    Abstract:

    BACKGROUND Driving under influence of Alcohol is very likely to lead to traffic accidents. Nowadays, fewer people are driving under influence of Alcohol, probably because of law enforcement and information campaigns: however, the number of serious offenders (Alcohol Blood Level > 1,3 per thousand upon arrest) and recidivists is on the increase. These serious offenders and recidivists are obliged to submit to an examination by a psychiatrist. AIM To determine the characteristics of the serious offenders and recidivists who have to be examined by a psychiatrist and to improve current preventive measures. METHOD By studying the files of 101 offenders, we had access to personal characteristics, circumstances of the arrest, Alcohol anamnesis, laboratory research results, and to physical and psychiatric research and results. By means of descriptive statistics we were able to identify characteristics of the group and, where possible, to compare these with the characteristics of the general population. RESULTS The group consisted mainly of men aged about 40 from the lower social classes who had been arrested for their deviant driving behaviour or for causing traffic accidents. Of these persons 92% were Alcohol abusers or were Alcohol dependent and, fairly often, had comorbid psychiatric problems as well. CONCLUSION Preventive measures can be further improved by an increase in social control, more rapid detection of deviant driving behaviour, appropriate and prompt treatment of Alcohol problems and comorbidity associated with such problems.

    There are factors changing kinetics of Alcohol turnover in comparison to widely accepted norm comprise e.g. effect of stomach contents on resorption of ethanol, activity of ethanol metabolizing enzyme systems, and hydration of organism (Blood volume). One of possible factors--effect of bleeding on Alcohol Blood Level--was studied in rats. Blood Alcohol Levels were significantly higher in a group of rats where bleeding is one hour after ethanol administration than in a group with bleeding before ethanol administration, and than in controls devoid of bleeding. That's question, this results may be applicable to man.

    Patients with obstructive sleep apnoea (OSA) are reported to have an increased risk of road traffic accidents. This study examines the nature of the impairment during simulated steering in patients with OSA, compared to normal subjects following either sleep deprivation or Alcohol ingestion. Twenty-six patients with OSA and 12 normal subjects, either deprived of one night's sleep or following Alcohol ingestion [mean (SD) Alcohol Blood Level 71.6 mg dl(-1) (19.6)], performed a simulated steering task for a total of 90 min. Performance was measured using the tendency to wander (SD), deterioration across the task, number of 'off-road' events and the reaction time to peripheral events. Control data for OSA, sleep deprivation and Alcohol were obtained following treatment with nasal continuous positive airway pressure (nCPAP), after a normal night of sleep, and following no Alcohol, respectively. Patients with untreated OSA, and sleep-deprived or Alcohol-intoxicated normal subjects performed significantly less well, compared to their respective controls (P
