2015 proceedings of the national heart lung and blood institute s state of the science in Transfusion Medicine symposium

Josephson, Darrell J Triulzi, Dana V Devine, Walter H Dzik, Anne F Eder, Terry Gernsheimer, Daryl J Kor, Nareg RoubinianAbstract:On March 25 and 26, 2015, the National Heart, Lung, and Blood Institute sponsored a meeting on the State of the Science in Transfusion Medicine on the National Institutes of Health (NIH) campus in Bethesda, Maryland, which was attended by a diverse group of 330 registrants. The meeting's goal was to identify important research questions that could be answered in the next 5 to 10 years and which would have the potential to transform the clinical practice of Transfusion Medicine. These questions could be addressed by basic, translational, and/or clinical research studies and were focused on four areas: the three "classical" Transfusion products (i.e., red blood cells, platelets, and plasma) and blood donor issues. Before the meeting, four working groups, one for each area, prepared five major questions for discussion along with a list of five to 10 additional questions for consideration. At the meeting itself, all of these questions, and others, were discussed in keynote lectures, small-group breakout sessions, and large-group sessions with open discourse involving all meeting attendees. In addition to the final lists of questions, provided herein, the meeting attendees identified multiple overarching, cross-cutting themes that addressed issues common to all four areas; the latter are also provided. It is anticipated that addressing these scientific priorities, with careful attention to the overarching themes, will inform funding priorities developed by the NIH and provide a solid research platform for transforming the future practice of Transfusion Medicine.

One size will never fit all: the future of research in pediatric Transfusion Medicine

WidnessAbstract:There is concern at the National Heart, Lung, and Blood Institute (NHLBI) and among Transfusion Medicine specialists regarding the small number of investigators and studies in the field of pediatric Transfusion Medicine (PTM). Accordingly, the objective of this article is to provide a snapshot of the clinical and translational PTM research considered to be of high priority by pediatricians, neonatologists, and Transfusion Medicine specialists. Included is a targeted review of three research areas of importance: (i) Transfusion strategies, (ii) short- and long-term clinical consequences, and (iii) Transfusion-transmitted infectious diseases. The recommendations by PTM and Transfusion Medicine specialists represent opportunities and innovative strategies to execute translational research, observational studies, and clinical trials of high relevance to PTM. With the explosion of new biomedical knowledge and increasingly sophisticated methodologies over the past decade, this is an exciting time to consider Transfusion Medicine as a paradigm for addressing questions related to fields such as cell biology, immunology, neurodevelopment, outcomes research, and many others. Increased awareness of PTM as an important, fertile field and the promotion of accompanying opportunities will help establish PTM as a viable career option and advance basic and clinical investigation to improve the health and wellbeing of children.

STUDY DESIGN AND METHODS: Seven multidisciplinary subcommittees developed proposals in the following areas: 1) platelet (PLT) product use, 2) neonatal and/or pediatric Transfusion practice, 3) surgical Transfusion practice, 4) intensive care unit and/or in trauma Transfusion practice, 5) plasma and/or cryoprecipitate product use and therapeutic apheresis practice, 6) red blood cell (RBC) product use and/or blood conservation management, and 7) medical Transfusion practice or blood donor studies. The committees consisted of Transfusion Medicine specialists, hematologists, cardiovascular surgeons, anesthesiologists, neonatologists, critical care physicians, and clinical trial methodologists. Proposals were presented and an external panel evaluated and prioritized each concept for scientific merit, clinical importance, and feasibility. RESULTS: Twenty-four concepts were presented by the subcommittees. Ten concepts addressed four areas deemed most important: 1) PLT Transfusion strategies to prevent and/or mitigate bleeding in neonates and patients with hematologic malignancies, 2) RBC Transfusion trigger strategies to improve overall outcomes in different patient populations, 3) evaluation of optimal plasma:PLT:RBC ratios in trauma resuscitation, and 4) pathogen inactivation of PLTs to improve PLT Transfusion safety. CONCLUSIONS: The proposal themes not only represent inquiries about the indications for Transfusion, but also epitomize the lack of consensus when clinical practice lacks a strong evidence base. Ultimately, the purpose of this publication is to provide a “blueprint” of ideas for further development rather than endorse any one specific clinical trial design.

