emergencies. We present a case of severe acute airway obstruction in a 66-year-old woman successfully treated with a placement of an awake PT. The woman's glottic obstruction was caused by a recurrent laryngeal neoplasia and revealed by nasoendoscopy. This acute condition required a serious effort from the patient to oxygenate and therefore prevented orotracheal intubation as well as the use of any supraglottic device and/or sedation. Blood aspiration after a first attempt to make a quick access to the tracheal lumen with an emergency cricothyroidotomy, and difficulties in the exact identification of tumor infiltration, led us to perform an awake Tracheostomy. Due to elevated risk of airway bleeding, we started with a surgical approach to better identify anatomical structures. After the correct inter-tracheal ring space identification, sudden worsening of clinical symptoms required that we complete the procedure quickly with the aid of a Ciaglia Blue Rhino™-Cook (CBR) Tracheostomy Kit. At the Tracheostomy tube placement, the patient quickly resolved her dyspnea and physiological breathing was restored.

Evaluation of a new Tracheostomy technique for morbidly obese patients on an intensive care unit

Although PDT has a low complication rate in nonobese patients, it is not technically possible to use standard PDT Kits in morbidly obese patients with thick necks. The skin to trachea distance renders traditional-length Tracheostomy tubes or dilators too short. Recently a Tracheostomy Kit (Uniperc, Smith Medicals, UK), designed for obese patients has become available. This Kit consists of an S-shaped dilator and a long adjustable reinforced Tracheostomy tube. The objective of this study was to evaluate the safety and complications of this new technique in morbidly obese patients.

an evaluation of a new percutaneous Tracheostomy Kit
Anaesthesia, 1995Co-Authors: L D Caldicott, G J Oldroyd, Andrew R BodenhamAbstract:Summary The Portex percutaneous trachestomy Kit incorporates guidewire dilating forceps which pass over a guidewire and perform rapid dilatation. These forceps have curved jaws which are designed to reduce possible damage to the lateral and posterior walls of the trachea. The guidewire dilating forceps are first opened in front of the trachea to dilate the pretracheal tissues and then opened in the trachea to dilate the anterior tracheal wall. A Tracheostomy tube is then passed over the guidewire and into the trachea. The procedure was rapid and successful in all 20 patients, with no serious complications. Mean time to perform Tracheostomy was 4.9min (range 90s–15min). Overall, the Portex percutaneous Tracheostomy Kit worked well and proved a rapid and acceptable method for performing percutaneous Tracheostomy.
