    Objective: To describe ventilator dynamics following the onset of ventricular fibrillation (VF) in an experimental swine model. Methods: Twelve female mixed-breed domestic swine (mean mass 21.3 2 1.7 kg) were sedated with IM ketamine (10 mag) and xylazine (1 mgntg), anesthetized with α-chloralose (40 mgkg loading dose, 10 mg/kg/hr rnaintenance infusion), incubated, and mechanically ventilated on room air. ECG, and aortic and pulmonary artery pressures were monitored continuously. VF was induced with a 3-sec, 60-Hz, 100-mA Tran thoracic shock, and left untreated for 8 minutes. Respiratory rate, tidal volume, and minute ventilation were recorded until respiratory activity ceased. Results: All 12 animals (100%) had Agonal Respirations through the first 2 minutes of arrest. This decreased to 11 (92%) at minute 3, five (42%) at minute 4, and two (17%) at minute 7. Mean respiratory rates ranged from 6 to 11 breathdmin. Mean tidal volumes ranged from 502 to 852 mL. Mean minute ventilations ranged from 3.3 to 5.8 L. Conclusion: In this swine model, 11 of 12 (92%) continued to have spontaneous Agonal Respirations for the first 3 minutes of VF cardiac arrest. Many animals had supernormal tidal volumes, and near-normal minute ventilations. These findings have potential implications for lay-rescuer and first-responder contributions to resuscitation of victims of out-of-hospital cardiac arrest.

    Abstract:

    Objective To explore the regularity of incidence of Agonal Respiration ( AR ) and Agonal Respiration frequency rate ( ARFR ) during untreated cardiac arrest ( CA ) after ventricular fibrillation ( VF ) in a swine model. Methods Ten healthy male domestic pigs weighing ( 25.0±3.0 ) kg were employed in this experiment. VF was induced by intraventricular shock with alternating current without treatment for 8 minutes. The incidence of AR and ARFR per minute were recorded for 8 minutes. Statistical analysis was performed using SPSS 19.0 system software. Results AR occurred in all animals after VF induced CA within 8 minutes. There was 1 animal showed AR at the first minute with ARFR ( 0.2±0.1 ) times/min, 4 animals showed AR at the second minute with ARFR ( 1.2±1.0 ) times/min, 7 animals showed AR at the third minute with ARFR ( 2.7±1.4 ) times/min, all animals showed AR at the fourth to fifth minute with ARFR ( 3.7±1.6 ) times/min and ( 3.2±1.9 ) times/min, 7 animals showed AR at the sixth minute with ARFR ( 1.3±1.0 ) times/min, no animal showed AR at the seventh minute, and 1 animal showed AR at the eighth minute with ARFR ( 0.2±0.1 ) times/min. The first and the last AR were observed at ( 2.02±0.84 ) minutes and ( 5.21±1.12 ) minutes respectively. Occurrence of AR reached its peak at the fourth to fifth minute, and it was absent at the seventh minute. ARFR after CA showed a crescendo-decrescendo pattern, which increased from ( 0.2±0.1 ) times/min to ( 3.7±1.6 ) times/min followed by a fall to ( 0.2±0.1 ) times/min. Conclusions AR is one of the symbolic signs after CA. AR occurred in all animals during untreated VF, and it reachesits peak at the fourth to fifth minute, with a crescendo-decrescendo pattern of ARFR. Effective identification and treatment in victim with AR timely can help to improve the success rate of cardiopulmonary resuscitation and survival rate.     Abstract:

    Abstract Objectives Bystanders cardiopulmonary resuscitation (CPR) increases survival in out-of-hospital cardiac arrest (OHCA). Emergency medical dispatchers (EMDs) can provide even totally inexperienced bystanders with instructions by telephone on how to resuscitate victims (T-CPR) until the emergency medical services (EMS) arrive. Agonal Respiration makes it difficult for EMDs to identify cardiac arrests (CAs) which will prevent or delay initiation of T-CPR. The aim of this investigation was to study if tuition of EMDs can improve their ability to identify Agonal Respiration in OHCA to allow for more frequent offers of T-CPR. Methods An observational study was made in 2004 and subsequently, a repeat study was made in 2006. All OHCA ( n  = 315 in 2004, n  = 255 in 2006) in the Stockholm region reported to the Swedish Cardiac Arrest Register were included and all corresponding EMS reports were reviewed. Emergency calls were recorded during the event. Witnessed cases of OHCA ( n  = 76 in both 2004 and 2006) were analyzed using a structured data collection tool. Results The frequency of offered T-CPR to all bystanders of OHCA in 2004 was 47%. After special tuition on Agonal Respiration in OHCA it rose to 68% in 2006 ( p  = 0.01). An even more marked rise was observed in OHCA cases with Agonal Respiration. In 2004 T-CPR was offered in 23% of these situations whereas the corresponding figures in 2006 had risen to 56% ( p  = 0.006). Conclusions Teaching EMDs to understand and recognize bystander descriptions of Agonal Respiration in patients with OHCA has resulted in a significant increase in offers of T-CPR in these situations.