Relevantly, significant practice variation in the use of blood products exists among practitioners and institutions, largely because of the lack of robust clinical trial data, in many instances, which are critical for providing practitioners with evidence-based guidelines for appropriate blood product utilization. Recognizing this gap, the National Heart, Lung, and Blood Institute recently established a State-of-the-Science Symposium to help define areas of clinical trial research that would enhance the opportunity for developing appropriate practice guidelines for both Transfusion Medicine and Hemostasis/Thrombosis. Such a Symposium was held in September 2009 to identify important clinical trial research issues in these 2 subject areas of endeavor. The aims of this Symposium were to specifically identify phase 2 and 3 clinical trials that, if conducted over the next 5 to 10 years, could impact the treatment of patients with hemostatic and other disorders as well as to optimize the use of blood products in patients who need such interventions. This article reports on the deliberations that were held relating to the various clinical trial concepts developed by 7 Transfusion Medicine subcommittees. This Symposium generated a rich assortment of clinical trial proposals that will undergo further refinement before final implementation into pilot or full randomized clinical trials. The various proposals identified many opportunities for clinical trial research and most importantly underscored the ongoing need for well-developed evidence-based clinical trial research in the field of Transfusion Medicine.

Sloan, Patricia S OsullivanAbstract:BACKGROUND: Pediatric Transfusion Medicine (PTM) is a subspecialty of Transfusion Medicine with no formal training program and few specialists. The Pediatric Transfusion Medicine Academic Awardees (PedsTMAA) group surveyed PTM content experts to identify relevant objectives for the first formal PTM curriculum. STUDY DESIGN AND METHODS: Eight North American PTM experts were invited to participate in a two-step consensus process. PTM-related objectives compiled from a review of existing training documents were organized into a survey. Experts were asked to rate each objective for relevancy for a clinical pathology trainee. Content validity indexes (CVIs) and asymmetric confidence intervals (ACIs) of expert ratings and analysis of respondents' comments were used to identify relevant objectives. RESULTS: Six experts participated and reviewed 117 objectives. Based on content validity criteria (CVI ≥ 0.83 and lower-limit 95% ACI ≥ 3), a total of 65 objectives were considered relevant. Twenty-three objectives were rated “very relevant” by all the experts while some proposed objectives were determined to be not relevant, out of date, or inappropriate for a resident trainee level. CONCLUSIONS: The PedsTMAA group identified 65 objectives for a PTM curriculum. Twenty-three represent a clear core set of objectives and should be considered for clinical pathology training. The next step is to consider the teaching strategies and evaluation methods that will be employed to best deliver this content addressing competency in medical knowledge.

Transfusion Medicine education for non Transfusion Medicine physicians a structured review

To close this gap, there is not only a need for increased Transfusion Medicine educational opportunities but for those using structured and proven instructional methods. Kern and colleagues have defined important steps to be considered in curricular design: general needs assessment; targeted needs assessment; goals and objectives; educational strategies; implementation; and evaluation and feedback. We use this framework to examine the current state of Transfusion Medicine educational initiatives for the non-Transfusion Medicine physician.

The current state of hematology trainee knowledge of Transfusion Medicine is not known. STUDY DESIGN AND METHODS A validated assessment tool developed by the Biomedical Excellence for Safer Transfusion (BEST) Collaborative was used to assess prior Transfusion Medicine education, attitudes, perceived ability, and Transfusion Medicine knowledge of hematology trainees. RESULTS A total of 149 hematology trainees at 17 international sites were assessed. The overall mean exam score was 61.6% (standard deviation, 13.4%; range, 30%-100%) with no correlation in exam scores with postgraduate year or previous Transfusion Medicine education in medical school or internal Medicine residency. However, better scores correlated with 3 or more hours of Transfusion Medicine education (p = 0.0003) and perceived higher-quality education during hematology training (p = 0.03). Hematology trainees at US sites, where hematology is often combined with oncology training, had statistically lower scores than trainees at non-US sites (56.2% vs. 67.4%; p < 0.0001). In terms of topic areas, although 93% of participants had obtained consent for Transfusion, the lowest scores were on Transfusion reaction–related questions. CONCLUSION Given the overall poor performance, this study serves as an impetus for all hematology training programs to reevaluate the quality and quantity of Transfusion Medicine training and can assist in the development of targeted curricula.

While inadequate physician Transfusion Medicine knowledge may lead to inappropriate practice, such an educational deficit has not been investigated on an international scale using a validated assessment tool. Identifying specific deficiencies is critical for developing curricula to improve patient care. Study Design and Methods Rasch analysis, a method used in high-stakes testing, was used to validate an assessment tool consisting of a 23-question survey and a 20-question examination. The assessment tool was administered to internal Medicine residents to determine prior training, attitudes, perceived ability, and actual knowledge related to Transfusion Medicine. Results A total of 474 residents at 23 programs in nine countries completed the examination. The overall mean score of correct responses was 45.7% (site range, 32%-56%). The mean score for Postgraduate Year (PGY)1 (43.9%) was significantly lower than for PGY3 (47.1%) and PGY4 (50.6%) residents. Although 89% of residents had participated in obtaining informed consent from a patient for Transfusion, residents scored poorly (<25% correct) on questions related to Transfusion reactions. The majority of residents (65%) would find additional Transfusion Medicine training “very” or “extremely” helpful. Conclusion Internationally, internal Medicine residents have poor Transfusion Medicine knowledge and would welcome additional training. The especially limited knowledge of Transfusion reactions suggests an initial area for focused training. This study not only represents the largest international assessment of Transfusion Medicine knowledge, but also serves as a model for rigorous, collaborative research in medical education.

