Hepatitis B, C, and HIV have all been documented to occur among Anabolic Steroid injectors (ASIs), most likely from the sharing of injection equipment. A survey was administered to 42 needle exchange programs (NEPs) from 17 states in the US to determine ASI participation. Sixty percent of the NEPs surveyed reported having at least some ASIs as participants, however, only 512 ASIs were identified among the 36 000 total monthly participants (1.4%). With the expanding number of NEPs in the US, it is possible to reach a higher proportion of ASIs with clean syringes and education, thus offering the means to prevent the spread of infection in this population.

Management of Anabolic Steroid-Induced Infertility: Novel Strategies for Fertility Maintenance and Recovery

LipshultzAbstract:There is often inherent conflict in the overlapping fields of male fertility and andrology. While the goal of all male fertility specialists is to facilitate and preserve biologic paternity, many practitioners also care for a significant number of patients suffering from hypogonadism. Exogenous testosterone administration, the gold standard for the management of these patients, almost universally impairs spermatogenesis and can even completely eradicate it in some men. With steady increases in both the incidence of hypogonadism and average paternal age, practitioners are now encountering hypogonadal men who desire future fertility or men suffering the effects of earlier androgenic Anabolic Steroid use with increasing frequency. In this manuscript, we review management strategies for these complex patients and explore novel medications that may be of use in this population.

Anabolic Steroid induced hypogonadism diagnosis and treatment

Design Review of published literature and expert opinions. Intended as a meta-analysis, but no quality studies met the inclusion criteria. Setting Not applicable. Patient(s) Men seeking treatment for symptomatic hypogonadism who have used nonprescribed AAS. Intervention(s) History and physical examination followed by medical intervention if necessary. Main Outcome Measures(s) Serum testosterone and gonadotropin levels, symptoms, and fertility restoration. Result(s) Symptomatic hypogonadism is a potential consequence of AAS use and may depend on dose, duration, and type of AAS used. Complete endocrine and metabolic assessment should be conducted. Management strategies for Anabolic Steroid–associated hypogonadism (ASIH) include judicious use of testosterone replacement therapy, hCG, and selective estrogen receptor modulators. Conclusion(s) Although complications of AAS use are variable and patient specific, they can be successfully managed. Treatment of ASIH depends on the type and duration of AAS use. Specific details regarding a patient's AAS cycle are important in medical management.

Anabolic Steroid induced hypogonadism in young men

LipshultzAbstract:Purpose: The use of Anabolic androgenic Steroids has not been traditionally discussed in mainstream medicine. With the increased diagnosis of hypogonadism a heterogeneous population of men is now being evaluated. In this larger patient population the existence of Anabolic Steroid induced hypogonadism, whether transient or permanent, should now be considered.Materials and Methods: We performed an initial retrospective database analysis of all 6,033 patients who sought treatment for hypogonadism from 2005 to 2010. An anonymous survey was subsequently distributed in 2012 to established patients undergoing testosterone replacement therapy.Results: Profound hypogonadism, defined as testosterone 50 ng/dl or less, was identified in 97 men (1.6%) in the large retrospective cohort initially reviewed. The most common etiology was prior Anabolic androgenic Steroid exposure, which was identified in 42 men (43%). Anabolic Steroid use strength training and multiple drug use among adolescents in the united states

This study examined the relationships between Anabolic-Steroid use and the use of other drugs, sports participation, strength training, and school performance among a nationally representative sample of US high school students. Design. Randomized survey data from the 1991 Centers for Disease Control and Prevention Youth Risk Behavior Survey. Setting. Public and private schools in the 50 United States and District of Columbia. Patients. A total of 12 272 9th through 12th grade students. Main Outcome Measured. Prevalence of AnabolicSteroid use. Results. The frequency of Anabolic-Steroid use was significantly associated with the frequency of use of cocaine, the use of other drugs such as amphetamines and heroin, tobacco smoking, and alcohol use. The weighted prevalences of Anabolic-Steroid use were higher among male (4.08%) than female students (1.2%). Students living in the South (3.46%) reported higher prevalences than students in the Midwest (3.0%), West (2.02%), or Northeast (1.71%). Students with self-perceived below-average academic performances (5.10%) and students reporting injected drug use also reported higher Anabolic-Steroid use (51.57%). Based on a multiple logistic regression, the following variables were found to be significant predictors of Anabolic-Steroid use: injectable drug use (odds ratio [OR], 17.86), use of other drugs (OR, 4.19), male gender (OR, 2.79), alcohol use (OR, 1.38), and strength training (OR, 1.73). The variables that were significantly associated with Anabolic-Steroid use varied by gender and by region of the country. Conclusion. These data suggest that adolescent Anabolic-Steroid users in this country are more likely to engage in strength training, injected drug use, and the use of multiple drugs, even after controlling for sports participation and poorer academic performance. These data confirm previous findings of an association between multiple drug use and Anabolic-Steroid use. Also, engaging in strength-training exercises continued to be associated with Anabolic-Steroid use after controlling for drug use and other predictors.
