The Impact of Offloading Footwear in Prevention and Healing Of Viral Foot Ulcers (Verruca Plantaris)

Methods: It was a prospective observational study conducted at Shree Vijay Vachhrajani memorial diabetic foot and wound management centre between 2016 to 2019. A total of 98 patients were included in the study. For each patient, clinical profile like age, gender, location of foot ulcer, site of ulcer, healing status, length of time the ulcer has been healed, amputation history, surgical history; family history; tobacco use; alcohol abuse and use of offloading footwear etc were studied. Results: The average age was 55.4 and patients between 9 to 90 years of age. Out of total 98 patients, 54 were males (55.10%) and 44 were females (44.90%) and male preponderance was observed in comparison to females. Among ulcer healed patients, we observed negative correlation (correlation coefficient= - 0.10823) between addiction history of patients and length of healing time. positive correlation (correlation coefficient= 0.27787) was seen between operation of foot and healing time. Early studies suggested that pulsed-dye laser (PDL) therapy might be highly effective in the treatment of Verrucae, although more recent reports have been less optimistic. Previous studies have been retrospective or have consisted of a series of patients treated with PDL alone. Objective: Our objective was to prospectively evaluate the efficacy of PDL therapy versus conventional therapy with liquid nitrogen cryotherapy or cantharidin in the treatment of warts. Methods: Forty healthy adult patients with Verrucae were randomized to receive either PDL (585 nm) therapy or conventional therapy. All enrollees also performed home therapy. Patients were eligible for up to 4 treatment sessions at 1-month intervals. Warts were individually counted and measured at the time of each treatment session. Complete response was defined as complete absence of Verrucae with the presence of dermatoglyphics, and partial response was defined as a 50% or greater reduction in wart size. Results: A total of 194 warts were evaluated by the conclusion of the study. Complete response was noted in an average of 70% of the warts treated with conventional therapy and in 66% of those in the PDL group; this was statistically insignificant. Partial response was observed in an average of 82% of the warts in patients treated with conventional therapy and 87% in the PDL group. Verruca vulgaris responded more readily than Verruca Plantaris in both treatment groups. The mean number of treatments to achieve success was similar in both groups. Conclusion: The PDL is an efficacious form of therapy for Verrucae, as is conventional therapy. PDL therapy should be considered among the better established approaches in the treatment of warts, although data from this trial suggest that this approach is probably not superior. They have the ability to encode proteins involved in viral replication and production of the protective coat within the host cells but require host cell ribosomes and mitochondria for translation. The members of the families Herpesviridae, Poxviridae, Papovaviridae, and Picornaviridae are the most commonly known agents for cutaneous viral diseases, but other virus families, such as Adenoviridae, Togaviridae, Parvoviridae, Paramyxoviridae, Flaviviridae, and Hepadnaviridae, can also infect the skin. Herpetic whitlow should be considered under the title of special viral infections of the acral region, where surgical incision is not recommended; along with Verruca Plantaris with its resistance to treatment and the search for a new group of treatments, including human papillomavirus vaccines; HIV with maculopapular eruptions and palmoplantar desquamation; orf and milker's nodule with its nodular lesions; papular-purpuric gloves and socks syndrome with its typical clinical presentation; necrolytic acral erythema with its relationship with zinc; and hand, foot, and mouth disease with its characteristics of causing infection with its strains, with high risk for complication.

Successful treatment of Verruca Plantaris with a single sublesional injection of interferon‐α2a

Interferons are molecules with antiviral effects, which have been used for the treatment of Verruca for many years. Aims. To determine if sublesional interferon (IFN)-α injection offers an effective alternative treatment for common warts. Methods. We compared the results of single-dose sublesional IFN application in different types of Verruca and with placebo for the treatment of single Verruca Plantaris lesions. In total, 53 patients (mean age 22.6 years) were enrolled in the study. Of these, 45 patients received a single sublesional injection of 4.5 MU IFN-α2a (three study groups), and eight patients with single Verruca Plantaris lesions were injected with physiological saline as placebo (control group). As local anaesthesia, liquid nitrogen was sprayed only on to the injection site for 3–4 s. The injection was made directly under the lesion through the border of the lesion, at with approximately a 45° angle from healthy skin. Results. At the 12-month follow-up in the group of patients with single Verruca Plantaris, there were 19 complete cures (7.2%) and 2 partial responses (8.3%), and 3 patients (12.5%) had no response. In the control group, only 2 patients (25%) had a partial response to treatment. Conclusions. These results suggest that a single sublesional dose of 4.5 MU IFN-α may be of value in the treatment of patients with Verruca, especially in those with single Verruca Plantaris lesions.
