Child sexual abuse (CSA) is a forced sexual behavior toward a child, either from the opposite or same sex. The types of child sexual abuse include exhibitionism, vouyerism, kissing, fondling, fellatio and cunnilingus, sexual intercourse, and pornography. The psychological effects of child sexual abuse often last a long time, in the form of anger, anxiety, nightmares, insecure, confused, scared, sad, and behavioral change from good to bad. The clinical effects of child sexual abuse ranges from bleeding in the genital and Anus, Fissure on Anus, enlargement of vaginal and anal openings, and thinning/damaged of the hymen in the vagina. The Indonesian law no. 290 mentions that sexual offender or perpetrator is to be jailed for at most 7 years. It is hoped this review can be used as a general reference for clinicians in obstetrics and gynecology as well as pediatrics, in diagnosing child sexual abuse. This diagnosis is important for helping the abused child to recover and as medical records for law enforcement. Key words: child sexual abuse, clinical effects, psychological effect

Anal Fissure - new concepts and controversies in the diagnosis and treatment Szczelina odbytu - najnowsze poglądy i kontrowersje na temat diagnostyki i leczenia
2013Co-Authors: Agnieszka Kucharczyk, Jacek BiercaAbstract:Summary Anal Fissure is one of the most common as well as widespread benign diseases of the Anus. Fissure development is initiated by a mechanical injury that is induced by hard faecal mass and remains unhealed if the blood flow in the tissue surrounding the Fissure is reduced and the relaxation of the internal anal sphincter is impaired. Conservative treatment is administered as the initial treatment for anal Fissures. In recent years, apart form the traditional treatment options, such as warm-water bath, ointments with nitro-glycerine or calcium-channel blockers, there have been reports of using new drugs for pharmacological sphincterotomy. They include bethanechol, which is a muscarinic receptor agonist, as well as sildenafil – a phosphodiesterase type 5 inhibitor which is a mild dilator of blood vessel smooth muscles. The authors describe a good therapeutic effect achieved with topical treatment with 7% sucralfate (sucrose sulphate-aluminium complex) ointment that is used for the treatment of peptic ulcers. Qualifications for the surgical treatment of anal Fissure have been made significantly more stringent. Lateral sphincterotomy is the gold standard. The article presents a review of literature on various modifications of lateral sphincterotomy (among other things, segmental internal lateral sphincterotomy was presented). The authors believe that the new operative techniques require due caution and their actual efficacy and possible functional complications such as impaired continence, may be assessed only after a few years of follow-up.

Clinical Features and Epidemiological Survey of Perianal Warts in 72 Males
Zhonghua nan ke xue = National journal of andrology, 2006Co-Authors: Xiu-lai Wang, Zhen-bang DongAbstract:OBJECTIVE To investigate the clinical features and epidemiological data of 72 cases of male perianal warts. METHODS Seventy-two cases of perianal warts in our clinic dated from June, 2004 to April, 2006 were enrolled in the study, whose clinical information and epidemiological data were collected and analyzed. RESULTS Perianal warts were most commonly seen in young and middle-aged men aged from 18 to 45, only 12.5% of whom had homosexual behaviors. Sauna was another predisposing factor of perianal warts in males in China (chi2 = 5.03, P < 0.05). Primary eruptions of the Anus and rectum, like perianal pruritus, eczema, Anus Fissure, and haemorrhoids, often impaired the local integrity of skin/mucosa. Classical condyloma acuminate was found in 61 (84.72%) of patients, who were susceptive to the infections of HPV 6/11, and were flat condylomas related to HPV16/18. Cryotherapy was believed to be one of the most efficient therapeutic choices for flat perianal warts. Suppression of cellular immune response was identified in the patients by comparison between the subgroups of peripheral T cells and the normal control. CONCLUSION Sauna is an essential predisposing factor of perianal warts in males, while Anus sexual intercourse is not the main route of HPV infection. Classical condylomata acuminate constitute the majority of the eruptions, and flat condylomata come next. The study also provides some useful data for understanding the clinical and epidemiological features of perianal warts in Chinese males for the sake of prevention and treatment of the disease.
