    Abstract:

    Malignant female Adnexal Tumor of probable wolffian origin is distinctly rare. To my knowledge, only four other cases have been reported in the English literature. This article reports a malignant female Adnexal Tumor of probable wolffian origin of the right broad ligament with extensive metastases to the omentum in an 81-year-old woman. The patient died 3 months after the diagnosis. The site and characteristic histologic features of the Tumor and the absence of carcinoma of the ovaries, fallopian tubes, and uterus indicate that this Tumor is of wolffian origin.

    Abstract:

    Female Adnexal Tumor of wolffian origin (FATPWO) is a lesion that predominantly occurs in the broad ligament. Its occurrence in the right paravaginal area is rare; only one other case has been reported to occur at this site. The authors report a case of paravaginal FATPWO in a 20-year-old woman that recurred twice during a 5-year period. The microscopic and ultrastructural features of this lesion were identical to other cases of FATPWO reported in the literature. This Tumor must be recognized, especially at this site, so that it is not confused with vaginal adenocarcinoma related to diethylstilbestrol exposure in utero.

    Abstract:

    Six adenocarcinomas of the fallopian tube that resembled the female Adnexal Tumor of probable wolffian origin are described. The Tumors, which occurred in patients from 38 to 66 (average 55) years of age, typically formed intraluminal masses. One was an incidental finding on microscopic examination. On microscopic examination, the Tumors were characterized by a predominant pattern of small, closely packed cells punctured by numerous glandular spaces, which were typically small but occasionally were cystically dilated. Many of the glands contained a dense colloid-like secretion that was positive with the periodic acid-Schiff stain. Small amounts of intracellular mucin were present in all cases. In the solid areas of three cases, spindle cells that focally formed concentric whorls were present. In all cases, small numbers of tubular glands typical of endometrioid adenocarcinoma were identified. The cytologic atypia and mitotic activity of the Tumors were variable, but they exceeded that usually seen in wolffian duct Tumors. The evidence indicates that this neoplasm represents an unusual form of endometrioid adenocarcinoma. It is important that it is distinguished from a Tumor of wolffian duct origin.

    Abstract:

    The purpose of this study is to assess the preoperative evaluation of an Adnexal mass using the GI-RADS classification and to verify whether CA-125 measurement can offer any additional benefits to the GI-RADS-based prediction of ovarian Tumor malignancy. In this study, we assessed a total of 215 women with an Adnexal Tumor using the GI-RADS classification combined with CA-125 measurement. All Adnexal masses underwent histological verification. Of a total of 215 lesions, we classified 2 lesions as GI-RADS 2 (0.9%), 118 lesions as GI-RADS 3 (54.9%), 86 lesions as GI-RADS 4 (40.0%) and 9 lesions as GI-RADS 5 (4.2%). For GI-RADS 4–5 lesions, the sensitivity, specificity, PPV, NPV, ACC and OR were as follows: 94.3, 72.2, 52.6, 97.5, 77.7%, and 43.3 (CI 12.0–146), respectively. The corresponding parameters resulting from combining the GI-RADS classification with the CA-125 marker were as follows: 66.0, 93.8, 77.8, 89.4, 87.0%, and 29.6 (CI 12.6–69.6), respectively, with p   30 IU/mL alone, the results were as follows: 70.0, 80.3, 53.8, 89.1, 77.7%, and 9.5 (4.6–19.6), respectively, with p   30 IU/mL and GIRADS 5). GI-RADS classification had good performance in discriminating ovarian Tumors. The additional measurement of CA-125 improves the system specificity, PPV and ACC for preoperative Adnexal Tumor assessment.

    Abstract:

    Endometrioid carcinoma is a common Tumor of the female genital tract, mainly affecting the uterus corpus and ovary. In the fallopian tube, endometrioid carcinoma is relatively uncommon. The female Adnexal Tumor of probable wolffian origin (FATWO), although occasionally seen elsewhere, is most typically encountered in the broad ligament. Endometrioid carcinoma of the fallopian tube resembling FATWO is a rare type of endometrioid carcinoma, and only 20 cases have been reported to date. Here, we report a case of endometrioid carcinoma of the fallopian tube resembling FATWO and review the literature. A 67-year-old woman presented with a history of prolapse for about one month outside the vulva. Physical examination and pelvic ultrasonography of the patient revealed a streak mass covering the left ovary. A total hysterectomy and a bilateral salpingo-oophorectomy were performed. The Tumor was confined to the tube and strongly resembled FATWO. Immunohistochemically, the Tumor cells were positive for AE1/AE3, epithelial membrane antigen (EMA), and p16, but negative for CD10, inhibin-alpha, calretinin, vimentin, myogenin, and desmin, CD99,CgA, and Syn. We review the previously published cases of endometrioid carcinomas of the fallopian tube resembling FATWO, and discuss the principal differential diagnosis of the Tumor in the fallopian tube.
