En patient i Addisonkris är i behov av omfattande omvårdnad och noga monitorering. För att ge patienten de bästa förutsättningarna för att överleva är det av yttersta vikt att förhindra, upptäcka och åtgärda eventuella komplikationer som kan tillstöta i sjukdomsförloppet. Djursjukskötaren har således en betydelsefull roll i vården av dessa patienter. Syftet med detta examensarbete i djuromvårdnad var att undersöka vilken omvårdnad och monitorering som ges till hundar i Addisonkris under vistelsen på en intensivvårdsavdelning på tre stora svenska djursjukhus. Detta med syftet att undersöka vilka parametrar inom respektive område som en djursjukskötare behöver vara särskilt observant på vid sjukdomstillståndet. En intervjustudie utfördes med tre erfarna djursjukskötare som är verksamma på olika intensivvårdsavdelningar. En litteraturstudie där vetenskapliga artiklar samt facklitteratur söktes inom ämnet användes som ett komplement till studien. Det sammanställda resultatet från intervjustudien och litteraturstudien visade att monitorering av cirkulation, blodtryck och vätskestatus var av särskild vikt vid tillståndet. Resultatet av intervjuerna visade att djursjukskötarna inte upplevde att en patient i Addisonkris har några specifika, utmärkande omvårdnadsbehov, utan att ett holistiskt perspektiv för individuell omvårdnad används. Det holistiska synsättet omfattade generella omvårdnadsbehov som patientkomfort, hygien, nutrition, smärtlindring samt vätsketerapi och skötsel av infartskatetrar. Konklusionen av denna studie är att det föreligger en brist på forskning inom ämnet omvårdnad av hundar i Addisonkris. Majoriteten av de studier som finns behandlar patofysiologi, diagnostisering samt den medicinska behandlingen av sjukdomstillståndet. Airy wound healing : a study about a new type of wound coverage

Veterinären har till uppgift att avgöra vilken hantering som är lämpligast för det aktuella såret. I de fall där det är möjligt brukar en primär sårläkning vara att föredra då såret läker både snabbare och med mindre ärrvävnad än vid sekundär sårläkning. Om såret är kontaminerat eller om sårkanterna inte når ihop för att sutureras blir sekundär sårläkning det enda alternativet. Ofta behandlas en sekundär sårläkning genom ett förband som täcker såret för att skydda samt helt eller delvis stänga in såret i en kontrollerad miljö. För att välja vilken typ av förband som bör användas är det viktigt att ha kunskap om och känna igen sårets olika läkningsfaser. Faserna beter sig olika och kräver olika miljöer för att läkning ska ske optimalt. En och samma typ av förband kan således inte användas på alla sår eller genom sårets alla faser. Vanliga komplikationer under sårläkning är infektioner eller bildandet av exuberant granulation tissue (EGT) vilka gör att såren riskerar att inte kunna läka. Sår på distala extremiteterna som bandageras utvecklar både biofilm och EGT i större utsträckning än obandagerade sår. Ett sår som hålls fuktigt läker snabbare och med bättre epitelisering. En ny typ av förband (Rheva) undersöktes med avseende på genomsläpplighet av smuts och fukt. Förbandet, vilket har formen av en kupol med perforeringar, låter såret luftas samtidigt som det påstås behålla en fuktig sårmiljö och skydda mot kontamination. Förbanden placerades på både klippt och oklippt päls på intakt hud så inga sår gjordes på hästarna. I varje förband placerades en kompress som före och efter varje försökstillfälle kontrollvägdes. I detta kandidatarbete inom djuromvårdnad har två studier utförts parallellt, en för att undersöka genomsläppligheten av smuts och en för att undersöka genomsläppligheten av fukt hos förbandet. I smutsstudien användes torra kompresser och i fuktstudien användes kompresser preparerade med vatten. Resultatet visade att samtliga kompresser hade en liten ökad vikt efter försökstillfället jämfört med innan de placerades i förbandet eller preparerades med vatten. Denna viktökning kan antydas bero på smuts men kan även bero på annat. Kompresserna som preparerades med vatten återgick nästan helt till sin ursprungliga vikt och kan därför inte anses behålla någon fukt.One of the most common injuries in horses are different types of wounds. The veterinarian is responsible for determining which kind of management is most appropriate for the specific wound. When possible, primary wound healing is usually preferred as the wound heals both faster and with less formation of scar tissue compare with secondary wound healing. If the wound is contaminated or if the edges of the wound is too far from each other to be sutured, secondary wound healing becomes the only option. A secondary wound healing is often handled with an occlusive or semiocclusive dressing to protect the wound and to create a controlled environment. In order to choose the right type of dressing it is important to have knowledge of the phases of wound healing. Different phases require different environment for optimal healing. There is not one single type of wound coverage that can be used on all wounds or through al wound healing phases. Common factors that impair wound healing are infections or formation of exuberant granulation tissue, which can lead to prolonged or unsuccessful healing. Bandaged wounds on the distal extremities of the horse develop biofilm and exuberant granulation tissue to a greater extent compared with unbandaged wounds. Wounds that heal in a moist environment heal both faster and with better epithelialisation. A new type of dressing (Rheva) has been investigated with respect to the permeability of dirt and moisture. The dressing, which have the shape of a dome with perforations, allows air to pass through at the same time as it is claimed to maintain a moist environment and protect the wound against contamination. The dressings were placed on intact skin, both on cut and uncut coat. No wounds were created. In each dressing a compress was placed. Before and after every study period the compresses was weighed. In this thesis in Animal Nursing two parallel studies were conducted, one to investigate the permeability of dirt and one to investigate the permeability of moisture. For the study about dirt, dry compresses were placed in the dressing. For the study about moisture, the compresses were prepared with water before application in the dressings. The results showed a slight increase in the weight of the compresses after the study period compared to before they were placed in the dressings or prepared with water. This increase in weight can be suggested to be due to dirt but could also have other explanations. The compresses prepared with water returned almost completely to their original weight and the dressing can therefore not be considered to retain any moisture

