    Abstract:

    Burning and/or stinging is one of the most common concerns expressed by patients using topical therapies for treatment of dermatologic disorders. Topical lactic acid preparations often are used to treat dry scaly skin. In this study, we compared the level of burning/stinging reported by participants with application of lactic acid cream 10% containing strontium versus Ammonium Lactate lotion 12% and cetearyl alcohol lotion. The mean rating of burning/stinging reported for lactic acid cream 10% with strontium and cetearyl alcohol lotion was lower than Ammonium Lactate lotion 12% (P<.0001). Based on the study results, lactic acid cream 10% with strontium causes less burning/stinging than Ammonium Lactate lotion 12%.

    Abstract:

    Consensus recommends a gradual reduction in the frequency or steroid potency of topical corticosteroids following clinical improvement in the treatment of psoriasis, although no established guidelines have been developed. The authors sought to evaluate a combination regimen in the treatment and maintenance of psoriasis. Patients with mild-to-moderate psoriasis were enrolled (n=55) in a randomized, double-blind, placebo-controlled study using Ammonium Lactate lotion and halobetasol ointment. Those with initial improvement of target plaques after two weeks of combination treatment twice daily were randomized to a maintenance phase (n=41). Patients applied Ammonium Lactate lotion twice daily everyday and either placebo ointment (n=20) or steroid ointment (n=21) twice daily on weekends only. Forty-one of 55 patients (74.6%) were rated as “clear” (0) or “almost clear” (1) after two weeks of combination treatment. In the maintenance phase, the probability of physician global assessment worsening at six weeks in the steroid group was only 10 percent while in the placebo group the probability rose to 75 percent (p<0.0001). The probability of physician global assessment worsening climbed to 100 percent by 14 weeks in the placebo group while only increasing to 29 percent in the steroid group (p<0.0001). Twelve patients at study termination still had not worsened. Worsening of the physician global assessment index was more likely (HR 7.8 [2.84, 21.43]) in the placebo group than in the steroid group (p<0.0001). No cutaneous side effects, such as steroid atrophy or irritation, were noted. Combination treatment effectively cleared plaque psoriasis initially, and Ammonium Lactate twice daily everyday with weekend-only applications of halobetasol ointment effectively sustained the initial improvement for a significantly longer period of time when compared with placebo without demonstrating any significant side effects, such as steroid atrophy.

    Scalp psoriasis is a common life-altering skin condition causing a great deal of distress. It significantly affects quality of life and is difficult to manage. Treatment can provide variable results, often impacting patient compliance with therapy. Salicylic acid is used as adjunctive therapy to other topical treatments because of its marked keratolytic effect. Its effectiveness as a monotherapy is not fully understood. An emollient foam formulation of 6% salicylic acid (Salkera) in an Ammonium Lactate vehicle has recently become available. Efficacy, tolerability and patient acceptability of salicylic acid 6% emollient foam were assessed in an open-label pilot study of 10 subjects with scalp psoriasis. All psoriasis severity parameters were reduced with a significant decrease in Psoriasis Scalp Severity Index (PSSI) score from 15.3 to 3.0 after four weeks of monotherapy (P<0.001). Sixty percent of subjects were either "completely cleared" or "almost cleared" of their psoriasis. No adverse events (AEs) were reported. All signs and symptom tolerability measures demonstrated statistically significant score decreases with the exception of oiliness severity and patient-reported burning tolerability. Salicylic acid 6% emollient foam provides a useful option in the treatment of psoriasis that is highly effective, well tolerated and acceptable to patients.

    Twenty subjects with healthy skin were treated with the following formulations for two weeks: drug-free W/O vehicle, 5% Ammonium Lactate (CAS 52003-58-4) in W/O vehicle, 5% urea (CAS 57-13-6) in W/O vehicle, 3% Ammonium Lactate + 3% urea in W/O vehicle, 5% Ammonium Lactate + 5% urea in W/O vehicle. These formulations were applied in randomized order to 6 test areas on the forearms; one area was left untreated. Repetitive washings were additionally performed in the second treatment week. TEWL, stratum corneum water content (corneometry-determined electrical capacitance), and cutaneous blood flow (laser Doppler) were measured at baseline, day 7, and day 14. 5% urea and both Ammonium Lactate/urea combinations produced significant stratum corneum hydration and improved stratum corneum barrier function; there were no significant differences between these three treatments.

  • Recovery of alkyl Lactate from Ammonium Lactate by an advanced precipitation process

    Abstract:

    Abstract The recovery of lactic acid from Ammonium Lactate solution is impossible by the conventional acidification process because of the high solubility and lack of precipitation of Ammonium sulfate. We found that only the addition of methanol to the solution during acidification of Ammonium Lactate was very effective in decreasing the solubility of the produced Ammonium sulfate. This product could then be separated from the solution by simple filtration. With this advanced acidification process, the lactic acid filtrate was transformed into methyl Lactate rapidly by an esterification reaction with methanol even at room temperature. The methyl Lactate produced from the fermentation broth of Ammonium Lactate by this technique could be separated by simple distillation without change in the optical purity of lactic acid. This approach is simple and cost effective because all processes can be carried out at room temperature without complicated equipment. It is also regenerative because the Ammonium sulfate byproduct can be utilized for production of sulfuric acid and ammonia.

    Fermentation-derived Ammonium Lactate was converted into ethyl Lactate by decomposition in various organic solvents followed by esterification with ethanol over Amberlyst catalyst. The Ammonium Lactate was decomposed more efficiently in an organic solvent with high boiling point, where the produced lactic acid was stabilized well as a monomer without oligomerization. However, only the nonreactive phosphate-type solvent such as triethyl phosphate and tributyl phosphate showed a notable ethyl Lactate yield in the subsequent esterification reaction compared with dimethyl sulfoxide and N-methyl pyrolidine. The lactic acid yield in Ammonium Lactate decomposition and the subsequent ethyl Lactate yield were also highly dependent on solvent ratio to Ammonium Lactate, temperature and pressure in Ammonium Lactate decomposition reaction. The amino acid impurity contained in the fermented Ammonium Lactate as well as the unreacted Ammonium Lactate reduced the acid strength of Amberlyst-36, which resulted in the final ethyl Lactate yield.

    Abstract Ethyl Lactate was synthesized selectively from aqueous Ammonium Lactate by coupling solvent extraction with esterification with ethanol. When Ammonium Lactate reacted directly with ethanol without solvent, the ethyl Lactate yield was very low due to significant formation of byproducts such as oligomer and lactamide (Conv. 70%, Sel. 10% at 10 h). The significant improvement in ethyl Lactate yield was obtained when lactic acid was extracted from Ammonium Lactate in tributyl phosphate and then it reacted with ethanol, where oligomer formation was decreased greatly (Conv. 78%, Sel. 95% at 10 h). Compared with the traditional method including acidulation of calcium Lactate, extraction, and esterification, the present technique is potentially more economical since it enhances not only the productivity of lactic acid fermentation but also the efficiency of overall process from fermentation broth to ethyl Lactate.
