Long trackways additionally permit the study of trackmaker foot anatomy, providing more insight on limb kinematics. In this paper we have restudied the extensive tracksite at Barranco de La Canal-1 (Lower Cretaceous, La Rioja, NW Spain) focussing on a 25-m-long dinosaur (ornithopod) trackway that was noted by an earlier study (Casanovas et al., 1995; Perez-Lorente, 2003) to display an irregular pace pattern. This asymmetric Gait has been quantified and photogrammetric models undertaken for each track, thus revealing distinct differences between the right and the left tracks, particularly in the relative position of the lateral digits II–IV with respect to the central digit III. Given that the substrate at this site is homogenous, the consistent repetition of the collected morphological data suggests that differences recorded between the right and the left tracks can be linked to the foot anatomy, but more interestingly, to an injury or pathology on left digit II. We suggest that the abnormal condition registered in digit II impression of the left pes can be linked to the statistically significant limping behaviour of the trackmaker. Furthermore, the abnormal condition registered did not affect the dinosaur's speed.

Background Identifying stress fractures of the hip can be a challenging differential diagnosis. Pain presentation is not always predictable and radiographs may not show the fracture, especially during its early stages. Hip stress fractures left untreated can displace and necessitate open reduction internal fixation or total hip arthroplasty. Diagnosis A 70-year-old woman presented to the physical therapy clinic with complaints of right hip pain. She had been evaluated by a physician and radiographs of the hip, which revealed some arthritic changes, were otherwise normal. Upon examination, the physical therapist observed an Antalgic Gait, a noncapsular pattern of limitation of hip motion, an empty painful end feel at the end range of motion (ROM) for hip abduction, external rotation, and flexion, and extreme tenderness to palpation over the anterior hip region. Restoring Functional Mobility For A Patient Following A Comminuted Patella Fracture Status Post Open Reduction Internal Fixation: A Case Report

A fracture of the patella typically requires surgery and immobilization, which can have detrimental impacts on functional mobility. The purpose of this case study was to document a rehabilitation program following a comminuted patella fracture status post open reduction internal fixation (ORIF).Case Description: The patient was a 62-year-old female who sustained a comminuted patella fracture following a traumatic fall down the stairs, and underwent ORIF to her right patella. She was prescribed a straight knee immobilizer for eight weeks leaving her unable to take daily walks, play with her grandchildren, and drive. Physical therapy (PT) interventions, which began four weeks post-operatively, were chosen based on deficits in range of motion (ROM) and strength, quadriceps atrophy, knee extensor lag, and an Antalgic Gait pattern. The interventions included a lower extremity (LE) strengthening program, ROM, manual therapy, and Gait training.Outcomes: The patient demonstrated gains in right knee flexion active ROM (48 ̊ to 120 ̊) and passive ROM (46 ̊ to 125 ̊). Strength gains were demonstrated through manual muscle testing of knee flexion (4-/5; full ROM against gravity, mild resistance) and knee extension (4/5; moderate resistance). Treatment continued beyond the time of publication, but progress notes suggested improvements in strength and ROM, despite persistent functional deficits.Discussion: The outcomes suggested a holistic LE strengthening program appeared to be successful in decreasing knee extensor lag, preparing the limb for ambulation without the immobilizer, and increasing functional mobility. Consistent with the literature, this patient had persistent functional deficits.
