    Vascular angiotensin plays an important role in the long-term regulation of the Blood Vessel Function and structure. Angiotensin stimulates vascular smooth-muscle cell growth via the induction of protooncogene and autocrine growth factor gene expressions. In hypertension, atherosclerosis and restenosis, vascular angiotensin activity is increased and participates in the pathobiology of these vascular diseases. Experimental data demonstrate that angiotensin-converting enzyme inhibitors can prevent vascular hypertrophy of hypertension, attenuate atherosclerosis, and inhibit neointimal hyperplasia of restenosis. Taken together, the data show that pharmacologic blockade of the vascular renin-angiotensin system may result in vascular protection

    The pathogenic process of systemic sclerosis targets the skin and internal organs, and involves sequential or concomitant abnormalities in Blood Vessel Function, in immunity and inflammation, and in subsequent fibroblast activation. These characteristics are disease-specific and may partly explain the unresolved therapeutic strategies. These latter must take into account not only the broad biological abnormalities but also the complexities of the disturbances throughout the duration of the disease. However, recent epidemiological data have suggested a decrease in excess mortality, which may be mostly due to the use of cardiovascular drugs. Indeed, the prognoses of the kidney, heart and pulmonary vascular involvements have been improved. Nevertheless and despite several trials, pulmonary fibrosis remains a challenge with no clear efficacy of the regimen of immunosuppressors that have been investigated. This article deals with current and possible future therapeutic options.

These characteristics are disease specific and may partly explain the unresolved therapeutic strategies that must take into account not only these various biological abnormalities, but also the complexities of the various abnormalities throughout the duration of the disease. However, recent epidemiological data have revealed a decrease in excess mortality, which may be mostly due to the use of cardiovascular drugs. This article deals with present and possible future therapeutic options.

    Systemic sclerosis is the most severe of all connective tissue diseases. The distinctive pathogenic process involves sequential or concomitant abnormalities in Blood Vessel Function, immunity and, ultimately, fibroblast Function. These specific characteristics may explain the results of treatment evaluations. The decrease in excess mortality shown in recent studies seems chiefly ascribable to the use of cardiovascular drugs. Angiotensin-converting enzyme (ACE) inhibitors are effective in resolving renal crisis, prostacyclins and endothelin antagonists improve pulmonary hypertension, and calcium antagonists and ACE inhibitors benefit patients with myocardial involvement. On the other hand, immunomodulatory drugs and other agents investigated for their disease-modifying potential failed to influence skin fibrosis in controlled trials. Trials of immunosuppressants are ongoing. Available results indicate that emphasis should be put on cardiovascular drugs. The development of criteria for disease activity and severity would facilitate future research on the treatment of systemic sclerosis.

    Vascular permeability factor (VPF), also known as vascular endothelial growth factor (VEGF), is a potent polypeptide regulator of Blood Vessel Function. VPF promotes an array of responses in endothelium, including hyperpermeability, endothelial cell growth, angiogenesis, and enhanced glucose transport. VPF regulates the expression of tissue factor and the glucose transporter. All of the endothelial cell responses to VPF are evidently mediated by high affinity cell surface receptors. Thus, endothelial cells have a unique and specific spectrum of responses to VPF. Since each of the responses of endothelial cells to VPF are also elicited by agonists, such as bFGF, TNF, histamine and others, it remains a major challenge to determine how post-receptor signalling pathways maintain both specificity and redundancy in cellular responses to various agonists.

    The Blood Vessels in and around tumors can be distinguished from normal vasculature in several ways. Tumor-associated Vessels are derived from surrounding host Blood Vessels as a result of neovascularization (Folkman, 1985) and are typically more permeable than normal Vessels (Dvorak et al., 1988, Gerlowski and Jain,1983). These observations lead to the hypothesis that tumors can alter Blood Vessel Function by producing regulator substances. A polypeptide regulator known as vascular permeability factor (VPF) or vascular endothelial growth factor (VEGF) has now been purified and found to display a surprisingly wide spectrum of vascular activities, some of which are pro-inflammatory in nature. In addition to synthesis by tumor cells, VPF has now been found to be a product of monocytes and lymphocytes. Taken together, these results suggest that VPF may be a new inflammatory cytokine.