To design needs-based educational interventions to address this gap, a validated assessment tool is required. Previously published exams have not been created or validated using rigorous psychometric methods. Study Design and Methods A modified Delphi method was used to achieve consensus regarding the essential knowledge and skills for physicians who transfuse blood products. To ensure content validity, members of an international organization of Transfusion Medicine experts (Biomedical Excellence for Safer Transfusion [BEST] Collaborative) participated in the exam design process. An exam, based on the most highly rated topics, was created and administered to individuals with a priori expected basic, intermediate, and expert levels of Transfusion Medicine knowledge. Rasch analysis, a psychometric technique used in high-stakes medical licensure and board testing, was used to determine exam accuracy and precision. Results Thirty-six topics achieved ratings sufficient to be considered for inclusion in the exam (content validity index > 0.8). A 23-question exam was administered to 49 individuals. Mean scores for individuals with expected basic, intermediate, and expert knowledge were 42, 62, and 82%, respectively (p < 0.0001). The exam achieved good fit with the Rasch model. Conclusion A validated exam has now been created to accurately assess Transfusion Medicine knowledge. This exam can be used to determine knowledge deficits and assist in the design of curricula to improve blood product utilization.

CER evaluates evidence from existing studies or generates new evidence, in different populations and under specific conditions in which the treatments are actually used. CER does not embrace one research design over another but compares treatments and variations in practice using methods that are most likely to yield widely generalizable results that are directly relevant to clinical practice. Treatments used in Transfusion Medicine (TM) are among the most widely used in clinical practice, but are among the least well studied. High-quality evidence is lacking for most Transfusion practices, with research efforts hampered by regulatory restrictions and ethical barriers. To begin addressing these issues, the National Heart, Lung, and Blood Institute convened a workshop in June 2011 to address the potential role of CER in the generation of high-quality evidence for TM decision making. Workshop goals were to: 1) evaluate the current landscape of clinical research, 2) review the potential application of CER methods to clinical research, 3) assess potential barriers to the use of CER methodology, 4) determine whether pilot or vanguard studies can be used to facilitate planning of future CER research, and 5) consider the need for and delivery of training in CER methods for researchers.

The COVID-19 pandemic has further heightened the inequity faced by female physicians as they are challenged by increasing household and childcare duties in addition to their professional responsibilities. Given these hurdles, fewer women than men have published in various medical disciplines. In this brief report, we wanted to determine the impact of the COVID-19 pandemic on the academic output of female physicians and researchers in Transfusion Medicine. Study design and methods We compared all articles in four Transfusion Medicine journals published from January 1 to July 31, 2019 with the same time period in 2020. Overall, 1024 articles were reviewed for whether they included women as first or senior authors. Results Overall, women were first authors in 45.9% (n = 458) of all publications and senior authors in 35% (n = 356) of all publications. There was a statistically significant decrease in the percentage of women as first authors between 2019 (49.1%) and 2020 (42.7%) (p = .04). There was no significant change in the percentage of women as senior authors between 2019 (35.4%) and 2020 (35.5%) (p = 0.99). Conclusions Similar to other medical specialties, the COVID-19 pandemic has further increased the disparities faced by female researchers in Transfusion Medicine as evidenced by a decrease in publications with women as first authors.

This synopsis is now made available as a manuscript published in Transfusion. Study design and methods CTMC committee members review original manuscripts including TM-related topics published in different journals between late 2018 and 2019. The selection of topics and manuscripts are discussed at committee meetings and are chosen based on relevance and originality. After the topics and manuscripts are selected, committee members work in pairs to create a synopsis of the topics, which is then reviewed by two committee members. The first and senior authors of this manuscript assembled the final manuscript. Although this synopsis is comprehensive, it is not exhaustive, and some papers may have been excluded or missed. Results The following topics are included: infectious risks to the blood supply, iron donor studies, pre-Transfusion testing interference and genotyping, cold agglutinin disease (CAD), HLA alloimmunization in platelet Transfusions, patient blood management, updates to TACO and TRALI definitions, pediatric TM, and advances in apheresis Medicine. Conclusion This synopsis provides easy access to relevant topics and may be useful as an educational tool.
