    Abstract:

    Objectives: To explore the clinical and pathological characteristics of Vulvar Disease and to provide guidance for clinical diagnosis and treatment.Methods: The clinical and histopathologic data of 250 biopsies of Vulvar Disease were analyzed.Results: Among all these 250 cases,12.8% suggests infectious Diseases,46% demonstrates cystic and tumor-like lesions,12.4% shows hyperplastic or atrophic Diseases,24.4% shows benign tumors and 4.4% shows malignant tumors.Infectious Disease occurs most frequently in the group of 18 to 35 years old women.The hyperplastic or atrophic lesion and malignant tumors of the Vulva more often occurs in the group of women elder than 50 years.All differences are statistically significant(P0.001).Conclusions: There are many types of Vulvar Diseases.Benign tumors and tumor-like lesions and cystic lesions are easily detected by biopsy.Young and middle-aged women should pay attention to prevent infectious Diseases and tumor-like lesions while old women should be alert to the risk of incidence of hyperplastic or atrophic Diseases and squamous cell carcinoma.Biopsy is helpful for diagnosis of Vulvar Disease and avoidance of misdiagnosis.Dermatologist particularly should attach more importance to lesion biopsy in diagnosis of Vulva Disease.

  • Long-term effect of metoprolol therapy in patients with rheumatic Vulva Disease induced congestive heart failure

    Abstract:

    [Objective] To study the long-term effects and mortality of metoprolol therapy in patients with rheumatic Vulva Disease induced congestive heart failure.[Methods] We collected 176 rheumatic heart Disease patients who was rejected the surgical therapy by the doctors or themselves rejected surgical therapy,with a cardiac function of New York Heart Association(NYHA)class Ⅲ(n=94)or Ⅳ(n=82).LVEDD was70 mm and LVEF was40% in all the patients.They were randomly divided into the control group and the metoprolol group.The control group patients were received routine therapy such as diuresis,digoxine and captopril.The metoprolol group received routine therapy and metoprolol therapy.Heart function parameters were repeatedly measured before and 6 months,1 year,2 years,3 years during therapy period.Mortality was also determined.The average follow up period was(29.17±6.83)months.[Results] Mortality:37 patients of control group died during follow up(11 sudden cardiac death and 26 refractory heart failure),24 patients of metoprolol group died also(7 sudden cardiac death and 17 refractory heart failure).Mortality was significantly reduced in metoprolol group.The mean 6 min walking distance was increased significantly(P 0.01)at 1 years to 2 years and 3 years of follow up.NYHA class:compared with the control group the cardiac function of the patients of metoprolol group improved significantly,with a reduction of mean NYHA class of more than one grade at 6 months to 3 years follow-up(P 0.01).LVEDD:compared with the control group the LVEDD reduced significantly of the patients of metoprolol group at 6 months to 3 years follow-up(P 0.01).[Conclusions] Metoprolol therapy for congestive heart failure patients induced by rheumatic Vulva heart Disease,may enhance quality of life and heart function and reverse LV remodeling.The long-term effects of treatments were stable and effective,leading to the reduction of mortality from sudden cardiac death and advanced heart failure.