Geriatriska katters särskilda omvårdnadsbehov

Däribland kattens. Kattens omvårdnadsbehov i geriatrisk ålder och nödvändigheten av eventuella anpassningar har studerats i endast en mindre omfattning, vilket är problematiskt då katter generellt anses dölja smärta och sjukdomssymptom. För att katter ska få en så god välfärd som möjligt under sitt allt längre liv är det viktigt att både djurhälsopersonal och kattägare utbildas angående anpassade åtgärder för geriatriska katter. Syftet med detta kandidatarbete inom djuromvårdnad var att undersöka specifika omvårdnadsbehov hos geriatriska katter och hur omvårdnad samt rådgivning till geriatriska katters ser ut på två svenska smådjurssjukhus. För att besvara frågeställningarna utfördes en litteraturstudie samt en intervjustudie med två legitimerade djursjukskötare vid två smådjurssjukhus i Sverige. Resultatet från intervjustudien visade att geriatriska katters särskilda omvårdnadsbehov inte tas i beaktning i lika stor utsträckning som exempelvis kattungars omvårdnadsbehov vid en stationärvårdsavdelning. Respondenterna hade inte tidigare reflekterat över just geriatriska katters omvårdnadsbehov och på arbetsplatserna saknades särskilda rutiner och standardåtgärder för omvårdnaden av geriatriska patienter. Då djursjukhusen fokuserade mycket på individanpassad omvårdnad upplevdes de därigenom ändå medvetna om de enskilda geriatriska patienternas behov och anpassade omvårdnaden därefter. I litteraturstudien hittades information om sjukdomar och hälsoproblem som äldre katter ofta drabbas av samt vilka anpassningar i hemmet som bör göras för att underlätta vardagen för den geriatriska individen. Detta hade även respondenterna i intervjustudien en del kunskap om och informationen om detta gavs i varierande omfattning som råd till ägarna. Överlag fanns det ingen standardiserad rutin på vare sig omvårdnad eller rådgivning gällande geriatriska katter på de två smådjurssjukhusen och respondenterna instämde i att en vetenskapsbaserad och enhetlig rutin för samtliga i personalen troligen hade förbättrat omvårdnaden. Sammanfattningsvis konstaterades att den geriatriska kattens omvårdnadsbehov är ett område som kräver fortsatt forskning. Djursjukvården skulle troligen gynnas av standardiserade protokoll för att öka medvetenheten hos både djurhälsopersonal och djurägare för den geriatriska kattens särskilda omvårdnadsbehov och för att säkerställa en likvärdig omvårdnad och rådgivning av dessa patienter.As a result of the constant increasing knowledge and competence in veterinary medicine as well as people’s growing commitment and emotional investment in their pets, our Animals now live longer. Including cats. The specific needs of the geriatric cat have only been investigated to a small extent, which is problematic since the cat is generally considered not to show signs of pain or disease. To be able to give the cat as good life quality as possible during its extended lifetime, it is important that both Animal health personnel and cat owners are educated about the specific needs of geriatric cats. The aim of this bachelor’s thesis in Animal Nursing was to investigate what specific needs cats develop in geriatric age as well as how they are being nursed and what kind of consulting cat owners get from two Swedish small Animal hospitals. A literature review and an interview study with two veterinary nurses from two small Animal hospitals in Sweden were conducted. The result from the interview study showed that the specific needs of geriatric cats are not taken into consideration to the same extent as for example the specific needs of kittens in a stationary care unit. The specific needs of geriatric cats were not an aspect that the respondents had reflected upon prior to the interview. However, the Animal hospitals focused on individualized care, and they appeared aware of the needs of the individual patients and adjusted the care thereafter. Information about diseases and health problems that geriatric cats often are affected by and how the home environment should be adjusted to facilitate for the cat were found in the literature. That was an area that the respondents in the interview study had some knowledge about and that was used in the consulting of pet owners in varying extent. Overall, a standardized routine about Nursing and consulting for geriatric cats at the two small Animal hospitals was lacking. The respondents did not consider this as a problem, since they always adjusted the Nursing to the needs of the individual regardless of its age. However, they agreed upon that the Nursing possibly would improve by a science based and unitary routine followed by all personnel. This thesis indicates that the specific Nursing needs of the geriatric cat require further investigation. It can also be noted that the Animal health care could benefit from standardized protocols to raise the awareness of both Animal health personnel and Animal owners of the specific Nursing requirements of the geriatric cat, and to ensure equal Nursing and counseling of geriatric patients
