    Abstract:

    Cutaneous Tuberculosis may present in diverse forms. We report a case of a 21-year-old Nigerian female with a 2-year history of an extensive ulceration and crusts on the nose and lips with almost complete obliteration of the nasal apertures. The clinical diagnosis of cutaneous Tuberculosis was confirmed by histopathological examination of the skin biopsy specimen. She showed a favourable and rapid response to anti-Tuberculosis therapy although there was disfiguring scar formation with almost complete obliteration of her nasal apertures. The importance of early recognition, histopathological examination and a remarkable response to anti-Tuberculosis drugs in the diagnosis of this rare presentation of cutaneous Tuberculosis is highlighted.

    Abstract:

    Background  Tuberculosis Cutis Orificialis is an extremely rare variant of cutaneous Tuberculosis. Perianal location is a possible site of presentation. Methods  We describe the management of a young male with a painless non-specific perianal ulcer, who presented an asymptomatic disseminate pulmonary and intestinal Tuberculosis. He had a history of pulmonary sarcoidosis and long-term corticosteroid therapy. Results  Healing of the ulcer was achieved after three-drug antituberculous therapy, with an improvement of the radiographic pulmonary alterations. Conclusions  Tuberculous origin should be considered in persistent perianal ulcers to avoid delays in the treatment of this rare form of Tuberculosis.

    A 61-year-old patient had a 25-year history of erythematous scaling lesions, diagnosed and treated as psoriasis vulgaris. He presented with a growing nodule within the erythematous plaque. Biopsy shows epithelioid cell granulomas with prominent Langhans giant cells. There was no sign of a squamous cell carcinoma. The tuberculin test was strongly positive and M. Tuberculosis complex was detected in the biopsy material by PCR. He was diagnosed with lupus vulgaris, the most frequent form of cutaneous Tuberculosis. Other types include Tuberculosis verrucosa Cutis, Tuberculosis Cutis Orificialis and disseminated military Tuberculosis. The patient was treated with rifampicin, isoniazid, pyrazinamide and ethambutol for two months, following a four month treatment with rifampicin and isoniazid. The skin lesions rapidly resolved under antituberculotic treatment. Ein 61-jähriger Patient stellt sich mit seit 25 Jahren bestehenden Hautveränderungen vor, die bislang als Psoriasis vulgaris behandelt wurden. Aktueller Vorstellungsgrund war ein in dieser Läsion wachsender Knoten. Die Histologie zeigt epitheloidzellige Granulome und große mehrkernige Riesenzellen vom Langhans-Typ. Ein Plattenepithelkarzinom konnte ausgeschlossen werden. Der Tuberkulintest nach Mendel und Mantoux ist positiv, mikrobiologisch zeigt sich im Biopsiematerial ein positives PCR- und Sequenzierungsergebnis für den Mycobacterium-Tuberculosis -Komplex. Wir stellen die Diagnose eines Lupus vulgaris, den häufigsten Typ der kutanen Tuberkulose. Andere, seltenere Formen sind die Tuberculosis Cutis verrucosa, die Tuberculosis Cutis Orificialis und die Miliartuberkulose. Unter 2-monatiger Vierfachtherapie mit Rifampicin, Isoniazid, Pyrazinamid und Ethambutol sowie folgender 4-monatiger Zweifachtherapie mit INH und Rifampicin kommt es zu einer deutlichen Befundbesserung.
