    Abstract:

    Aims In patients with myelomeningocele (MMC) and coexistent scoliosis, a spinal cord transection (SC-transection) is sometimes performed before scoliosis correction to prevent traction on the myelum after stretching the spinal column. Performing a SC-transection may have positive effects on Bladder Function, especially in case of refractory detrusor-sphincter dyssynergia. This study investigates the effects of SC-transection on lower and upper urinary tract outcomes. Methods All children with MMC who underwent scoliosis correction (1989–2009) were retrospectively reviewed. Cases were defined as those who underwent a SC-transection before scoliosis correction, whereas the control group comprised children who had a scoliosis correction alone. Urodynamic and clinical outcomes were examined. Results A total of 7 cases and 13 controls were identified. Postoperatively, compared to the control group, cases had relatively more often improvement of compliance (improvement in 6/7 vs. 9/13) and Bladder capacity (improvement in 6/7 vs. 8/13). No effect of SC-transection was found on incontinence severity, clean intermittent catheterization frequency, use of antimuscarinic drugs, or signs of renal damage on ultrasound. Conclusions SC-transection before scoliosis correction in children with MMC without lower extremity Function, may improve Bladder Function with respect to Bladder compliance and Bladder capacity. Changes in symptoms or renal ultrasound were not found. No harmful effects of SC-transection were found, indicating that this procedure can be performed safely with respect to Bladder Function in these patients. Whether or not SC-transection should be recommended during scoliosis correction in patients with MMC to improve Bladder Function requires further study. Neurourol.     Objective To measure the effects of nonhypercalcemic vitamin D receptor agonist elocalcitol on Bladder Function in rats with cyclophosphamide-induced cystitis and on Bladder Function and sensory nerve activity in a mouse with acetic acid-evoked Bladder irritation. Materials and Methods Female Wistar rats and male Balb/C mice were gavaged once daily with elocalcitol diluted in miglyol 812 (treatment group) or miglyol alone (control group). On experimental day 12, polyethylene tubing was implanted into the urinary Bladder in all the animals. In the mice, a bipolar electrode was positioned under a single postganglionic Bladder nerve. At 48 hours after surgery, Bladder Function was measured in awake, freely moving rats during Bladder filling with 0.9% NaCl and both Bladder Function and sensory nerve activity was measured in awake, restrained mice during continuous intravesical infusion of 0.9% NaCl followed by 0.25% acetic acid. Results In rats, the treatment group showed a significant increase in Bladder capacity and decrease in number of nonvoiding Bladder contractions. In mice, the filling pressure during saline infusion was similar in both groups; however, during acetic acid infusion, the average filling pressure was significantly increased (47%) in the control group but not in the elocalcitol treatment group. The firing rate at filling pressure for the treatment group was 3.6-fold and 2.7-fold lower than that in the control group during the saline and acetic acid infusion, respectively. Conclusion Oral treatment with elocalcitol suppressed signs of detrusor overactivity in both animal models and exerted strong suppressive effect on urinary Bladder sensory signaling during filling in mice.

    Suprasacral spinal cord injury induces changes in the mechanical and neuronal properties of the Bladder resulting in Bladder areflexia followed by Bladder-sphincter dyssynergia and detrusor muscle hypertrophy, which lead to urinary retention and increased Bladder size. These changes are most often quantified using highly skilled urodynamic techniques that involve catheterization. We investigated whether a hand-held digital ultrasound imaging system could monitor urinary retention in the Bladder following spinal cord injury in adult rats. Our findings indicate that contusive spinal cord injury resulted in high residual Bladder volumes that decreased and stabilized by 2 weeks post-injury but remained significantly higher than control Bladder volumes up to 46 days post-injury (the longest time point examined). Post hoc analysis indicated that the degree of Bladder Function recovery recorded at the end of the study correlated with the degree of Bladder Function recovery recorded at 6 days post-injury, indicating that Bladder Function recovery can be predicted by analyzing Bladder volume as early as 6 days post-injury. Bladder Function recovery correlated with locomotor recovery as assessed using the BBB locomotor rating scale. While providing a noninvasive assessment of Bladder Function with no detrimental impact on locomotor Function or assessment, this protocol provides researchers with a clinically relevant outcome measure for quantifying Bladder Function recovery after spinal cord injury or after experimental treatments for spinal cord injury.

    In the urinary Bladder, the capsaicin-gated ion channel TRPV1 is expressed both within afferent nerve terminals and within the epithelial cells that line the Bladder lumen. To determine the significance of this expression pattern, we analyzed Bladder Function in mice lacking TRPV1. Compared with wild-type littermates, trpv1−/− mice had a higher frequency of low-amplitude, non-voiding Bladder contractions. This alteration was accompanied by reductions in both spinal cord signaling and reflex voiding during Bladder filling (under anesthesia). In vitro, stretch-evoked ATP release and membrane capacitance changes were diminished in Bladders excised from trpv1−/− mice, as was hypoosmolality-evoked ATP release from cultured trpv1−/− urothelial cells. These findings indicate that TRPV1 participates in normal Bladder Function and is essential for normal mechanically evoked purinergic signaling by the urothelium.
